    Notes: Summary The decoupling of bone formation and bone resorption causes an insidious bone loss that is responsible for the negative skeletal balance in the frequent form of low turnover osteoporosis. The reduction of bone formation can hardly be verified by clinical methods. Osteocalcin, a non-collagenous bone protein, has proved to be a useful new indicator of bone formation. To establish its predictable value, plasma levels of osteocalcin were compared to conventional serological data of bone turnover and to histomorphometric parameters of iliac crest trabecular bone. In cases of osteoporosis with normal bone turnover activity (as confirmed by histomorphometry) no differences were observed in any of our laboratory data including osteocalcin. However, there was a significant lower mean serum level of osteocalcin in a group of patients with histomorphometrically proven low turnover osteoporosis in comparison to those with normal bone turnover. Serum levels of osteocalcin below 2.0 ng/ml seem to indicate a low turnover in the individual case of osteoporosis while this is unlikely when serum levels above 6.0 ng/ml are measured (according to our RIA).

    Notes: Summary The present study analyzed the location and number of the diaphysial nutrient foramina in six long bones of adult human skeletons of unknown age and sex from a statistical view point. The diaphysial nutrient foramina in the humerus, are located at between 50 and 65 % of the total length; in the radius and ulna, at between 25 and 50 %; in the femur, at between 25 and 58 %; in the tibia, at between 30 and 40 %; and in the fibula at between 35 and 67 %, i.e. the middle third of the bone. Also studied were the anatomical position and number of the diaphysial nutrient foramina in each bone.

    Notes: Abstract The periosteal sunburst reaction is an unusual response to metastatic bone tumours. Fifty such cases are reported in the literature, and 20 additional cases are reported in this study. The most common primary tumour showing this response is prostatic carcinoma, and the majority of cases are associated with osteoblastic metastases. Twenty percent of sunburst metastases occur in bones distal to the elbow and knee, a much higher prevalence than is found overall in bone metastases.

    Notes: Abstract In young spontaneously hypertensive rats (SHR) and in normotensive Wistar-Kyoto controls (WKY) several parameters of phosphate and calcium homeostasis were determined. At 6 and 8 weeks, blood analysis revealed a significant hypophosphatemia (p〈0.001) in SHR and twice as high plasma calcitonin levels in SHR than in WKY controls. At 8 weeks, 1,25-dihydroxycholecalciferol concentration was 20% higher in SHR (p〈0.02) while 25-hydroxycholecalciferol was unaltered (p〈0.51). In addition total immunoreactive PTH, iPTH, was slightly increased (p〈0.07) but intact PTH (1–84) (p〉0.90) was not significantly different from age matched WKY controls. Also at 8 weeks, a slightly reduced serum ionized Ca2+ concentration (p〈0.001) with no change in total serum calcium was found in SHR (p〉0.39). Balance studies at 6 and 8 weeks of age revealed no significantly different balances for phosphate (F=2.5,p〉0.10) and for calcium (F=2.6,p〉0.09), although a tendency for slightly more positive balaces existed in SHR when compared to WKY. However, SHR excreted significantly less phosphate in the urine than WKY control (F=0.2,p〈0.0009). Bone analysis was performed on femora of SHR and WKY of 6 weeks of age. Femora were significantly shorter in SHR (20.54±0.35 vs. 21.50±0.05 mm in WKY), whereas bone dry weight (127±6 vs. 107±2mg), bone ash weight (79±4 vs. 66±1 mg) and bone volume (0.196±0.007 vs. 0.165±0.004 cm3) were significantly greater in SHR. Calcium content per femur (717±35 vs. 617±11 μmol Ca/femur) and phosphate content per femur (512±23 vs. 447±8 μmol P/femur) were also significantly higher in SHR. It is discussed that the disturbances in phosphate homeostasis may be secondary to the strikingly increased plasma calcitonin levels present in young SHR.

    Notes: Summary A retrospective multifactorial analysis on 261 previously untreated patients with Ewing's sarcoma (Es) of bone has been carried out in order to ascertain the prognostic value of several histological variables on survival. Among those cases accepted as Es, 208 (80% of the patients) were considered to be “typical Es”, while 40 (15%) displayed a large cell predominance, being subclassified as “atypical large cell Es”. Furthermore, 13 patients (5%) possessed tumours of endothelial-like appearance. Eleven cases which displayed a mixed histological configuration were finally included within one of the three previous groups according to their predominant histological pattern. After adjustment for therapeutic regimens and initial location of the tumour, only two histological characteristics remain significant; i.e. the presence of necrosis (p=0.002) and, to a lesser degree, the presence of filagree “en damier” pattern (p=0.08), both of which are of poor prognostic value. From this study, it can be assumed that the morphological (and possibly histogenetical) heterogeneity of Es of bone has no prognostic influence on survival.

