    Notizen: Summary Parathyroid hormone (PTH) metabolism in pregnancy has not been clearly defined. Studies have reported either increased or unchanged values of immunoreactive PTH (iPTH). However, iPTH levels do not necessarily correlate with hormonal bioactivity due to the presence of immunoreactive but nonbioactive PTH fragments. In this study we evaluated PTH metabolism in the third trimester of pregnancy by determining iPTH blood levels as well as the biological effects of PTH, assessed by tubular maximum phosphate reabsorption (TmP) and nephrogenous cAMP (ncAMP) excretion, in 10 young, healthy pregnant patients (mean gestational age 35 weeks) and 10 young, healthy age-matched female controls. Pregnancy was associated with a significant increase in creatinine clearance (146±13 vs 106±9 ml/min,P〈0.01), and a significant decrease in total fasting serum calcium (8.4±0.1 vs 9.0±0.1 mg/dl,P〈0.001) and serum albumin (3.6±0.1 vs 4.2±0.1 g/dl,P〈0.001). There was no significant difference in iPTH (3.7±0.4 vs 4.3±0.5 µlEq/ml), serum phosphorus (3.6±0.1 vs 3.8±0.2 mg/dl), TmP (3.61±0.13 vs 3.75±0.25 mg/100 ml GFR), or ncAMP (1.68±0.20 vs 1.88±0.23 nmoles/100 ml GFR) between the two groups. Pregnancy was attended by a significant increase in fasting urinary calcium to creatinine ratio (0.14±0.03 vs 0.06±0.01,P〈0.05), an index of bone resorption. The data suggest that the biological effects of PTH are unchanged in pregnancy, and that reported increments in 1,25(OH)2 vitamin D in pregnancy are not regulated by changes in either PTH, calcium, or phosphate.

    Notizen: Summary We have previously demonstrated that ethanol has a direct toxic effect on the rat skeleton characterized by decreased trabecular bone volume. In the present study, we examined the ultrastructure of the distal radial epiphyseal growth plates in these same animals. Eight weeks of ethanol administration to 12 male rats results in serum alcohol levels of 140 mg/dl but did not alter the width or light microscopic appearance of the radial growth plate. Quantitative electron microscopy failed to demonstrate morphologic evidence of toxicity in the skeletal cells. We conclude that although ethanol appears to have a direct effect on rat bone characterized by enhanced resorption, toxicity is not attended by ultrastructural changes in the skeletal cells.

    Notizen: Summary Osteoporosis has been reported to complicate androgen deficiency in males. Accordingly, we have evaluated an osteoporotic hypogonadal male with bone histomorphometry before and after 6 months of testosterone replacement. Androgen therapy resulted in increases in relative osteoid volum, total osteoid surface, linear extent of bone formation, and bone mineralization. The dramatic histological response to hormonal replacement confirms the importance of androgens in bone modeling and remodeling.

    Notizen: Summary Lithium treatment of humans and animals has been associated with adverse effects on bone and mineral metabolism. In order to determine whether lithium was altering the skeletal response to parathyroid hormone, we incubated bone rudiments for 5 days in the presence or absence of the drugs. Lithium had no effect on either parathyroid hormone-induced cyclic AMP generation or45Ca release from the bone rudiments. The data are consistent with the hypothesis that the skeletal effects of lithium are not mediated via inhibition of the parathyroid hormone-adenyl cyclase-cyclic AMP system.

    Notizen: Summary The effect of the vitamin D metabolites 1,25 dihydroxyvitamin D (100 pg/ml) and 25-hydroxyvitamin D (30 ng/ml) on hepatic production of3H-25 hydroxyvitamin D was investigated using rachitic liver perfusions and homogenates. 1,25 dihydroxyvitamin D inhibited hepatic3H-25 hydroxyvitamin D production in the liver perfusion (3.6 ± 0.4 vs 2.0 ± 0.5 pmol/liver,P〈0.05) and in liver homogenates (11.9 ± 0.6 vs 10.1 ± 0.4 pmol/g liver protein/3 h,P〈0.02). Inhibition was time and dose dependent. 25-hydroxyvitamin D inhibited production in liver homogenates (11.9 ± 0.6 vs 9.2 ± 0.1 pmol/g liver protein/3 h,P〈0.05) but not in the intact liver (3.6 ± 0.4 vs 3.4 ± 0.5 pmol/liver). The data indicate that 1,25 dihydroxyvitamin D is able to feedback regulate the production of its precursor, 25-hydroxyvitamin D. Although 25-hydroxyvitamin D also inhibits its own production in liver homogenates, it failed to alter total production in the intact liver, suggesting that this metabolite may require immediate access to the vitamin D 25-hydroxylase, located on the microsomes and mitochondria, to induce inhibition.

