    Notes: Abstract Bleeding from the suprahepatic inferior vena cava (IVC) was encountered in a patient undergoing repeat liver resection for the fourth time due to a recurrence of colorectal liver metastases. Bleeding was observed from an IVC tear above the liver and it was not possible to control it with traditional clamping procedures including total vascular exclusion (a suprahepatic clamp). Hypothermia, cardioplegia, and circulatory arrest were all required to control the bleeding. The inferior vena cava was reconstructed with a pericardial patch. The patient recovered well and was discharged on the 14th postoperative day.

    Notes: Abstract Mixing models for bioreactors on the basis of the tanks-in-series concept are presented and a suitable parameter-estimation method is introduced. The Monte-Carlo-optimization procedure with the inhomogeneity-curve included in the objective function is used. Results of the parameter optimization procedure are given for stirred-tank-bioreactors equipped with one and three Rushton turbines under aerated conditions. The model designed for the stirredtank with three Rushton turbines is capable to describe the mixing properties, while in case of the stirred-tank with one Rushton turbine the simulated radial circulation time does not correlate with the measured one.

    Notes: Abstract A mixing model is coupled with fermentation kinetics in order to simulate a fermentation as a function of mixing conditions and scale-up. The mixing model for a batch stirred tank with three stirrers consists of three regions, each of them characterized by an ideally mixed compartment around the stirrer and two macromixers, i.e. cascades of tank-in-series, describing the recirculation flow. The model contains four parameters — radial and axial circulation time, volume of the ideally mixed stirrer compartment and the number of tanks in each cascade. These values, determined by Mayr et al. in function of the operational conditions and scale-up, were choosen to simulate the fermentation of glutamic acid to show the pH-fluctuation at different control and scale conditions. By choosing optimal regulation properties, such as input flow rate and/or concentration of the base, regulation span, position of the pH-electrode and base input location, etc., fluctuations of the pH-value in the bio-reactor can be minimized. However, the negative effect of insufficient mixing conditions can be reduced only by an increasing number of the base input places. In large scale fermentors, the axial circulation time is rather high, about 5–10 times larger than the radial one. This might result in a large amplitude of the pH-fluctuation. As it is shown, using an input place for base in each stirrer region, the negative impact of the insufficient axial mixing on the fermentation can be diminished perfectly. In this case ammonia should be fed into the reactor as an aqueous solution.

    Notes: Abstract An aorto-duodenal fistula, an infrequent complication of abdominal aortic aneurysm, was diagnosed on the basis of characteristic CT findings: signs of leakage of the aneurysm, blood-filled duodenum and jejunum, and small gas collections within the wall of the aneurysm. The role of imaging techniques in the diagnosis is discussed and the importance of dynamic contrast-enhanced CT is emphasized.

    Notes: Abstract Heart rate changes were measured as a psychophysiological indicator during grasping reflex in 27 full-term healthy newborns at 2.5–50 h after birth. A significant heart rate deceleration accompanied the grasping response. Conclusion Reflex grasping of a mother's hand, as a component of human attachment behaviour, may have a positive calming effect on the infant's state and could have therapeutic value for high risk mother-infant dyads.

    Notes: Zusammenfassung Die Versuche wurden an Fröschen und Katzen vorgenommen. Nach Reizung des N. ischiadicus mit maximalen Kondensatorenentladungen 60/Sek. gelangt die neuromuskuläre Synapse in den Zustand vollständiger Ermüdung (4. Stadium). In diesem Stadium war der Acetylcholingehalt der Nerven bedeutend niedriger als derjenige des gegenseitigen nicht gereizten N. ischiadicus. Der Acetylcholingehalt des entsprechenden Muskels war dagegen erhöht, seine Cholinesteraseaktivität vermindert. Wenn die Reizung im Stadium der vollständigen Ermüdung fortgesetzt wurde, so nahm die Spannung in der 2.–3. Stunde wieder zu und die Kontraktionsfähigkeit des Muskels wurde wieder hergestellt (5. Stadium,Rosenblueth undLuco). In diesem Stadium war der Acetylcholingehalt wieder gleich dem vom ruhenden Nerven, er überstieg sogar den letzteren; der Acetylcholingehalt des Muskels ist dagegen geringer, die Cholinesteraseaktivität höher als auf der normalen Seite. Kontrollversuche zeigten, daß im Acetylcholingehalt und in der Cholinesteraseaktivität der beiderseitigen ruhenden Muskeln und Nerven kein bedeutender Unterschied besteht. Es kann also auf Grund der Versuche festgestellt werden, daß in den verschiedenen Stadien der neuromuskulären Transmission, zwischen dem Zustand der Synapsen bzw. der Kontraktionsfähigkeit der Muskeln und dem Acetylcholin-Cholinesterasesystem ein enger Zusammenhang besteht. Das Acetylcholin spielt also eine wichtige Rolle in der Reizübertragung und kann keinesfalls als Nebenprodukt des Nervenstoffwechsels betrachtet werden.

    Notes: Zusammenfassung Aus den Untersuchungen über den Acetylcholin- und Cholinesterasegehalt verschiedener Organe von normalen und B1-avitaminotischen Ratten geht hervor, daß bei den B1-avitaminotischen Tieren der Acetylcholingehalt der cholinergischen Effectoren (Herz-, quergestreifte- und Dünndarm-Muskulatur) erhöht und der des Zentralnervensystems vermindert ist; der Acetylcholingehalt des N. ischiadicus bleibt unverändert. Der Serum-Cholinesterasegehalt war in den untersuchten Fällen bedeutend vermindert, in den verschiedenen Organen änderte sich der Cholinesterasegehalt entsprechend dem Acetylcholingehalt. Im N. ischiadicus war dagegen neben einem hohen unveränderten Acetylcholingehalt der Cholinesterasegehalt niedriger als im normalen Nerven.

    Notes: Summary A comparative pharmacokinetic study was performed with two types of suppository each containing 20 mg piroxicam. The bioavailability of piroxicam was studied in 24 healthy volunteers in a double blind, randomised, cross over study. No significant difference was found in the pharmacokinetic parameters [AUC(0–72 h), AUC, Cmax, t1/2β calculated from plasma concentration time curves, indicating that the two preparations were bioequivalent.
