    Notes: Abstract Streptococcus faecium strain 25 produced a bacteriocin (enterococcin Sf25), metabolized sucrose and contained three plasmids of 2.4, 4.7 and 13.0 MDa. Plasmids Sfp2.4 and Sfp4.7 were cotransferred in a filter mating procedure to sucrose negative and bacteriocin negative S. faecium strain M16. Strain M16 harboured a nonselftransferable plasmid Sfp 19.1 MDa, which was responsible for erythromycin resistance. Transcipient cells of S. faecium M16 contained the 19.1-MDa plasmid and plasmids Sfp2.4 and Sfp4.7, produced the enterococcin Sf25 and gained the ability to degrade sucrose.

    Notes: Abstract Streptococcus faecium strain 3 produced a bacteriocin (enterococcin Sf3) and contained a homogeneous species of plasmid DNA (pJK3) with an Mr of 3.5 · 106. Plasmid pJK3 was transferable in a filter mating procedure to S. faecium M16. The non-bacteriocinogenic strain M16 was susceptible to enterococcin Sf3 and harboured a non-selftransferable 19.1 MDa plasmid, which was responsible for erythromycin resistance. Transcipient cells of S. faecium M16 contained the 19.1 MDa and the pJK3 plasmid, produced the enterococcin Sf3 and were resistant against the inhibitory action of this bacteriocin.

    Description / Table of Contents: Zusammenfassung Bei zwei Mädchen im Alter von 2 und 11 Jahren wurde eine monartikuläre Arthritis beobachtet, die wegen ihres protrahierten Verlaufes und einiger serologischer Hinweise die Diagnose einer rheumatoiden Arthritis nahelegte. Die Gelenkbeschwerden bis zur Ergußbildung blieben bei beiden Kindern auf ein Gelenk beschränkt. Chronisch-rezidivierende Ergüsse machten eine Synovektomie notwendig. Vergleichende licht-, transmissions- und rasterelektronenmikroskopische Untersuchungen des gesamten Synovektomiematerials bestätigten die klinische Verdachtsdiagnose einer rheumatoiden Arthritis. Morphologisch ist bei monartikulärer Manifestation die rheumatoide Synovitis gekennzeichnet durch: Eine ausgeprägte Hyperplasie der gelenkauskleidenden Zellen, eine Polymorphie dieser Zellen bis zur Ausbildung von Riesenzellen, eine absolute Vermehrung der intercellulären Grundsubstanz, ein Überwiegen der ergastoplasmareichen B-Zellen und herdförmige leukocytenreiche perivasale Zellinfiltrate sowie gleichmäßige Präcipitationen von Fibrin an der Oberfläche der Synovialmembran.
    Notes: Summary In 2 girls, aged 2 and 11 years, a diagnosis of juvenile rheumatoid arthritis was suggested by clinical and serological examinations. Joint symptoms including effusion were limited to one joint in each case. Recurrence of joint effusion necessitated synovectomy in both children. Comparative light, transmission and scanning electron microscopic studies performed on the complete synovectomy specimens confirmed the diagnosis of rheumatoid arthritis. In these cases of monarticular rheumatoid arthritis the hallmarks were: Hyperplasia of the synovial lining, cell polymorphism with occasional giant cells, an absolute increase of intercellular matrix, predominance of B-cells rich in ergastoplasm, focal perivascular leucocytic infiltrates and homogeneous precipitations of fibrin on the synovial surface.

Die klinisch bedeutsamen Höhenminderungen im dorsalen Abschnitt der Segmente L4/5 and L5/S1 mit Verstärkung der Lendenlordose sowie deren therapeutische Aufrichtung durch konservative bzw. operative Maßnahmen lassen sich damit genau aufzeichnen.
    Notes: Summary A lumbosacral trapezium can be constructed using the parallels to the lower plate L4 and the upper plate S1 and two perpendiculars to the former; it yields exact measurement data for changes in height and curvature in the lower lumbar motion segments. The clinically relevant reductions in height in the dorsal part of the segments L4-5 and L5-S1, the increase in lumbar lordosis, and therapeutic straightening by means of conservative or operative treatment can thus be plotted exactly.

    Description / Table of Contents: Zusammenfassung Um neben der Fixation auch eine Dekompression im dorsalen Anteil der unteren beiden lumbalen Bewegungssegmente zu erreichen, ergänzen wir die posterolaterale Fusion durch eine mediale interspinale Distraktionsspondylodese. Ein genau vermessener kortikospongiöser Span aus der Spina iliaca posterior wird unter Distraktion bei maximaler Kyphosierung der LWS ohne alloplastisches Osteosynthesematerial zwischen die Dornfortsätze L4 und S1 geklemmt. Der Span soll die Distraktion bis zum Festwerden der posterolateralen Knochenstraßen aufrechthalten. Außerdem wird damit eine dritte Fusionsstrecke geschaffen. Indikation, Technik und postoperatives Vorgehen werden beschrieben. Distraktionsergebnis und -beständigkeit werden mit einem standardisierten Meßverfahren für die Bewegungssegmente L4 bis S1 überprüft.
    Notes: Summary We supplement posterolateral fusion with medial interspinal distraction spondylodesis to attain decompression in addition to fixation in the dorsal part of the two lower lumbar motion segments. An exactly fitted corticospongiosa bone graft from the spina iliaca posterior is wedged between the spinal processes L4 and S1 under distraction with maximal kyphosis of the lumbar spine; metallic implants are not used. The graft maintains the distraction until the posterolateral fusion band is fixed. Furthermore, a third fusion band is thus made. Indication, technique, and postoperative treatment are described. The outcome and durability of the distraction are checked with a standardized measurement procedure for motion segments L4 to S1.

