    Notizen: Background: Monoclonal CD4 antibodies have been proposed as a new immunosuppressant drug in the treatment of inflammatory bowel disease. We report our experience of treatment with a monoclonal anti-CD4 (B-F5) antibody in severe refractory Crohn's disease. Methods: Twelve patients with severe refractory Crohn's disease were treated in an open clinical trial. B-F5 was given intravenously at a dose of 0.5 mg.day/kg for 7 consecutive days (patients 1–8). For patients 9–12, B-F5 was given at a dose of 0.5 mg.day/kg on the first day (day 0) and of 1 mg.day/kg on days 1–6. Follow-up examinations were carried out at days 8, 15, 22 and 30. Endoscopic evaluation was performed on days 0 and 30 in eight of 12 patients. Results: Immediately after the first infusion, one patient had dyspnoea and tachycardia requiring cessation of the treatment. Among the 11 patients who received the complete course of treatment, two had prolonged clinical improvement and two had partial clinical improvement. Significant endoscopic improvement was observed in only one patient. No sustained depletion of CD4+ cells could be observed. Conclusion: In this uncontrolled open trial, monoclonal anti-CD4 B-F5 antibody was not successful in severe Crohn's disease.

    Notizen: The modulatory effect of human heat-aggregated IgG on human B-cell differentiation induced by pokeweed mitogen was investigated with three experimental protocols. Pulse exposure to aggregated IgG, followed by extensive washings before culture, of peripheral blood mononuclear cell suspensions rigorously depleted of platelets and containing less than 4% monocytes resulted in a selective decrease of the numbers of IgG-containing cells and IgG-secreting cells, whereas a simultaneous decrease of the numbers of cells producing IgG and, to a lesser extent, of those producing IgM or IgA was observed when the pulsed suspensions contained platelets and more than 4% monocytes. This non-isotype-specific suppression was shown to be more pronounced when aggregated IgG and platelets were present in the cell suspensions throughout the cultures. The results suggest that two distinct suppressor pathways can be triggered by aggregated IgG. The first one is restricted to cells producing the matching isotype, in the absence of platelets, with few monocytes in the cell suspensions. The second one leads to a nonspecific suppression of the three major Ig classes. It requires the presence of platelets and/or a high percentage of monocytes and, although it remains to be demonstrated, is probably mediated by prostaglandin E2.

    Notizen: Epstein-Barr virus (EBV) infection of human B lymphocvtes involves a specific receptor closely associated with, or identical to, the C3d complement receptor, CR2, Thus. 25 out of 29 EBV-positive Burkilt's lymphuma (BL) cell lines but none of 15 EBV-negative BL lines were found to express C3 receptors. Furthermore, in vitro in association with EBV of six EBV-negative cell lines resulted in the expression of C3 receptors in association with that of EBV-determined nuclear antigen (EBNA). Rosette assays using erythrocytes coated with human C3b, C3bi, and C3d, inhibition of rosette formation with anti-receptor antibodies, and flow cytometry analysis of stained cells demonstrated that EBV-converted lines expressed C3b ami C3d receptors, CRl and CR2. Anti-receptor antibodies recognized an average of40,700 anti-CR1 and 140,000 anti-CR2 binding sites on an EBV-converted line(BL41/B95), whereas no specifie binding occurred on the corresponding EBV-negative (BL41) cells, Because CR1 and CR2 are involved in B-cell proliferation and/or differcntiation, enhanced expression of C3 receptors following the interaction between EBV and B celK and/or subsequent infection of the cells by EBV may provide a basis for positive control of B lymphocyte proliferation by EBV.

    Notizen: BL13, a mouse monoclonal IgGl antibody raised against human B cells, blocked the function of the C3d receptor (CR2) and bound with high affinity (5x 108 L M−1) to CR2 on B lymphoma cells. Following capping with the second antibody BL13 inhibited C3d-dependent rosette formation of Daudi and Raji cells and C3b-dependent CR2-mediated rosette formation with B lymphoma cells, but did not inhibit CRI-mediated rosettes between C3b-bearing cells and peripheral blood lymphocytes. Competitive binding experiments between biotinylated BL 13 or anti-CR2 antibody HB-5 and unlabelled antibodies demonstrated that BL13 bound to an epitope that is distinct from that recognized In HB-5, and closely associated with that recognized by monoclonal antibody ann-B2, BL13 only reacted with some B cells and follicular dendritic cells in germinal centres in human lymph nodes, whereas HB-5 strongly reacted with circulating B cells and bound to most cells in the follicles. These results demonstrate the heterogeneity of antigenically defined CR2.