    Notes: Summary Previous studies have shown that osteoclasts obtained from chopped bones resorb surrogate calcified tissue substrata in vitro. These cultures contained all bone and marrow cell type pooled together. We have now parted the marrow from the bone and cultured the cells from the two fractions separately: on both resorbable substrates and on plastic in order to test their longevity in culture and ability to resorb following trypsinisation. Marrow-fraction, bone-fraction or whole bone derived cells were harvested from prehatch chick long bone shafts after removing the periosteum; seeded on sperm whale dentine (SWD) slices or plastic dishes and cultured continously, or trypsinised and reseeded on to fresh substrata at weekly or half-weekly intervals. Observations were made by light microscopy and SEM. Many multinucleate cells were observed in the marrow fraction immediately after settling, deriving presumably from poorly adherent osteoclasts, next to bone, which had not been resorbing at the time of harvesting. By three days in culture on plastic, multinucleate cells were very large both in terms of plant extent and nuclear number: cell fusion occurred between osteoclasts and between osteoclasts and small, round uninuclear cells. SWD was extensively resorbed. The adherence of the osteoclasts was greater (a) to plasuc upon trypsinisation than that of the other cells; and (b) to SWD than to plastic, particularly if the cells were resorbing. Trypsinised cells regained their resorptive capacity after seeding on to new SWD, but only for 1 or 2 treatments. Bone derived cells were similar to the marrow cultures, except for the much higher proportion of other bone cell types. Trypsinisation caused a higher proportional loss of multinucleate cells from both SWD and plastic. Resorption was still occurring at 6 weeks in all cultures. A wide diversity existed in the shapes, depths, plan areas and volumes of the resorption pits.

    Notes: Summary The scanning electron microscope (SEM) has been used to study the three-dimensional organisation of collagen in slices of human rib and femur which were “etched” by chick osteoclasts, mechanically isolated and grown on their surfaces in vitro. Collagen organisation in the two bones showed a spectrum of appearances, ranging from lamellae of approximately equal thickness, but alternating fibre orientations, to an almost exclusive orientation of collagen apparently in a longitudinal direction. The rib contained a smaller component of transversely oriented collagen which may be related to a different functional loading. The thickness of circumferential lamellae was less than that of osteonal lamellae in the two adult ribs examined. Also, in the rib there was a trend towards increased average lamellar thickness with age in the range studied. This may be related to the fact that more of the lamellae in the rib cortex in children have been formed circumferentially. Correlation of results obtained with the SEM and the polarised light microscope (PLM) from the same substratum demonstrated that the latter grossly exaggerated the apparent component of collagen with a transverse orientation. This will always be true unless sections comparable with the lamellar thickness are used with the PLM.

    Description / Table of Contents: Zusammenfassung Bei offenen Frakturen des Unterschenkels tritt eine posttraumatische Osteitis im Durchschnitt in ca. 12% der Fälle auf. Dabei kann Sequestrierung großer Tibiaanteile zu erheblichen Knochenverlusten führen, die angesichts der prekären Weichteilsituation am Unterschenkel erhebliche Probleme bieten. Zum Wiederaufbau und Herstellung der Statik ist die autologe Transplantation cortico-spongiöser Späne und Spongiosa aus dem Beckenbereich die Methode der Wahl. Voraussetzung zur Einheilung des Transplantates ist die Schaffung eines optimalen Transplantatlagers mit der Möglichkeit allseitiger Gefäßinvasion ins Transplantat. Durch autochthone Muskellappenplastiken sowie mikrovasculäre Transplantation von myocutanen Lappen läßt sich das Transplantatlager konditionieren. Mit ihrem separaten Gefäßanschluß an den Blutkreislauf wird humoralen und zellulären Antikörpern der Zutritt zum Transplantat erlaubt. 54 Weichteildefekte am Unterschenkel wurden 41mal mit Muskel- und 13mal mit mikrovasculären Lappen gedeckt. 21mal wurde neben der autologen Spongiosaplastik ein corticospongiöser Span fest verankert. Zur Lappennekrose bzw. Teilnekrose kam es in 12 Fällen, die über Sekundärmaßnahmen zur Deckung und Heilung führten. Durch die Spongiosaplastik und kombinierte Weichteildeckung mit Muskulatur oder mikrovasculären Lappen wurde das therapeutische Ziel der Schaffung einer belastungsfähigen Extremität erreicht.
    Notes: Summary About 12% of compound fractures of the lower leg give rise to posttraumatic osteitis. Loss of bone by sequestration causes many problems particularly if soft tissue quality is poor. Cancellous and corticocancellous bone from the iliac crest is used for bony reconstruction. To obtain bony union, a well vascularised recipient for the graft is essential. Local muscle flaps and distant microvascular flaps and distant microvascular flaps improve vascularity by their separate connection to the vascular system. We treated 54 soft tissue defects by local muscle flaps in 41 and by microvascular flaps in 13 cases. In 21 cases a cancellous or a corticocancellous bone graft was used. Necrosis or partial necrosis of the flap occurred in 12 cases. Skin closure and healing was obtained by secondary procedures. Combined muscle flap and bone grafting was found to allow full weight bearing within reasonable period of time.

