    Notes: Abstract A pump source with eight angular multiplexed laser diodes is built. The diode laser emission is collimated and focused using relatively inexpensive aspherical lenses. The maximum output power is 6.3W at 970nm. The overall transmittance of the beam shaping optics is T=85%. The electrical, temporal and spectral properties are studied. The variances of the spatial intensity distribution are Δr0x=99m and Δr0y=85m. The beam propagation factors are M2x=38 and M2y=112. The high intensity in the focus makes this system ideal for end pumping of solid state lasers.

    Notes: Abstract The objective of this research was to investigate the cellular source of soluble ICAM-1 (siCAM-1) from rheumatoid synovial tissue (RS) and its relation to sICAM-1 in synovial fluid (SF) and serum, and to study the expression of ICAM-1 in isolated cells of RS. sICAM-1 was determined by using the enzyme-linked immunosorbent assay (ELISA) and Western blot analysis in supernatants from RS cultured for short periods (n = 19), in SF (n = 7) and in serum (n = 19). ICAM-1 expression, vascularization and inflammatory infiltration (CD3, CD68, CD22) were characterized immunohistochemically in cytospin preparations (n = 18), cryosections (n = 18) and in conventionally stained paraffin sections (n = 19) of RS. The degree of RS vascularization was analysed morphometrically in immunohistochemically stained cryosections (factor VIII related antigen). We found 90-kD sICAM-1 in supernatants of cultured cells, in SF and in sera. sICAM-1 in cellular supernatant correlated significantly (P 〈 0.01) with SF sICAM-1. The amount of sICAM in cellular supernatants showed no correlation to the score of inflammatory infiltration, but correlated significantly (P 〈 0.001) with the vascularization index of RS. The percentage of ICAM-1-expressing cells correlated significantly (P 〈 0.001) with the percentage of CD68-positive macrophages, but not with CD3- and CD22-positive lymphocytes. Macrophages, multinucleated giant cells and endothelial cells exhibited a higher expression of ICAM-1 as compared to lymphocytes and fibroblasts. The differential expression of ICAM-1 on infiltrating leucocytes and resident cells of RS indicates a functional role of ICAM-1 in the local inflammatory process. SF sICAM-1 originated in RS, but serum sICAM-1 did not. Shedding of sICAM-1 by RS was independent of inflammatory infiltration, but depended on the degree of vascularization, indicating that endothelial cells are the major source of sICAM-1 in RS.

    Description / Table of Contents: Summary We report on a 8 year old boy in whom ophthalmological signs (scotoma, fundus hypertonicus stage IV) and an elevated blood pressure lead to the diagnosis of an isolated pheochromocytoma of the left adrenal. 6 months before, a cerebral convolsion was interpreted as a benign convulsion caused by infection. The boy was cured by adrenalectomy. However, 3 years after the initial diagnosis, he showed again typical clinical signs of a pheochromocytoma which was finally confirmed by laboratory tests and computertomography. The right adrenal was removed. Family history was negative. By molecular analysis, an association with von Hippel-Lindau disease was found. Discussion: Our case emphasizes the necessity to exclude a phacomatosis in all cases of pheochromocytoma.
    Notes: Zusammenfassung Wir berichten über einen 8 Jahre alten Jungen, bei dem 6 Monate nach einem unklaren zerebralen Krampfanfall aufgrund der augenärztlichen Symptomatik (Skotom, Fundus hypertonicus Grad IV) ein arterieller Hochdruck diagnostiziert wurde. Die weiteren Untersuchungen ergaben ein isoliertes Phäochromozytom der linken Nebenniere. Der Tumor wurde durch eine Adrenalektomie entfernt, und der Junge war im weiteren Verlauf beschwerdefrei. 3 Jahre später wurde er mit der gleichen klinischen Symptomatik wie bei der Erstdiagnose stationär aufgenommen. Jetzt wurde ein Phäochromozytom in der rechten Nebenniere diagnostiziert. Der Tumor wurde erneut mittels Adrenalektomie entfernt. Andere Tumoren konnten bislang bei dem Jungen nicht gefunden werden. Die Familienanamnese für Phäochromozytome war negativ. Die molekulargenetische Diagnostik ergab eine Mutation im Von-Hippel-Lindau-Gen. Diskussion: Auch bei einem isolierten biadrenalen Phäochromozytom muß eine Phakomatose molekulargenetisch ausgeschlossen werden. In unserem Fall wurde die Diagnose eines Von-Hippel-Lindau-Syndroms gestellt.

    Notes: Abstract Nineteen out of 83 children with asymptomatic haematuria were classified as having IgA glomerulonephritis, characterized by diffuse mesangial deposition of IgA in the absence of systematic disease. Different histological patterns were observed: i.e., minor glomerular lesions (5 cases), focal and segmental proliferative glomerulonephritis (8 cases) and diffuse proliferative glomerulonephritis (6 cases). Recurrent gross haematuria is the clinical hallmark of the disease. Proteinuria was present in 14 children and exceeded 1 g/m2/day in 3 patients. Clinico-pathological correlations showed a close relation between the degree of proteinuria and the histological lesions. Serum IgA levels were elevated in 3 children. Glomerular filtration rate remained above 80 ml/min/1.73 m2 in a 1 to 9 year follow-up.

    Notes: Abstract The aim of the present study was to determine the frequency and amount of chili taken by peptic ulcer patients and control subjects. One hundred three Chinese patients with peptic ulcer and 87 control patients were interviewed using a standard questionnaire. Those subjects who deliberately avoided chili use because of symptoms or advice from friends or medical practitioners were excluded. The median number of times of chili use per month was eight in the ulcer group (25–75% quartiles 1–30) compared to 24 (8–56) in the control group (P〈0.001). The median amount of chili used per month was 312 units (25–75% quartiles 38–899) in the ulcer group compared to 834 units (274–1892) in the control group (P〈0.001). The odds ratio of having peptic ulcer disease, adjusted for age, sex, analgesic use, and smoking by multiple logistic regression, was 0.47 (95% confidence intervals: 0.25–0.89) for subjects who had a higher intake of chili both in terms of frequency as well as amount used compared to those who took less chili. Our data support the hypothesis that chili use has a protective effect against peptic ulcer disease.