    Notizen: The relative proportions of cells synthesizing the three major Ig classes or one of the four IgG subclasses in cultures stimulated with pokeweed mitogen (PWM) or Nocardia-delipidated cell mitogen (NDCM) were investigated. In cultures of human peripheral Mood mononuclear cells (PB MNC) stimulated with PWM, the number of IgG-comaining cells (CC) was higher than the number of IgM-CC, and a substantial number of IgA-CC was found. Conversely, in NDCM-stimulated PB MNC cultures IgM-CC outnumbered IgG-CC and only few IgA-CC were detected. In those cultures, the removal of T cells resulted in an increase in the number of IgM-CC concomitant with a decrease in the number of IgG-CC. A substantial number of cells containing simultaneously IgG or IgA in addition to IgM could be found in PWM-stimulated cultures. These cells were virtually absent in NDCM-stimulated cultures. The relative proportions of IgG subclass-CC were IgG1-CC〉IgG2-CC〉IgG3-CC≥IgG4-CC in PWM-stimulated and IgG2-CC〉IgG1-CC〉IgG3-CC≥IgG4-CC in NDCM-stimulated cultures. The removal of T cells from NDCM-stimulated cultures did not result in major alteration of this distribution. The role of T cells and of the genomic order of the Igh-C genes in their phenotypic expression triggered in vitro by PBA is discussed.

    Notizen: The binding of a 34-kDa (mol. wt.) acylpoly(1,3)galactoside (APG) extracted from a membrane proteoglycan of Klebsiella pneumoinae to human blood leucocytes was investigated. APG is made of a long polyl (1,3)galactose chain, a core-like region and a lipid moiety which comprises two glucosamine residues bound to a phosphate group and two βOH myristic acids, Fluoresceinated APG was shown to bind preferentially to monocytes and to a lesser extent to polymorphonuclear neutrophils, as determined by flow cytometry. Binding of fluoresceinated APG was inhibited by unlabelled APG; it was concentration dependent, but not saturable, with rapid kinetics. It occurred at +4°C but was markedly increased at 37°C. It involved trypsin-sensitive molecules on the membrane of monocytes. Neither the parent proteoglycan nor lipopolysaccharide from K. pneumoniae or Salmonella minnesota competed for APG binding. A minor non-specific binding to lymphocytes, occurring predominantly on B cells, was observed. Unlike that of lipopolysaccharide, the APG binding was not blocked by polymyxin B sulphate. Interaction between the galactose chain of APG and the galactose receptor does not account for the binding of APG to monocytes because the galactose receptor (Mac-2) is expressed at high density on activated macrophages but not on monocytes. Despite its strong binding to human blood monocytes, APG displayed a much weaker activity than K. pneumoniae membrane proteoglycan with respect to induction of monocyte cytokine synthesis. When administered as a Technetium 99 conjugate, APG was shown to label inflammatory foci in experimental animals, and its property as a marker of macrophages is currently being evaluated in clinical trials.

    Notizen: The effector cell(s) in human antibody-dependent cell cytotoxicity (ADCC). with antibody-coated chicken erythrocytcs as targets, was studied by comparison of cell suspensions from various lymphoid organs and by means of various cell fractionation methods. Effector cells (K) were found mostly in peripheral blood, spleen, and bone marrow but not in tonsils, lymph nodes, and thymus. Effector cells bear Fc receptors and can form EA rosettes with the antibody-coated target cells. About 1.5% peripheral blood lymphocytes can form ‘high-avidity’ EA rosettes with targets coated at low antiserum concentration. Most of the effector cells belong to this small subset, as shown by experiments of selective depletion. Removal of most monocytes, T cells, or B cells from, or addition of T-cell-specific antiserum to, the effector cell suspensions did not affect ADCC. Effector cells in this model of ADCC therefore lack the conventional B- or T-cell markers but at least some of them are likely to bear C3 receptors.

    Notizen: Cells bearing Fc receptors were studied using a rosette technique with chicken erythrocytes coated with rabbit antibodies (EA). By comparison with other markers and by selective depletion of T and B cells it was demonstrated that EA rosettes were formed by about 10% of the monocytes, 7% of the T lymphocytes, and 45% of the B cells. In addition, 26% of EA-rosette-forming cells (EA-RFC) in the peripheral blood carried no other markers than C3 receptors. Marked differences were found in the percentages of EA-RFC in cell suspensions from various lymphoid organs: peripheral blood, 15.2%; spleen, 26.6%; bone marrow, 30.9%; lymph nodes, 2.8%; thymus, 0.3%; and tonsils, 0.5%. It was concluded that Fc receptors were present on subsets of different lymphocyte sub-populations; their presence could not be regarded as a specific marker of one of these subpopulations.