    Notes: Summary In order to obtain frames of reference to determine the place of intersection of the carotid siphon with the dura mater in conventional arteriograms, a debated topic lacking general agreement in the literature, 34 cadaveric sphenoids were prepared with the injection of radiopaque medium in both internal carotid arteries. After having marked the dura mater with steel wire, the pieces were radiographed in lateral view. The points of intersection of the clinoclinoid line and the sphenoidal line with the internal carotid artery as well as the emergence of the ophthalmic artery and its posterior projection were marked on the radiographs obtained. The distances between the various points were measured and submitted to statistical treatment. The results showed multiple regressions with a strong correlation coefficient in two equations. A table was built in which, knowing the distances between the points of the clinoclinoid and the sphenoidal plane lines and those of the ophthalmic artery and its projection, the position of the dura mater which covers the cavernous sinus may be estimated in arteriograms.

    Notes: Summary During nitrogen starvation, a 20- to 250-fold increase in specific urease activity was observed in extracts of P. aeruginosa, P. fluorescens, Hydrogenomonas, M. denitrificans, M. cerificans and B. megaterium. In contrast to these species, high levels of urease were observed in P. vulgaris strains and in S. ureae under all growth conditions. No urease was detectable in strains of E. coli, S. marcescens and B. polymyxa, regardless of growth conditions. Incubated in the absence of an exogenous nitrogen source, the specific urease activity increased during a period of 10 to 20 h in P. aeruginosa, Hydrogenomonas and M. denitrificans. Phosphate starvation did not significantly effect urease formation in these strains. The increase in specific urease activity was found to be repressed by exogenous nitrogen sources, including urea. Inhibition by chloramphenicol, other inhibitors, and by the lack of oxygen or fructose, indicated that a derepressive urease formation may occur in these strains. The involvement of traces of urea possibly released from endogenous sources during starvation is discussed.

    Description / Table of Contents: Zusammenfassung Selbst wenn keine sogenannte Sterilbox zur Verfügung steht, ist es möglich, die Infektionsrate bei orthopädischen Operationen niedrig zu halten. Maßgebend ist die Beachtung allgemeiner Grundsätze der Asepsis, Antisepsis und der antibakteriellen Chemotherapie. Bactericide, atoxische knochengängige Breitbandantibiotica werden entsprechend der jeweiligen Erreger- und Resistenzsituation an einer Klinik nur bei besonders infektionsgefährdeten Operationen eingesetzt. In den Jahren 1971 bis 1973 wurden in der Orthopädischen Universitätsklinik Düsseldorf 1459 aseptische orthopädische Operationen durchgeführt. Im gleichen Zeitraum traten 12 (0,8%) postoperative Wundheilungsstörungen auf.
    Notes: Summary Even if there is no sterile operation box, it is possible to keep the incidence of infections in orthopedic surgery low. It is important to observe the well known principles of asepsis, antisepsis and antibacterial chemotherapy. In operations involving patients especially disposed to infection, it is necessary to apply bactericidal and nontoxic antibiotics maintaining a high bone concentration and include the actual tested bacterial spectrum of the clinic. From 1971–1973 1459 aseptic orthopedic operations were performed in the Orthopädische Universitätsklinik Düsseldorf. Of these operations postoperative wound infections developed in 12 cases (0.8%).

    Description / Table of Contents: Zusammenfassung Schlechte Früh- und Spätergebnisse bei der „klassischen“ Hüftluxationsbehandlung und Frühoperation veranlaßten uns, im Frühjahr 1969 zu einer vorwiegend konservativen funktionellen Behandlungsmethode überzugehen. Die Behandlung mit dem Hanausek-Apparat, der schon seit 30 Jahren in der Tschechoslowakei erfolgreich eingesetzt wird, mußte allerdings gewissen Modifikationen unterworfen werden, weil unsere Luxationskinder infolge verspäteter Diagnose bei Therapiebeginn wesentlich älter waren als dort. Unser Behandlungsschema, das sich bisher als zweckmäßig erwiesen hat, wird erläutert. Aus unseren ersten Behandlungsergebnissen an 194 Luxationshüften geht hervor, daß die konservative funktionelle Therapie mit dem Hanausek-Apparat wegen der geringen Zahl an Reluxationen und Kopfnekrosen der bisher üblichen Behandlung mit Einrenkung in Narkose, Lorenzgips und Frühoperation eindeutig überlegen ist.
    Notes: Summary Poor early and late results of the classical treatment of dislocation of hip joint and operation at an early date led us to adopt, in spring 1969, a basically conservative functional method. Application of the Hanausek apparatus, which has been successfully employed in Czechoslavakia for 30 years, required, however, certain modifications because our children, due to a delayed diagnosis of the dislocation, are much older when treatment begins. The therapeutic scheme which has proved suitable to us is described. The results of the first 194 dislocated hip joints treated show already clearly that conservative functional therapy with the Hanausek apparatus because of the small number of re-dislocations and head necroses is superior over the conventional methods or repositioning under anaesthesia, Lorenz's plaster cast and early operation.
