Notes: Sixty-six patients, diagnosed as Japanese summer-type hypersensitivity pneumonitis at Osaka Prefectural Habikino hospital between 1973 and 1980, were studied. The diagnosis was based on the clinical features and summer-seasonal nature of the disease. The presence of an aetiological agent within patients' home environment was suggested by the recurrence of acute symptoms of high fever, cough and dyspnoea 5–8 hr after coming home from hospital, and by spontaneous improvement on leaving home. Immunological studies revealed the presence of anti-Cryptococcus antibody in sixty-four of sixty-five patients' sera, by indirect immunofluorescence against Cryptococcus neoformans, Precipitating antibody against culture supernatant protein-antigen of Cr. neoformans was detected in more than 80% of sera obtained from patients during the active stage of the disease. The positive result on inhalation provocation-challenge, using culture supernatant protein-antigen, suggested that Cr. neoformans or antigenically related Cryptococcus species may cause Japanese summer-type hypersensitivity pneumonitis.

Notes: Summary Ultrasonic measurement (0.333–200 MHz) of melanosomes isolated from B16 and Harding Passey (HP) mouse melanomas indicates that the partial wave resonance and principal relaxation of the 2 kinds of melanosomes are similar, but that their stochastic resonance is markedly different. The structure of the melanosomes appears basically amorphous, but linearly ordered and copolymeric in the molecular dimension of a segment composed of 5–6 zigzag units, which are packed closely in B16 and more openly in HP.

Notes: Summary Morphological alterations of oligodendroglia were investigated in the spinal cord of the twitcher mouse, an authentic murine model of human globoid cell leukodystrophy (GLD) from day 5 to day 45 postnatal (p.n.). Typical inclusions were seen in the perikarya as well as the processes of oligodendroglia after day 10 with increasing frequency. The majority of the inclusions was non-crystalloid but rather needle-like or slender tubular in appearance. Ultrastructural features of cellular degeneration became first noticeable on days 25–30 in the oligodendroglial cytoplasm. These consisted of an increased number of microtubules and/or smooth cisterns, dispersed ribosomes, alteration of endoplasmic reticulum forming stacked lamellae or whorles, vesiculation or vacuolation of cytoplasm. The number of degenerating oligodendroglia increased in the older twitcher mice, so did the degenerating myelin sheath. However, even on day 45, when globoid cells became conspicuous in subpial and perivascular regions, many oligodendroglia and myelin sheaths were still well preserved. These observations suggested that oligodendrogial degeneration resulted in the degeneration of myelin sheaths but globoid cells appeared even before morphological evidence of myelin degeneration, presumably in response to the biochemical alterations resulted from the deficiency of galactosylceramidase.

Notes: Summary A case of anterior sacral meningocele which was clearly visualized on whole body computerized tomography is presented. Posterior surgical approach with ligation of the narrow communication between the sacral dural sac and the meningocele alleviated the subjective complaints of a feeling of abdominal distention and of dysuria. For the purpose of postoperative follow-up study, examination by whole body computerized tomography proved to be the best method of evaluating the size of the meningocele and also the state of neighbouring structures.

Notes: Abstract Clinical features of ten Japanese children with cerebrovascular Moyamoya disease are reported and non-Japanese paediatric cases of the disease are reviewed from the world literature. The most common initial manifestations were headache in four cases (40%), motor deficit and convulsion in three cases (30%). As their recurrent and/or residual symptoms, eight children (80%) developed motor deficit consisting of hemiplegia in five cases and paresis or weakness of the extremities in three cases, and four (40%) had headaches. The mode of presentation in our cases was similar to that of non-Japanese cases, in addition to a female preponderance. Electroencephalographic findings of prominent high voltage delta bursts following hyperventilation and slowness of returning to the normal pattern, seen in all hyperventilated cases, is one of the features. Of 15 carotid arteries visualised in ten patients, sites of occlusion or stenosis were seen between the bifurcation of the posterior communicating artery and that of the anterior cerebral artery or the middle cerebral artery in 13 arteries, with a vascular network in the basal ganglia. This study suggests that if hyperventilation procedures produce prominent high voltage delta bursts during electroencephalography in children with headache and/or motor deficit, cerebrovascular disease, especially Moyamoya disease, should be suspected.

Notes: Summary A patient with haemorrhage in the right tegmentum of the pons demonstrated ipsilateral disturbance of taste on both the anterior two-thirds and posterior one-third of the tongue. The contralateral medial lemniscus was not disturbed, contrary to the early descriptions that the secondary gustatory neurons from the nucleus of the solitary tract cross and turn upwards in the pons through the contralateral medial lemniscus. This finding would accord with the concept of the pontine taste area which receives ipsilaterally the secondary gustatory neurons from the anterior pole of the nucleus of the solitary tract.

Notes: Summary Subplasmalemmal linear densities (Yajima et al. 1977 a) were the membrane specializations observed in globoid cells in globoid cell leukodystrophy (GLD) and in the cells of the mononuclear phagocytic system (Kawanami et al. 1980). In the spinal cord of the twitcher mouse, an authentic murine model of GLD, somewhat similar membrane specializations were noted in astrocytes, and on some occasions, a spot desmosome-like cellular contact was observed between globoid cells, which were likely to be mesodermal in origin, and astrocytes, which are of ectodermal origin. Possible significance of such apparent cellular contact is discussed briefly.

Notes: Summary Chronologic events of demyelination were investigated in the spinal cord of the twicher mouse, an authentic murine model of human globoid cell leukodystrophy (GLD) from 5 to 45 days postnatal. There was very little evidence of myelin degeneration before day 25 although clustered or scattered globoid cells were already noted in the dorsal columns and intramedullary portion of the ventral roots. Globoid cells contained typical cytoplasmic inclusions and in those which were found adjacent to degenerating myelin and naked axons, myelin debris were conspicuous in their cytoplasm. Vesiculation of myelin and a feature of globoid cells stripping myelin lamellae were noted in the area of demyelination. Myelin and oligodendroglial degeneration became pronounced throughout the spinal white matter after day 40 but globoid cells tended to be more concentrated in the dorsal columns. Our observations suggest that the emergence of globoid cells in GLD is in response to the changes in biochemical environment (i.e., excessive presence of galactosylceramide in the tissue?), and these cells appear to have a role as phagocytic cells in removing myelin lamellae.
