    Description / Table of Contents: Zusammenfassung Als Alternative zu der nur unbefriedigenden dorsalen Laminektomie bei Wirbelkörpermetastasen und drohendem Querschnitt bietet sich die Wirbelkörperausräumung vom ventrolateralen Zugang mit Verblockung durch Knochenzement und Seitenplatte an. Indikation: drohender Querschnitt, therapieresistenze Schmerzen, Wirbelsäuleninstabilität. Material und Resultate: bei allen 27 Fällen bestand sofort Schmerzfreiheit, die durchschnittliche Zeit der Gehfähigkeit bei den 21 komplikationslosen Fällen: 11,4 Monate. Beste Ergebnisse bei Thyreoidea-Mammacarcinomen und Myelomen.
    Notes: Summary Dorsal laminectomy has unsatisfactory results in many cases of metastases to vertebral bodies and threatened paraplegia. The method with ventrolateral approach, partial dissection of the vertebral body, and stabilization by means of bone cement on a plate presents an alternative form of treatment. Indications are threatened paraplegia, pain, and instability of the vertebral column. In a series of 27 patients, freedom from pain was experienced immediately in all cases; in the 21 cases with no complications the average period of walking ability was 11.4 months. The best results were obtained in patients with carcinoma of the thyroid or the breast and in those with myeloma.