    Notizen: Summary Classic (type I) renal tubular acidosis in children is attended by growth retardation and rickets, abnormalities that can be corrected by alkali therapy alone. We have employed the NH4Cl-treated rachitic chick as a model to investigate vitamin D metabolism in the acidotic state. NH4Cl ingestion for 96 h was associated with a rise in serum calcium, a significant decrease in blood pH (7.42+0.08 vs 7.30±0.08,P〈0.005), decreased [3H]1,25(OH)2D3 following [3H]25OHD D3 injections, and enhanced metabolic clearance of administered [3H]1,25(OH)2D3. The data collectively suggest that metabolic acidosis in the chick alters the production and degradation of 1,25(OH)2D3.

    Notizen: Recent studies indicate that the vitamin D hormone, 1α,25-Dohydroxyvitamin D3 exerts rapid effects (seconds to minutes) in a variety of cell types. These rapid nongenomic actions in osteoblasts include effects on membrance voltage-gated calcium chananels, phosphlipase C activity, and the sodium/dydrogen antiport. Since the rapid effects occur in osteoblasts that lack the neclear vitamin D receptor, it is postulated that the nongenomic responses to the hormone reflect interaction with a separate, membrane localized signalling system. Preliminary studies demonstrate the presence of a receptor on the membranes of osteoblasts that lack the neclear vitamin D. This membranes receptors recognizes 1 a, 25-dihyrooxyvitamin D3 and its inaction 1β epimer, but not 25-hydrovitamin D3. These rapid nongenomic actions generated by interaction with the membrane receptor modulate the effect of the hormone on gene transcription. Thus, the rapid nongenomic pathway may play a regulatory function in modulating the genomic pathways affected by 1 a 25-dihydroxyvitamin D3.

    Notizen: 1α,25-Dihydroxyvitamin D3 increases intracellular calcium in rat osteoblast-like cells that possess the classic receptor (ROS 17/2.8) as well as those that lack the classic receptor (ROS 24/1), indicating that a separate signalling system mediates this rapid nongenomic action. To determine the intracellular sites of this calcium increase, cytosolic and nuclear fluorescence (340 nm/380 nm ratio) were measured in Fura 2AM loaded ROS 17/2.8 cells using digital microscopy. Within 5 min, cytosolic fluorescence increased by 29% (P 〈 0.05) and nuclear fluorescence by 30% (P 〈 0.01) after exposure to 1α,25-dihydroxyvitamin D3 (20 nM). This effect was blocked by the inactive epimer 1β,25-dihydroxyvitamin D3. In an individual cell, cytosolic and nuclear fluorescence increased gradually after 1, 3, and 5 min exposure to vitamin D. Nuclei were then isolated from ROS 17/2.8 cells to directly measure the hormone's effect on nuclear calcium. The calcium content of Fura 2AM loaded nuclei was not affected by increasing the calcium concentration in the incubation buffer from 50 nM to 200 nM. After 5 min, 1α,25-dihydroxyvitamin D3, 20 nM, increased the calcium of isolated nuclei in medium containing 50 nM calcium and 200 nM calcium. 1β,25-dihydroxyvitamin D3, 20 nM, had no effect on nuclear calcium but blocked the 1α,25-dihydroxyvitamin D3 induced rise in the isolated nuclei. The results indicate that the nuclear membrane of the ROS 17/2.8 cells contain calcium permeability barriers and transport systems that are sensitive to and specific for 1α,25-dihydroxyvitamin D3.1α,25-Dihydroxyvitamin D3 rapidly increases nuclear calcium levels in both intact cells and isolated nuclei suggesting that rapid nongenomic activation of nuclear calcium may play a functional role in osteoblastic activity.

    Notizen: We have previously shown that one of the rapid nongenomic actions of 1α,25-dihydroxyvitamin D3 (1α,25-(OH)2D3), the increase in intracellular calcium (Ca2+), accompanies the increased osteocalcin (OC) mRNA steady-state levels in rat osteosarcoma cells. To determine the functional significance of the nongenomic actions, we have measured changes in intracellular Ca2+ as an indicator of the rapid effects and have assessed the effect of inhibition of the rapid increase in cellular Ca2+ by the inactive epimer, 1β,25-dihydroxyvitamin D3 (1β,25-(OH)2D3) on OC mRNA steady-state levels and transcription. 1β,25-dihydroxyvitamin D3 inhibited 1α,25-(OH)2D3 induced increases in intracellular Ca2+ and OC mRNA transcription at 1 hr and OC mRNA steady state levels at 3 hr. 1β,25-Dihydroxyvitamin D3 did not alter the binding of the vitamin D receptor complex to the vitamin D responsive element of the OC gene. The results demonstrate the functional importance of the rapid, nongenomic actions of 1α,25-(OH)2D3 in the genomic activation of the OC gene by the hormone in rat osteoblast-like cells, perhaps by modifying subtle structural and/or functional properties of the vitamin D-receptor DNA complex or by affecting other protein DNA interactions that support OC gene transcription. © 1992 Wiley-Liss, Inc.
