Zusammenfassung Bei Trümmerbrüchen des Ellbogens kommt es fast immer zur Stufenbildung in den Gelenkflächen. Diese stellt eine absolute Indikation zur Operation dar. Zusammen mit einer Sammelstatistik der Deutschen AO wurden 245 distale intraarticuläre Humerusfrakturen ausgewertet. Verfahrenswahl. Operation in Bauchlage des Patienten, dorsaler Zugang (60 %), Olecranonosteotomie (52 %), Rekonstruktion mit verschiedenen Implantaten. Bei schwerer Weichteilverletzung temporär Fixateur externe. Sofortige kontrollierte Krankengymnastik aus der Gipsschiene (70 %). Ergebnisse. 29,5 % sehr gut, 39,6 % gut, 19,4 mäßig, 11,5 % schlecht.
