Zusammenfassung Bei 14 Kindern wurde 1/2 Std vor Austauschbeginn 1 g/kg KG salzarmes Humanalbumin injiziert, um die Bilirubinausfuhr zu verbessern. Im Vergleich mit einer Kontrollgruppe von 17 Kindern, bei denen unter sonst gleichen Bedingungen die Blutaustauschtransfusion ohne Humanalbumin erfolgte, wurde kein signifikanter Unterschied in der Menge des entfernten Bilirubins festgestellt. Der fehlende Effekt des Humanalbumin wird darauf zurückgeführt, daß eine Spenderblutmenge von 250 bis 300 ml/kg KG angewandt wurde. Im Vergleich mit der Literatur konnte gezeigt werden, daß der Austauscheffekt, bezogen auf das aus dem kindlichen Kreislauf entfernte Bilirubin, bei einem Spenderblutvolumen von 250–300 ml/kg KG ebenso gut ist wie der Effekt, der entsteht durch Zusatz von Humanalbumin zu einer kleineren Spenderblutmenge.

Summary In order to improve elimination of bilirubin during exchange transfusion, 14 children got an injection of 1 g human albumin with a low salt content per kg body weight half an hour prior to the exchange transfusion. In comparison with a control group of 17 children who got exchange transfusions under equal conditions, but without human albumin, no significant difference in the amount of eliminated bilirubin could be found. The inefficiency of human albumin may be due to the large amount of donor blood used for each exchange transfusion (250–300 ml/kg body weight). As compared with the literature it could be shown that the effect of exchange transfusions, as far as the elimination of bilirubin is concerned, is as good in patients who got exchange transfusions with a donor blood volume of 250–300 ml/kg body weight, as in those cases who got a smaller amount of donor blood, but an additional injection of human albumin.
