    Notes: Summary The diffusion of oxyphenbutazone into synovial and cerebrospinal fluids and synovium and joint cartilage was investigated in 25 patients receiving short-term treatment. In the synovial fluid, the mean oxyphenbutazone concentration, was 57.1±13.4% of the plasma level, due to its excellent diffusion into the joint cavity. In synovial tissue, the oxyphenbutazone level was higher in patients with severe inflammation than in those with no or little inflammation. Penetration into joint cartilage is less than into synovial tissue. In cerebrospinal fluid the concentration was close to the level of free plasma oxyphenbutazone. The findings show increased diffusion of oxyphenbutazone towards its site of action in inflammation.

    Notes: Abstract The pharmacokinetics of cefroxadin have been studied after the administration of single oral and intravenous doses to healthy volunteers. Cefroxadin was assayed by HPLC. The kinetics in plasma following i.v. administration were described by using a three-compartment model. An additional disposition phase was observed following oral administration that could not be detected after the low i.v. dose. The terminal half-life was 1.03 h. The apparent volume of distribution at the steady state was consistent with a diffusion of the antibiotic in all extracellular fluids. The AUCafter oral administration was linearly related to the dose. The urinary excretion amounted to 95% of the dose with virtually complete absorption of orally administered drug.

    Notes: Abstract The DNA of Blue-Green Algae may be specifically and most efficiently elucidated by the combined use of the fluorochrome DAPI and DNAse treatment. InChroococcus turgidus andSynechococcus aeruginosus it forms a network which usually is positioned at the periphery of the centroplasm. At the beginning of cell division it seems to be invaginated by the ingrowing cell wall. Later it must be distributed by other means because two equal parts are connected by a number of fine and straight DNA threads. InOscillatoria limosa usually all parts of the centroplasm are interspersed with the DNA network. In surface view, the DNA occasionally seems to consist of a number of independant rods, but in reality these are connected to the general network. Division apparantly occurs by ingrowth of the crosswall and usually results in equal daughter nuclear equivalents although occasionally they are unequal. Four strains ofMicrocoleus vaginatus were found to have nuclear equivalents of the same general appearance as inOscillatoria limosa.

    Description / Table of Contents: Summary Legionella infections can take the clinical course of a relatively harmless respiratory infection. However, serious, atypical pneumonia is a more frequent manifestation of infection with these pathogens. As yet, six differentLegionella species can be identified;Legionella pneumophila appears to be the most common. Legionnaires' pneumonia is being found with increasing regularity during summer and autumn in elderly male patients with previous illnesses. The clinical picture is characterised by viral “prodrome”, high fever, a dry cough, breast pain, confusion, diarrhoea, haematuria, moderate leukocytosis with lymphopenia, low concentrations of sodium in the serum and negative results from microbiological analysis of the sputum and pleural exudate. Diagnosis is confirmed culturally, microscopically and serologically; the indirect immunofluorescence test is of particular value for this purpose. Erythromycin alone or in combination with rifampicin is the treatment of choice.
    Notes: Zusammenfassung Die Legionellosen können klinisch als relativ harmlose Atemwegsinfektion ablaufen, jedoch treten sie häufiger als schwere atypische Pneumonie in Erscheinung. Sechs unterschiedlicheLegionella-Spezies sind bisher bekannt, von denenLegionella pneumophila am häufigsten zu sein scheint. Die Legionärspneumonie tritt vermehrt bei männlichen älteren Patienten mit Vorerkrankungen bevorzugt im Sommer oder Herbst auf; die Klinik ist charakterisiert durch virale „Prodromi“, hohes Fieber, unproduktiven Reizhusten, Thoraxschmerzen, Verwirrtheit, Diarrhöen, Hämaturie, mäßige Leukozytose mit Lymphopenie, niedrigem Serum-Natrium und negativen mikrobiologischen Analysen von Sputum und Pleuraexsudat. Die Diagnosesicherung erfolgt kulturell, mikroskopisch und serologisch, wobei zur Zeit der indirekte Immunfluoreszenztest besonders hilfreich ist. Mittel der Wahl zur Behandlung sind Erythromycin alleine oder in Kombination mit Rifampicin.

    Notes: Abstract To determine whether the aminopyrine breath test reflects the presence of increased pericentral fibrosis, steatonecrosis, and cirrhosis following jejunoileal bypass, 21 patients were evaluated with liver biopsies, liver chemistry tests, 45-min bromosulfothalein retention tests, and aminopyrine breath tests prior to bypass and at 3, 6, and 12 months thereafter. Following bypass, 15 biopsies demonstrated increased pericentral fibrosis, steatonecrosis, or cirrhosis, and 35 biopsies revealed increased fatty infiltration alone. Although the aminopyrine breath test results were significantly lower in patients with increased pericentral fibrosis, steatonecrosis, and cirrhosis at 6 and 12 months following bypass (P〈0.05), this test had a positive predictability of only 0.67. An abnormal AST had a positive predictability of 0.87. Other liver function tests were less reliable in identifying patients with potentially progressive lesions following bypass. Thus, the AST is more useful than the aminopyrine breath test in reflecting increased pericentral fibrosis, steatonecrosis, and cirrhosis following jejunoileal bypass. The nonspecific effects of obesity and increased fatty infiltration of the liver following jejunoileal bypass may account for the low predictability of the aminopyrine breath test.