    Notes: Technetium-99m methylene diphosphonate ([99mTc]MDP) is the most widely used bone-scanning agent today. In order to determine the precise bone locus of 99mTc corresponding to the delayed bone scan image, and to test the potential clinical use of this agent in enhancing the information obtained by bone scan, we employed [99mTc]MDP in this microautoradiographic study of normal bone and bone repairative tissue. Four white rabbits underwent operations in which two 1.5-mm drill holes were created in the subtrochanteric regions of both of their femora. An additional four white rabbits underwent sham operations of their femora, in which neither drilling nor periosteal injury occurred. Two rabbits were controls and did not undergo operation. After 7 days, the first two groups of rabbits were injected with [99mTc]MDP and bone scanned 2 h later. After the scans were completed, all three groups of animals were killed and their femora histologically processed for microautoradiography and routine histopathology. In the two groups that were injected with [99mTc]MDP, all bones showed the isotope to be clearly localized along mineralization fronts. The isotope was occasionally found in the substance of the osteoid, but was absent from the cytoplasm and nuclei of osteoblasts and osteocytes. Osteocytic lacunae showed the presence of isotope at their borders, but no identifiable pattern of isotope uptake was noted. Neither osteoclasts nor Howships lacunae showed isotope uptake. The animals that received drill holes showed corresponding areas of uptake by bone scan, whereas those that did not receive drill holes showed no bone scan activity at these sites. The histological findings were qualitatively similar in the bones from both groups of animals, but were quantitatively greater in the bones undergoing repair. The uninjected animals showed no positive chemography on their slides, indicating that the histological fixing and embedding solutions did not introduce artifacts into the technique. Skeletal scintigraphy has been shown to be a valuable clinical tool in the assessment of pathological conditions of bone. Through the use of microautoradiography, a correlation between the information gained by a bone scan and that of a bone biopsy or excised bony lesion can be made. This may aid in the assessment of the biological behavior of tumors or other pathological conditions of bone, as well as the precision of the bone scan in defining or localizing an osseous lesion.

    Notes: A nonimmobilized rat tibial fracture model of endochondral osseous repair was examined for the unique localizations of specific collagen genetic types. At various stages of the healing process, the demineralized callus was reacted with immunofluorescent antibodies directed against the type specific forms of matrix collagen. Type III collagen rapidly appeared (day 8-10) and remained in the primitive mesenchymal callus until remodeled. It was particularly prominent in the highly vasoformative regions and the pericallus encapsulation but not present in preexisting cortical and neoformed lamellar bone. The type II collagen, a marker of cartilage, was uniquely located only in areas of chondroid differentiation and calcification. Type II collagen was absnet from all bone and was not identified beneath the repairing intact periosteum. The differentiating chondrocytes synthesized type II collagen on an underlayer of type III collagen already within the mesenchymal matrix. From these studies of genetically unique collagen markers, it appears that only in areas of motion or anoxia does an intermediate of chondroid tissue appear. The utilization of specific type II and type III collagen immunofluorescent antibodies has facilitated the understanding of the fracture repair process and has acted as an indicator for unique matrix components.

    Notes: Geometric, elastic, and structural properties of growing rat femora were determined from bending and torsion tests followed by bone sectioning and measurement of areal properties. Rosette strain gages bonded to the bone surface measured the strain during testing. A computer generated elliptical cross-sectional representation of the cross section geometry was used for calculation of material and structural properties. All structural and material properties increased with increasing age, exhibiting age-related changes that were best represented by an allometric or “heterauxic” growth pattern (y = axb) up to maturity. The femoral axial, flexural, and torsional rigidity increased 5.7, 10.1, and 14.8 fold, respectively, during maturation from 21 to 119 days of age. The increase in whole bone rigidity during maturation was caused primarily by changes in geometry. The bone tissue tensile longitudinal elastic modulus and shear modulus approximately doubled, and the shear strength increased approximately fourfold over this same period. Following maturity, a much slower increase in bending and torsional properties was noted. The results suggest that bone structural properties are regulated by changes in both geometric and material properties.
