    Notes: A 59-year-old Japanese man with myasthenia gravis, who had a 10-year history of temperature-sensitive pain in the lower extremities, i.e. improved by cooling and worsened by warming, consulted us because the pain had become intolerable during the previous 4 months. Bilateral erythema, swelling and large ulcers were noted on the calves, dorsal aspects of the feet, and soles. Laboratory data showed thrombocythaemia and a positive antibody to the acetylcholine receptor, but were negative for antinuclear and antiphospholipid antibodies. A diagnosis of secondary erythermalgia was made because of the clinical features, the laboratory data, and the lack of family history of this disease. Although steroid pulse therapy, oral aspirin and antiserotonin drugs were ineffective, bilateral lumbar sympathetic ganglion block succeeded in relieving the severe pain and curing the ulcers. The clinical course in our patient suggests that sympathetic ganglion block may be one of the most effective treatments for secondary erythermalgia. Although the mechanism of this effect is uncertain, microcirculation disturbance in secondary erythermalgia, if any, may be improved by this block.

    Notes: A highly sensitive beam position monitor has been designed for use with synchrotron radiation sources. This beam position monitor consists of two gas-flow ionization chambers having three parallel plate conductors. The incident beam is divided by a central plate. Therefore, a displacement of the beam position is detected with the two outside ionization chambers. The position monitor was tested at the BL6Cl beamline of the Photon Factory (PF). The results show a good performance.

    Notes: Background Serum eosinophil cationic protein (ECP) levels reflect ongoing eosinophilic airway inflammation and are used as a marker for asthma activity. ECP levels, however, may not be elevated in some asthmatic patients, even when they are symptomatic.Objective To clarify the characteristics of patients with ‘low’ ECP titres despite asthma exacerbation.Methods Serum ECP levels were measured in 113 asthmatic patients during exacerbation. Patients were divided into two groups according to ECP titre: a high ECP group (H; ECP ≥ 16.0 µg/L) and a low ECP group (L; ECP 〈16.0 µg/L). Twenty-two patients who had recently received systemic steroids were excluded and the clinical features of the remaining patients in H (n = 54) and L (n = 37 were compared.Results Gender, atopic or smoking status, disease severity, inhaled steroid or theophylline usage, peak expiratory flow (% personal best) and forced expiratory volume in 1 s (FEV1) (% predicted) did not significantly differ between the two groups. Patients in L were significantly older and had longer disease duration and lower serum IgE levels than those in H. Multivariate analysis combining age, disease duration and IgE levels showed that age and disease duration were independently associated with ECP level. Airway wall thickness, assessed in a subset of patients using computed tomography, was significantly larger in L.Conclusion Serum ECP levels in asthmatic patients may not be elevated during exacerbation and thus may not be a useful marker in patients who are older, have longer disease duration or possibly have thicker airway walls. Mechanisms other than eosinophilic inflammation, such as airway remodelling, may be involved in asthma exacerbation in these patients.

    Notes: Abstract In order to investigate the interactions of poly(2-diethylaminoethyl methacrylate) (PDEAEMA) with methyl orange and its homologs in solution, temperature dependence of the complex formation has been examined in detail by the measurements of transmittance and specific conductance for the systems. Furthermore, the binding course of dyes to PDEAEMA has been studied on the basis of thermodynamic parameters obtained from equilibrium dialysis experiments at different temperatures. It was observed that the flocculation process shifted to lower dye concentrations in accordance with increasing hydrophobicity of the dyes in the order, methyl orange 〈 ethyl orange 〈 butyl orange, and the process of complex formation was characterized by three separate regions according to the slope of specific conductivity-mixing ratio curve for mixtures of PDEAEMA and dye. The temperature dependences of ΔF,ΔH and ΔS suggest that, for dyes-PDEAEMA complex formation, the hydrophobic interaction is predominant at a low temperature but the electrostatic interaction becomes important as the temperature increases.

    Notes: Abstract A rare case of primary squamous cell carcinoma of the endometrium, probably originating from adenomyosis, is presented. A 58-year-old Japanese woman was referred to Kitasato Institution Medical Center Hospital to explore the cause of her postmenopausal bleeding. The endometrial cytology specimen obtained with the Endocyte (an device for endometrial check up) showed cells suggesting well differentiated squamous cell carcinoma. The patient underwent modified radical hysterectomy, bilateral salpingo-oophorectomy, and pelvic lymphadenectomy, followed by postoperative radiation therapy. The dissected uterus revealed keratinizing-type squamous cell carcinoma localized in the fundal portion of the uterine body and invading deeply into the myometrium, coexistent with a large area of adenomyosis with a transitional area to carcinoma. The residual endometrium was atrophic. There was no atypical finding in the cervix. Human papilloma virus DNA was not detected. The carcinoma recurred 26 months after the surgery, and the patient died of the disease.

    Notes: Abstract The K+ channel in rat parotid gland acinar cells were investigated by ensemble current noise analysis in single isolated cells employing the giga-seal whole cell current recording mode. Sets of 20–40 identical de- and hyperpolarization voltage steps were applied and the resultant current records were processed by computer to obtain the mean and the variance of the current. The time-course of the mean current could be fitted by the sum of two exponentials, suggesting a 3-state model. The simplest plausible hypothesis is a model with one open and two closed states. Assuming this model, the relationship between the variance (σ2) and the mean current (I) could be fitted by the function σ2/I=i−I/N. The estimated single channeli/V-relations were similar to those taken from single channel current recordings, and the size of the population of channels per cell (N) was 76±26 (n=12). The validity of the model was tested by a successful simulation of the time-course of the variance.

    Notes: Abstract The interactions between poly(2-diethylaminoethyl methacrylate) (PDEAEMA) and orange II (OII) or ethyl orange (EO) have been examined in detail by measuring the transmittance and the specific conductance of the solutions. The mechanisms of flocculation and deflocculation have been investigated by changing the intrinsic viscosity, the degree of neutralization and alkyl groups of PDEAEMA or by adding NaCl, urea or alcohols. The complexation of PDEAEMA with dyes is completed at the same mole ratio but is not dependent on the molecular weight of PDEAEMA. OII molecules bind with PDEAEMA by electrostatic interactions, followed by flocculation through hydrogen bondings between hydroxyl groups on OII molecules bound. EO molecules first interact with PDEAEMA electrostatically in the same ways as OII, but flocculation occurs by hydrophobic interactions between ethyl groups on EO molecules bound. By further addition of dyes to the complex, OII or EO molecules already bound can interact with free OII or EO molecules newly added through hydrogen/hydrophobic interaction between OII or EO molecules. Deflocculation occurs by the electrostatic repulsion between anions of dyes bound, such as (PDEAEMA-OII)-hydrogen bonding-(OII-anions) or (PDEAEMA-EO)-hydrophobic bonding-(EO-anions) newly formed.

    Notes: Summary A 55-year-old woman with sarcoidosis developed hyperparathyroidism that was associated with fluctuating serum calcium levels. Ultrasonography and a computed tomography (CT) scan of the neck demonstrated a cystic lesion in the lower posterior area of the left thyroid lobe. Histological examination after subsequent surgery revealed sarcoid granulomas within a parathyroid adenoma. The pathogenesis of this rare association is discussed.
