    Description / Table of Contents: Summary From January 1997 to August 1998 all stabel and nonstabel trochanteric femoral fractures (n=72) were treated routinely by gamma nail using the correct operative technique. Patients showing additional osteoarthritis of the hip in radiographs hip replacement was performed by a cementless modular femoral hip prostheses from January 1996 to August 1998 (n=28). Follow up period was 6 to 18 months. Operation time and blood loss were higher using the prostheses. However complications and letality (〈5%) were not different during postoperativ course. In each group three operative technical complications occured. Using a modified Harris Hip Score (without range of motion, contractions) the score was decreased non significant comparing both groups first of all in unstabel fractures until follow up. In each group one revision (loosening of prostheses, excessive shortening of femoral neck) was necessary. Using the correct operative technique, the gamma nail prooved to be a save device with good outcome. Outcome using modular prostheses is comparable to gamma nail. Therefore the use of modular prostheses is justified in case of osteoarthrosis and in some cases of very unstable fracture.
    Notes: Zusammenfassung In der Zeit vom 1.1.1997 bis 1.8.1998 wurden alle stabilen und instabilen per- bis subtrochantären Femurfrakturen (n=72) routinemäßig durch einen Gammanagel (GN) unter Anwendung der korrekten Operationstechnik stabilisiert. Ausgenommen waren Patienten, die radiologisch eindeutige Koxarthrosezeichen hatten. In dieser Vergleichsgruppe (n=28) wurde der Gelenkersatz durch eine zementfreie, modulare Hüftprothese (MHP) durchgeführt und die Patienten im Zeitraum vom 1.1.96 bis 1.8.98 ebenfalls prospektiv erfaßt. Die Nachuntersuchung erfolgte nach 6–18 Monaten. Trotz längerer Operationszeit und höherem Blutverlust in der MHP Gruppe bestand kein Unterschied an Komplikationen und Letalität (〈5%) während des stationären Aufenthalts. In jeder Gruppe waren drei operationstechnische Komplikationen nachweisbar. Unter Verwendung eines modifizierten Harris-Hip-Scores (ohne Bewegungsumfang, Deformität) zeigten beide Gruppen eine nicht signifikant unterschiedliche Abnahme der Punktzahl vor allem bei den instabilen Frakturen zum Zeitpunkt der Nachuntersuchung. In jeder Gruppe mußte ein Patient (Sinterung im Frakturbereich, Prothesenlockerung) revidiert werden. Unter Anwendung der korrekten Operationstechnik ist der GN ein sicheres Implantat. Die MHP zeigt gleich gute Ergebnisse mit gleicher Letalität, so daß der Einsatz bei Koxarthrose und in ausgewählten Fällen auch bei hochgradig instabilen Frakturen gerechtfertigt ist.

    Description / Table of Contents: Zusammenfassung Bei Trümmerbrüchen des Ellbogens kommt es fast immer zur Stufenbildung in den Gelenkflächen. Diese stellt eine absolute Indikation zur Operation dar. Zusammen mit einer Sammelstatistik der Deutschen AO wurden 245 distale intraarticuläre Humerusfrakturen ausgewertet. Verfahrenswahl. Operation in Bauchlage des Patienten, dorsaler Zugang (60 %), Olecranonosteotomie (52 %), Rekonstruktion mit verschiedenen Implantaten. Bei schwerer Weichteilverletzung temporär Fixateur externe. Sofortige kontrollierte Krankengymnastik aus der Gipsschiene (70 %). Ergebnisse. 29,5 % sehr gut, 39,6 % gut, 19,4 mäßig, 11,5 % schlecht.
    Notes: Summary Comminuted fractures of the distal humerus require operative treatment. Only by anatomical reconstruction of the joint can good functional results be achieved. Intensive postoperative kinesitherapy is of great importance. In a comprehensive statistical study by the German section of AO International 245 patients were reexamined. The remaining disability can be controlled in two-thirds of the cases to such an extent that no alterations are necessary in private or professional life. Of the results, 29.5 % could be classified as very good, an additional 39.6 % as good. 19.4 % as moderate, and 11.5 % as poor.

    Description / Table of Contents: Zusammenfassung Bis auf wenige Ausnahmeindikationen führen wir bei Oberarmschaftbrüchen die von Sarmiento angegebene funktionelle Behandlung mittels Kunststoffoberarmmanschette durch. Nach der Reposition wird zur Abschwellung eine Oberarmgipsschiene für 5–10 Tage angelegt. Anschließend wird die Sarmientomanschette angepaßt und mit funktionellen Bewegungsübungen begonnen. Von 31 konsekutiven Humerusschaftfrakturen wurden 26 funktionell mit der Sarmientomanschette behandelt. Es kam in allen Fällen zu einer knöchernen Konsolidierung in 6–8 (in einem Fall in 11) Wochen ohne sekundäre Komplikationen. In zwei Drittel der Fälle kam es zu geringen Achsabweichungen. Das funktionelle Ergebnis war jedoch durchwegs gut.
    Notes: Summary With a few exceptions, we only treat fractures of the shaft of the humerus with a functional method consisting of a plastic sleeve, as indicated by Sarmiento. After reduction and detumescence of the swelling in an upper-arm cast for 5–10 days, a Sarmiento Brace is fit and functional body exercises started. Out of 31 consecutive fractures of the shaft of the humerus, 26 were treated by means of a Sarmiento Brace. In all cases bony consolidation was achieved in 6–8 (in one case in 11) weeks without secondary complications. A slight shift of the axis was occurred in two-thirds of the cases. However, the functional result was thoroughly good.

    Description / Table of Contents: Zusammenfassung Distale intraarticuläre Humerusfrakturen erfordern im allgemeinen eine operative Behandlung. Das funktionelle Ergebnis ist abhängig von der anatomischen Rekonstruktion der Gelenkflächen und von der frühzeitigen kontrollierten Nachbehandlung. Das Gesamtergebnis wurde bewertet mit 23,8% sehr gut, 36,0% gut, 22,3% mäßig und 17,9% schlecht. Hierbei ist zu berücksichtigen, daß 18% der Patienten polytraumatisiert waren und in 27% eine offene Gelenktrümmerfraktur vorgelegen hatte.
    Notes: Summary Intraarticular fractures require a surgical therapy. The functional results are depending on the anatomical reconstruction of the joint surfaces and on early kinesitherapy. Remaining disabilities may be controlled in 2/3 of the cases to such an extent that no alterations are necessary in private or professional life. The overall result was very good in 23.8%, good in 36.0%, moderate in 22.3% and bad in 17.9%. These bad results are due to the rate of polytraumatized patients (18%) and the high rate of open comminuted fractures (27%).
