    Description / Table of Contents: Résumé Quarante patients atteints de coxarthrose ont été examinés par absorptiométrie radiologique à double énergie afin de mesurer l'ostéopénie du côté malade. L'étude a été essentiellement centrée sur le fémur et le tibia proximal. La perte osseuse a été évaluée quantitativement selon une valeur dite “pourcentage controlatéral” (% CL), qui consiste dans la comparaison entre la densité minérale osseuse du côté sain et celle du côté atteint. Pour le col du fémur et le triangle de Ward, le % CL s'établit respectivement à 113% (p〈0.01) et 118% (p〈0.01), et à 75% (p〈0.01) pour le tibia. Le % CL du tibia proximal témoigne de la perte osseuse du côté atteint et pourrait avoir un intérêt clinique pour évaluer la marche, car il concorde avec les paramètres de celle-ci sur l'échelle de cotation de la hanche, par exemple le rapport entre claudication et distance parcourue. Par contre, le % CL du fémur proximal montre un accroissement de la densité minérale osseuse qui ne concorde qu'avec le degré de déformation du col du fémur.
    Notes: Summary Forty patients with unilateral osteoarthritis of the hip were studied with dual energy X-ray absorptiometry to quantify disuse osteopenia in their affected leg by examining the proximal femur and tibia. Bone loss was assessed as a percentage of the contralateral value which compares bone mineral density of the affected and normal sides. The percentage contralateral value in the femoral neck and Ward's triangle was 113% and 118% respectively, while that in the tibia was 75%. Bone loss in the proximal tibia of the affected leg could be of value in assessing gait since it correlates with the gait parameters on the hip rating scale. By contrast, bone mineral increase in the proximal femur and correlates only with the degree of valgus deformity of the femoral neck.

    Description / Table of Contents: Summary We have designed a new type of clamping plate called the “Alligator plate” for posterior cervical spinal fixation and used it in some cases with cervical instability. Seventeen patients with cervical instability treated by posterior cervical fixation using Alligator plate were clinically evaluated. As a control group, 21 patients treated by a wire fixation method (cerclage) were also evaluated. Clinical results were evaluated radiographically on dynamic flexion-extension lateral radiographs with confirmation of the fusion mass on oblique-view radiographs. The results of Alligator plate fixation were excellent in 15 patients, good in one and poor in one. The results of wire fixation were excellent in 15 patients, good in three, fair in one and poor in two. A safe and simple alternative instrumentation, the Alligator plate has sufficient stability to be useful for treating instability of the lower cervical spine.
    Notes: Résumé Nous avons créé un nouveau type de plaque à machoire appelée plaque “Alligator” destinée à l'arthrodèse postérieure du rachis cervical et nous l'avons appliquée à quelques cas d'unstabilité cervicale. L'évaluation clinique a porté sur 17 cas présentant une instabilité cervicale traitée par arthrodèse postérieure à l'aide de plaques “Alligator”. L'évaluation a aussi porté sur un groupe témoin comportant 21 cas traités par la méthode du cerclage. Les résultats cliniques ont été évalués radiologiquement d'après les clichés dynamiques de profil en flexion-extension, et confirmation de la fusion osseuse sur les radios de profil. Les résultats obtenus dans le groupe traité par les plaques “Alligator” ont été excellents chez 15 patients, bons chez l'un et mauvais chez un autre. Les résultats obtenus avec le cerclage ont été excellents chez 15 patients, bons chez trois, passables chez un et mauvais chez deux. La plaque “Alligator” représente une alternative intéressante en matière d'implants cervicaux; elle est d'une stabilité suffisante et peut se montrer de ce fait utile dans le traitement de l'instabilité du rachis cervical inférieur.

This is the fifth case of the both conditions in English literature. Congenital chylothorax is very rare in a trisomy 21 patient. There, however, may be causal relationship between the two conditions.

    Notes: Abstract The relationship between the adhesive properties of the interphase of glass fibre/resin and the resultant composite Mode I delamination fracture toughness in glass fibre fabric laminate (GFFL) was studied. The Mode I interlaminar fracture toughness of GFFL was obtained by using a double cantilever beam (DCB) specimen. The delamination resistance of GFFLs which have two silane coupling agents and three concentration finishes is discussed on the basis of interlaminar fracture toughness. The crack propagation behaviour of DCB testing was mainly divided into stable and unstable manners. The fracture toughness and the crack propagation behaviour were dependent on the types and concentration of silane coupling agents.

