Zussammenfassung Eine prospektive klinische Studie ergab, dass als früheste Organstörung nach schwerem Trauma ein Lungenversagen zwischen dem 3. und 5. Tag entsteht (Anstieg des Lungenwassers). Mediatoren wie Elastase, Complement (C3a) und Thromboxan verlaufen biphasisch (initialer und zweiter Peak). Die Granulocyten sind anfangs metabolisch maximal stimuliert (Chemiluminescenzsteigerung), in den Alveolen werden die Granulocyten erschöpft aufgefunden. In den Alveolen ist die Konzentration der Mediatoren extracellulär deutlich höher. Zur Therapie sind prinzipiell zwei Wege möglich: l. Inhibition der einzelnen Mediatoren. 2. Verhinderung des Granulocytensticking in der Lunge und damit Verhinderung von Freisetzung der Mediatoren und Durchwanderung der Granulocyten in die Alveole.

Summary A prospective clinical study showed that pulmonary failure between days 3 and 5 after the trauma is the earliest organ disturbance (increase of extravascular lung water). Mediators like elastase, complement (c3a), and thromboxane have a biphasic course (initial and secondary peaks). Initially, the metabolic response of granulocytes is stimulated the most (increase in chemiluminescence) in combination with intra-alveolar exhaustion. The extracellular concentration of mediators in the alveoli is significantly higher. Therapeutic considerations include (I) inhibition of mediators and (2) inhibition of granulocytes adhering to the lungs and, thereby, inhibition of mediator release and intra-alveolar granulocyte diapedesis.
