    Notes: Abstract The effect of temperature on CH4 production, turnover of dissolved H2, and enrichment of H2-utilizing anaerobic bacteria was studied in anoxic paddy soil and sediment of Lake Constance. When anoxic paddy soil was incubated under an atmosphere of H2/CO2, rates of CH4 production increased 25°C, but decreased at temperatures lower than 20°C. Chloroform completely inhibited methano-genesis in anoxic paddy soil and lake sediment, but did not or only partially inhibit the turnover of dissolved H2, especially at low incubation temperatures. Cultures with H2 as energy source resulted in the enrichment of chemolithotrophic homoacetogenic bacteria whenever incubation temperatures were lower than 20°C.     Description / Table of Contents: Summary Introduction: Drug therapy plays often an important role in the treatment of chronic pain. Former studies and also experiences of clinical practice have given evidence, that insufficient patient compliance is a widespread event. Therefore drug screening becomes a significant tool for quality assurance. In the following the methodical and practical aspects of drug screening are described and recommendations for its use in clinical practice are suggested. Objective of drug screening: This tool can be used to check the intake of prescribed drugs, to detect hidden taken drugs, to decide whether side effects result of the prescribed medication or not and to support a withdrawal therapy. Practical basics and interpretation of the findings: Drug screening presupposes an informed patient, who consents to the investigation. Otherwise confidence is risked and the handling with unexpected results is not possible any more. Testing of urine has the advantage of a non invasive procedure and makes use of the reservoir of the bladder. A close co-operation and good communication between therapist and laboratory prevents misinterpretations, which might result from inaccurate formulated questions or methodical limitations, for instance because of cross reacting substances or a too insensitive detection level for the respective drug. The finding must primary bee used to value the therapy and improve the compliance but can not prove the patient himself. Conclusions: Even if the value of drug screening is not prospectively investigated until now, clinical experiences reveals it is as powerful tool for monitoring effectiveness and side effects of pharmacotherapy of pain and also of patient compliance which the prescribed drug regimes.
    Notes: Zusammenfassung Einführung: Die Pharmakotherapie hat einen hohen Stellenwert bei der Therapie chronischer Schmerzen. Klinische Untersuchungen zeigen allerdings, daß verordnete Medikamente häufig nicht korrekt eingenommen werden; dies bestätigten eigene Erfahrungen in der schmerztherapeutischen Praxis durch z.T. sehr überraschende Befunde eines Medikamenten-Screenings im Urin der Patienten. Derartige Urinuntersuchungen bieten somit ein Instrument zur Qualitätssicherung, dessen Grundlagen und praktische Anwendung in einem Überblick dargestellt werden. Ziele eines Medikamenten-Screenings: Das Screening soll die Umsetzung einer Therapie kontrollieren, die Bewertung von Nebenwirkungen ermöglichen, nicht verordnete Beimedikation aufdecken sowie Entzugstherapien sichern. Praktische Durchführung und Interpretation der Befunde: Um das Vertrauensverhältnis nicht zu gefährden, darf ein Medikamenten-Screening nur nach Aufklärung und mit Zustimmung des Patienten erfolgen. Die Untersuchung von Urin bietet sich als gering invasives Verfahren und wegen der Reservoirfunktion des Urins vorrangig an. Bei der Formulierung der Anforderungen und Interpretation der Befunde ist ein enger Kontakt zwischen Labor und dem Kliniker notwendig, damit – wie beispielhaft verdeutlicht wird – Fehlinterpretationen durch Kreuzreaktionen verschiedener Substanzen oder zu hohe Nachweisgrenzen vermieden werden. Die erhobenen Befunde sollten in erster Linie der Gütekontrolle der Therapie und nicht einer Bewertung des Patienten dienen. Folgerungen: Auch wenn der Stellenwert von Urinuntersuchungen oder anderer Verfahren eines Medikamenten-Screenings bisher nicht durch prospektive Untersuchungen belegbar ist, erscheint es als wichtiges Werkzeug in der Therapie chronischer Schmerzen, mit dem jeder Therapeut vertraut sein sollte.

    Notes: Abstract Plasma membrane vesicles were isolated from rabbit kidney outer medulla and employed in sodium, rubidium, and chloride flux studies. Chloride dependence and bumetanide sensitivity of (part of) the sodium and rubidium flux indicate that this plasma membrane fraction can be used to study the properties of Na−K−2Cl cotransport system present in the luminal membrane of the medullary thick ascending limb. The anion specificity of the cotransporter was investigated by determining the effect of anion replacement on sodium fluxes. When chloride was completely replaced by bromide, iodide, nitrate, or thiocyanate only bromide could effectively substitute for chloride (90% activity), whereas sodium uptake in the presence of iodide, nitrate, and thiocyanate amounted to only 25% of the sodium uptake observed in the presence of chloride. When similar replacement experiments were performed in the presence of 10 mmol/l chloride, bromide could substitute for chloride by 110%, iodide and nitrate by 60%, and thiocyanate by 70%. In the presence of 10 mmol/l bromide iodide, nitrate, and thiocyanate were similarly effective. The effect of nitrate and chloride on sodium flux was additive. Bumetanide-sensitive chloride uptake was inhibited by nitrate, the inhibition was however only partly, amounting to 60%. The results obtained are compatible with the view that the two anion binding sites of the Na−K−2Cl cotransporter can exhibit a different substrate specificity and that the transporter in addition to a 2Cl mode can also operate in a 2Br, Cl−, A− and Br−, A− mode, A− representing iodide, nitrate, or thiocyanate.

    Description / Table of Contents: Zusammenfassung Mit der kontinuierlichen lumbalen Liquordruckmessung wurde bei 10 Patienten mit supratentoriellen Hirntumoren und klinischen Zeichen intrakranieller Drucksteigerung über 24 Std die Wirkung hoher Dosen Dexamethason (80 mg) auf den Liquordruck (CSFP) untersucht. Während es bei der Kontrollgruppe (unbehandelte Patienten) im Laufe der Registrierung zu einem weiteren leichten CSFP-Anstieg bei unverändertem Druckmuster kam, wurde bei 7 der behandelten Patienten innerhalb 6 Std nach initialer intravenöser Dexamethasongabe eine deutliche CSFP-Senkung beobachtet. Bei 4 der behandelten Patienten wurde zum Ende der Registrierung eine eindeutige, bei 5 eine nur mäßige CSFP-Senkung gemessen. 1 Patient wies einen leichten Druckanstieg auf. Bei allen behandelten Patienten nahmen die Plateauwellen innerhalb der ersten Stunden an Höhe, Häufigkeit und Dauer ab, Druckniveauschwankungen und Liquordruckamplituden verringerten sich.
    Notes: Summary The effect of large doses of dexamethasone (80 mg) on the cerebrospinal fluid pressure (CSFP) was investigated with continuous lumbar CSFP measurement over 24h in 10 patients with supratentorial cerebral tumors and clinical signs of elevated intracranial pressure. There was a further slight rise in CSFP with an unchanged pressure pattern in the course of observation of the untreated control group. A marked lowering of CSFP was observed within 6h after initial intravenous administration of dexamethasone to seven of the treated patients. In four of the treated patients an unequivocal and in five only a moderate CSFP lowering was measured at the end of the registration. One patient had a slight rise in pressure. The plateau waves of all the patients treated decreased in height, frequency and duration within the first hours, while fluctuations in pressure level and CSFP amplitude were reduced.