    Notes: Abstract The stress corrosion cracking of glass fibre-reinforced plastic (GFRP) accompanies a phenomenon of catastrophic failure as a result of the rapid fall in strength owing to corrosion breaking of the glass fibres. This produces a flat surface without pulling fibres out of the plane. Attack on the glass fibres can only occur by contact with an acid which must first diffuse into the matrix resin. It is confirmed, however, that no diffusion occurs or that it is too slow to be detected. The relationship between fatigue damage and stress corrosion in an acidic environment, has been investigated, focusing on the effect of matrix toughness on the resistance to stress corrosion failure of GFRP. Three types of GFRP, made from matrices with different toughness, were studied after subjecting them to fatigue damage at different levels.

    Notes: Abstract A new method was proposed to evaluate the mechanical properties at the interface between the fibres and the matrix in composites using an embedded single fibre coupon test. A mechanical parameter at the interface (called the interfacial transmissibility, κ) was derived from the fibre strength and the apparent stress of the fibre immediately before the first fracture of embedded fibre, σfa. This parameter indicated the degree of the mechanical transmission from the matrix to the fibre through the interface. This avoided some complicated problems such as the stress distribution along fibre fragments and the critical fragment state in a typical single-fibre test. This new method was tried to determine the κ-values for a fibre glass/epoxy resin with different amounts of a coupling agent at the interface. In order to measure the stress at the first fracture, the fracture process was monitored with a video camera during the single fibre test. The stress values at the first fracture for many coupons were analysed as a function of the three-parameter Weibull distribution. The resulting average stress and its coefficient of variation indicated that the reliability of the measurement for the stress at the first fracture was not less than that obtained by the usual single-fibre test. The change of interfacial transmissibility with amount of the coupling agent revealed the existence of an optimal interface.

    Notes: Summary Between 1979 and 1990 reconstruction using a ceramic prosthesis with a polycrystal alumina segment and a monocrystal alumina stem was carried out in 65 patients after the resection of malignant or benign aggressive bone tumors. Resection of 18 osteosarcomas, 5 chondrosarcomas, 9 other sarcomas, 10 giant cell tumors, 20 metastatic bone tumors, and 3 other bone tumors was followed by replacement of 17 proximal femurs, 12 distal femurs, 12 proximal tibia, 11 proximal humeri, 3 distal radii, 5 midshafts of the long bone, 2 pelvises, and 3 other parts. Results were rated excellent in 4 cases, good in 43, fair in 13, and poor in 4. In the cases with benignly aggressive or low-grade malignant tumors and those with tumors of the proximal femur, proximal tibia, or midshaft, satisfactory results can be obtained. Four skin ulcers, three dislocations, three loosenings, two infections, and two breaks were noted. Close interfacing between the ceramic prosthesis and the bone was observed radiologically in all cases with cementless fixation except in cases with high-grade malignancies in the knee joint. These results demonstrate that the ceramic prosthesis can be beneficial for the management of patients with benignly aggressive or low-grade malignant bone tumors who have retained adequate muscle strength around the joint even after tumor resection.

    Notes: Abstract An embedded single-filament shear-strength test was employed to explore the effect of the reinforced interface on the fracture of single filaments in glass fibre-epoxy resin composites. The interfacial effect was evaluated not only by critical fibre length but also by the fracture process of an embedded single filament observed by applying tensile load to the composites. The mean fragment length, measured at different tensile stresses, decreased with an increase in stress and finally reached a value correlated with critical fragment length. Interfacial reinforcement by silane treatment affects the fracture process rather than the critical fibre length. The behaviour was examined in several factors, such as filament diameter, the tension of filament on the moulding of composites, and the scatter of the composites in mechanical properties. A useful method was proposed to exclude such scatter: specimens treated on a half-length of embedded filament were subjected to this test. The present method leads us to confirm that the reinforced interface has no effect on the critical fibre length, while it does promote the progress of the fracture.
