    Notes: Cerebral blood flow, cerebral oxygen and glucose consumption, and cerebral lactate and pyruvate release were measured; spectral analysis of the EEG was recorded in 10 male patients who had coronary artery bypass surgery. The measurements were taken to evaluate the effects of fentanyl–midazolam anaesthesia during normothermia and during hypothermic nonpulsatile cardiopulmonary bypass at 26°C venous blood temperature, when a temperature-corrected Paco2-value of 5.3 kPa was maintained. Anaesthesia with fentanyl 7 μg/kg and midazolam 200 μg/kg as induction doses, followed by infusions of fentanyl 0.15 μg/kg/minute and midazolam 3 μg/kg/minute, was characterised by a decrease in fast-wave activity and an increase in high-amplitude, slow-wave activity in the EEG. There was also a decrease in cerebral blood flow (38%), oxygen consumption (22%) and glucose consumption (25%), while lactate and pyruvate production remained unchanged. Hypothermia of 26°C venous blood temperature suppressed EEG almost completely and decreased oxygen and glucose consumption by a further 61% and 54%, respectively, with no changes in lactate and pyruvate production while cerebral blood flow increased by 145%. These results show that the effects of fentanyl–midazolam anaesthesia on cerebral metabolism are enhanced during hypothermic cardiopulmonary bypass while the influence of anaesthesia on cerebral blood flow is overshadowed by the practice of a temperature-corrected acid-base management.

    Notes: Abstract The interaction energy-distance curves of fractionated and unfractionated homo- and copolymers were measured. The results were compared quantitatively with the HVO theory. It was found that only a small number of segments per tail are necessary to obtain stability. The experimental energy-distance curves for low molecular weights are described sufficiently well by the HVO theory, with exponentional distribution of tail sizes. For high molecular weights the exponential distribution cannot be considered as a reasonable assumption because the number of segments in the tails is very low. Polydisperse samples of PVA with different contents of acetate groups, and similar molecular weight, indicate an increasing extension of the adsorbed polymer layer with decreasing acetate content.

    Description / Table of Contents: Zusammenfassung 21 Patienten erhielten eine tägliche Standarddosis von 4 g Cefoperazon ohne Rücksicht auf die Nierenfunktion. Thrombozytenfunktion und plasmatische Gerinnung wurden vor und am Tag 7 der Therapie mit Cefoperazon untersucht. Bei Patienten mit normaler Nierenfunktion blieb die Hämostase unverändert. Dagegen trat bei Patienten mit präterminaler Urämie und zusätzlicher komplexer klinischer Situation eine Hämostasestörung auf, welche durch hohe Antibiotikatalspiegel verursacht war. Die gestörte Hämostase war gekennzeichnet durch eine verlängerte Blutungszeit und durch eine Erniedrigung des Quickwertes, hervorgerufen durch einen erworbenen Vitamin-K-Mangel. Ursache des Vitamin-K-Mangels war eine verminderte Zufuhr mit der Nahrung und eine kumarinähnliche Hemmung der hepatischen reduktiven Regeneration von Vitamin K. Um Hämostasestörungen zu vermeiden, werden bei Patienten mit präterminaler Urämie eine Dosisreduktion von Cefoperazon und häufige Kontrollen des Quickwertes empfohlen.
    Notes: Summary 21 Patients with normal and impaired renal function were given cefoperazone in a recommended dose of 4 g/day, irrespective of renal function. Platelet function and plasmatic coagulation were analyzed before and on day 7 of therapy. In patients with normal renal function on their usual diets, there was neither impairment of platelet function nor plasmatic coagulation. High serum antibiotic trough levels, prolongation of bleeding time and decreased vitamin K-dependent coagulation factors, as verified by the prolongation of prothrombin time and the appearance of descarboxyprothrombin, could be observed in those patients with impaired renal function whose insufficiency was far advanced and accompanied by a complex clinical picture. In this situation vitamin K deficiency may be due to poor oral intake, along with interference of hepatic vitamin K metabolism, showing an effect similar to that seen after coumarin therapy. Dosage reduction of the antibiotic in advanced renal failure and repeated control of prothrombin time is advised.
