    Notes: Abstract Solitary kidneys in renal donors and patients who have undergone unilateral nephrectomy for malignant disease have been reported to undergo hyperfiltration injury. This study was undertaken to evaluate the somatic growth and development of followed-up patient after Wilms' tumor to evaluate their renal function and identify any evidence of injury in the remaining kidney. The growth and development of all the children was found to be normal, as was DTPA clearance. Microalbuminuria in 24-h urinary collections was detected in 84% of the patients, indicating evidence of hyperfiltration injury. This study highlights the need for close monitoring of the renal function of long-term follow-up patients after Wilms' tumor in addition to the routine monitoring for tumor recurrence.

    Notes: Abstract  Although there is voluminous literature describing various aspects of hydatid disease in children, little attention has been paid to the small group of patients whose symptoms result in atypical presentation. This article addresses this problem, describing the features in ten children aged from 2 to 12 years. The sites of involvement were within a choledochal cyst (1). the pelvic cavity (1), the spleen (1), and transverse mesocolon. Albendazole was efficacious in the treatment of one recurrent case, as well as in preventing recurrence.

    Notes: Abstract In the last 2 years, two newborns were found to have persistent hypoglycaemia due to nesidioblastosis. Both required more than 15 mg/kg/min IV glucose and had inappropriately high plasma insulin levels. Near-total pancreatectomy (NTP) with splenic conservation was curative in both, with negligible morbidity and no mortality. Prompt diagnosis and stringent control of plasma glucose with hourly monitoring in an intensive care unit, use of a central venous line, and oral diazoxide prevented subsequent neurological handicaps. A high index of suspicion for this rare disorder should be kept in mind in a chubby infant who is jittery, apathetic, and has seizures with hypoglycaemia. Medical management is required to confirm the nonketotic, hyperinsulinaemic hypoglycaemia, whereas NTP provides a long-term cure.

    Notes: Abstract Mucormycosis of the neonatal gut is a rare disease, and most cases have been fatal. Three such cases were seen in quick succession; their detailed clinical and pathological findings are discussed. Experience in managing the first two fatal cases was successfully utilized in the management of the third baby, with gratifying results. A high index of suspicion and characteristic distinguishing features from necrotizing enterocolitis can facilitate an early diagnosis followed by specific therapy.

    Notes: Abstract An unusual case of neonatal hydrometrocolpos is reported where uterovaginal distension with fluid had produced not only a midline abdominal mass, but also bilateral cystic gluteal swelling. Perineal extension of a hydrometrocolpos causing bilateral gluteal swelling has not been reported previously in the literature. Our patient also had multiple associated anomalies suggestive of a defect in mesodermal embryogenesis similar to that in the müllerian dysgenesis syndrome.

    Notes: Abstract Gastric teratoma (GT) comprises less than 1% of all teratomas in children. Though GT in the presence of immature neuroepethelial elements is regarded as malignant, the prognosis is excellent after complete excision of the tumor. Because of its rarity the world literature lacks a large study. Clinical experience with ten cases of GT is presented and discussed. Only one patient was female; the mean age at presentation was 3.2 months. Two cases were immature grade III GT; one of these had infiltrated the left lobe of the liver and the transverse colon while the other had metastasized to the regional lymph nodes and omentum. All the patients underwent complete excision. There were no deaths, and after a mean follow-up period of 4.2 years, all the patients had no recurrence and were healthy. Both the mature and immature types of GT have an excellent prognosis after complete excision of the tumor. Even when the immature type infiltrates surrounding structures, complete excision offers recurrence-free survival without requiring chemo- or radiotherapy.

    Notes: Abstract In India, due to the limited pediatric surgical services available, the majority of the patients with indirect inguinal hernia (IIH) are managed by general surgeons. Between January 1986 and June 1992, at a tertiary care level center, 392 children with inguinal hernia were seen from 0–12 years of age (4.5% of all pediatric surgical procedures). Sixty-five percent of these presented when below 5 years of age. The majority of these (376, 96%) were boys. There were 212 (54%) right sided, 150 (38.3%) left sided and 30 (7.7%) bilateral hernias. Eighteen children (4.6%) presented with incarceration, and 6 of these required intestinal resections. The results following surgical repair were excellent. Contralateral exploration was reserved for babies under 6 months of age and girls (intersex disorders excluded). In a non-teaching hospital in Delhi inguinal hernias constituted 12.5% of all surgical procedures. Most of the children were over 1 year of age and only the symptomatic inguinal hernias were operated upon, without a policy for contralateral exploration. Due to the high risk of incarceration in premature babies, a careful search for the presence of inguinal hernia is recommended.

    Notes: Abstract The results of treatment of 341 consecutive cases of esophageal atresia/tracheoesophageal fistula over an 11-year period have been analyzed to determine the factors resulting in a poorer prognosis of these cases in a developing country; 121 neonates in the first 6-year period are compared with 220 in the last 5 years. In the latter group only 8% were in Waterston's group A; 46% reached the hospital within 24 h of birth, 13% were normothermic on presentation, 70% had a chest infection, and 28% had major associated malformations. This was similar to the preceding 6-year period. In the last 5 years the overall mortality was 58%, compared to 67% in the preceding 6 years. Despite improvement in survival in the last 5 years, the overall survival is far inferior to that reported from the developed countries. The factors resulting in poorer results have been analyzed and remedial actions suggested.

    Notes: Abstract A new variant of esophageal atresia (EA) with tracheoesophageal fistula (TEF) associated with duodenal atresia is reported. The TEF was between the lower pouch and the trachea, with a cystic dilatation in the midportion. The tracheal end of the fistula was obstructed by a membranous septum at both ends of a cystic dilatation, leading to a diagnosis of pure EA (gasless abdomen). After the lower pouch was opened beyond the cystic dilatation, 100 ml nonbilious fluid was obtained. A laparotomy revealed a type III atresia of the first part of the duodenum.
