    Notes: SYNOPSIS. Studies were carried out on the introduction of labeled precursors into the DNA of Paramecium aurelia (syngen 4, stock 51) by way of the bacteria that are used for food. A thymine-requiring strain of Escherichia coli (15 T−) was labeled by growth in either H3-methyl thymidine or 2-C14 bromouracil, washed free of the exogenous label, and fed to the paramecia. The tritium label from the bacteria was incorporated almost exclusively into the DNA of the paramecia, whereas it was much less specifically incorporated when introduced directly from the medium. The Cu label from bromouracil was also incorporated mainly into the DNA of the paramecia although a small amount appeared in RNA. The formation of labeled food vacuoles was followed. Food vacuoles were formed at a nearly constant rate, with the total number of vacuoles increasing throughout the cycle. The lifetime of the vacuoles was about 2.5 hours. Incorporation of the label into the DXA of the paramecia begins within a few minutes of the formation of the first labeled vacuole. DNA synthesis begins about 1.5 hr after the previous fission (total cell cycle about 5.8 hr) and progresses at a nearly constant rate throughout the remainder of the cycle.

    Notes: A new type of vena caval filler was to be removed from a woman who had a high risk of massive pulmonary thromboembolism because of extensive thrombus in the iliac vein incorporating the filter. The removal was performed under general anaesthesia in the cardiac operating theatre using transoesophageal echocardiography to monitor the right heart and the pulmonary artery during the critical phase of removal. The manoeuvre succeeded with only insignificant embolisation occurring, and that was identified by transoesophageal echocardiography.

    Notes: Summary A 26-year-old patient presented with epigastric pain of sudden onset and severe puffy swelling of both legs and forearms. An irregularly shaped nodular filling defect on selective jejunal films, severe hypoproteinemia, low IgG concentration, and lymphopenia were suggestive of primary intestinal lymphangiectasia with protein-losing enteropathy, and the patient was placed on a low-fat diet with medium-chain triglycerides. This initially improved his condition, but some weeks later he developed obstructive ileus of the small intestine. On laparotomy yellowish to whitish deposits were found to be present in some segments of the small intestine and a fist-sized mass 100 cm distal to the duodenojejunal flexure was resected without complications. Histologically, the submucosal lymphatics were dilated, and the jejunal wall showed extensive pseudocystic, intramural submucosal lymph edema with secondary bleeding and tight stenosis of the jejunal lumen. During the 14-month follow-up time after discharge the patient has been asymptomatic and working, on no treatment other than a low-fat diet with medium-chain triglycerides.

    Notes: Abstract A case report of a 2-year-old boy who developed severe post-operative low cardiac output, sudden cardiac arrest and consecutive multiple organ system failure after open heart surgery is described. Initially the hemodynamic situation was stabilized by extracorporeal membrane oxygenation (ECMO) combined with continuous hemofiltration (CHF) carried out for 7 days. Continuous veno-venous hemofiltration (CVVH) was necessary for further 17 days. One episode of plasmapheresis restored transiently a severe post-operative coagulopathy and enabled the removal of the pulmonary artery catheter and the chest tubes.

    Notes: Abstract In a retrospective study of 78 cases of adult meningitis, the CSF lactate was measured on the first spinal tap (ST); 25 had a bacterial meningitis, 28 a viral meningitis; 22 other cases had been on antibiotics prior to admission; 3 cases had meningitis of rare aetiology. The median CSF lactate level among the 25 bacterial cases amounted to 13.6 mmol/l (range: 3.5–24.5) whereas it remained low in the 28 viral cases: 2.7 mmol/l (range: 1.4–4.2). These differences are highly significant. The comparison of the CSF lactate level with the other tests routinely performed showed that the CSF lactate level had the highest sensitivity, specificity and predictive values. The CSF lactate level on the first ST had no prognostic value, but a rapid decrease of the CSF lactate during the treatment is indicative of good prognosis. Among the pretreated cases, a high lactate level could be an indication that bacteria were the causal agents. In conclusion, the measurement of the CSF lactate, quickly performed and inexpensive, is worth performing when a meningitis is suspected, as it appears to be the best way of distinguishing bacterial from non-bacterial meningitis.

    Description / Table of Contents: Summary This study compares osteotomy and total hip arthroplasty on advanced osteoarthrosis of the hip. A series of 10 Chiari pelvic osteotomies is presented with the essential prerequisites and the aims of the method. The technical difficulty in executing a cylindrical osteotomy prompted its modification in the oblique plane. This series is too short to permit original deductions, but confirms the conclusions of large reported series, that is, an immediate effect on the Merle d'Aubigné scores (4.6 points), in particular on pain.
    Notes: Résumé Le but du GECO est d'analyser la comparaison de résultats des ostéotomies et des prothèses totales sur les coxarthroses évoluées. Cette série de 10 cas d'ostéotomies du bassin selon Chiari est donc à intégrer dans ce rapport. Le but et les impératifs de la méthode sont rappelés. La difficulté technique de réaliser une ostéotomie cylindrique suggère la possibilité de la modifier en plane-oblique. La série, trop courte pour prêter à des déductions originales, permet cependant d'adhérer aux conclusions issues des grandes séries rapportées, soit un effet immédiat sur les éléments de la cotation de Merle d'Aubigné (4,6 points) et en particulier sur la douleur.
