    Notizen: Summary Three adult dialysis patients developed ascites after having received repeatedly the plasma substitue hydroxyethyl starch (HES 40/0.5). In two cases (total dose 180, and 330 g HES, respectively) the ascites was reversible after discontinuation of the HES administration. In the third case (total dose 915 g HES) the ascites could be controlled only by implantation of a Denver shunt. In this latter case it was shown by histological, electron microscopical, and biochemical findings that the ascites was caused by hepatic sinusoidal obstruction due to an extreme storage of HES in the sinusoidal lining cells. Additional storage was detected in hepatocytes, bile duct epithelia, endothelial cells, and fibroblasts in the portal tracts. Biochemically HES was found in liver tissue at a concentration of 4% (w/w). Although in renal impairment plasma clearance of HES is not significantly different from normal individuals, long-term administration of HES must be regarded inadvisable because of tissue storage which apparently is especially significant in this condition.

    Notizen: Summary For histologic differential diagnosis of fibrotic and cirrhotic liver changes in early infancy Zellweger's cerebro-hepato-renal syndrome has to be considered. A case is reported where a female newborn failed to thrive and developed severe muscular hypotonia, defective intelligence, and seizures. At an age of 12 months an open biopsy was taken from the enlarged liver. Biochemical tests for disorders of carbohydrate metabolism were negative. The light microscopic examination showed severe fibrosis, lobular disarray, and single cell necroses. Neither siderin nor bile pigment could be detected. There were no changes of the small bile ducts and no signs of inflammation — the child died at 18 months. A postmortem needle biopsy of the liver showed the advanced stage of micronodular cirrhosis. The electron microscopic examination of the first biopsy revealed hypertrophy of smooth endoplasmic reticulum, probably induced by anticonvulsive drugs. The mitochondria showed an increase in the density of their matrix and in the number of sometimes tubular cristae. In accordance with reports in literature, no regular peroxisomes (microbodies) could be found in the hepatocytes. Only a very few profiles with an average diameter of 0.16 μm looked like rather small peroxisomes. The cytoplasmic volume fraction of those organelles, however, amounted to 3×10−5 which is only 1/500 of the normal value in human hepatocytes. Although the significance of the absence of peroxisomes is not yet definitively proved, the term ‘Peroxisome Deficiency’ is proposed as a name which probably is more closely related to the pathogenetic mechanism than the hitherto usual designation cerebro-hepato-renal syndrome.

    Notizen: Summary The construction of a simple “incubatin chamber” is presented by which a homogeneous incubation of tissue sections with calculable amounts of immunoreagents is guaranteed. The incubation of large area sections, and parallel incubations of 10 slides are possible. As an example of application a case of alpha-1-antitrypsin deficiency in the liver is demonstrated.

    Beschreibung / Inhaltsverzeichnis: Zusammenfassung Am Modell der taurocholatinduzierten akuten hämorrhagischen Pankreatitis der Ratte wurden die Auswirkungen dieser Erkrankung auf das respiratorische System untersucht. Vier bzw. 24 h nach Induktion kam es zu einem Abfall des pH, des Basenexcess und des pO2, wärend der pCO2 geringgradig erhöht war. Die Compliance-Parameter erfuhren eine pankreatitisbedingte Verschlechterung; das extravasale Lungenwasser als Maß für ein evtl. auftretendes Lungenödem blieb dagegen weitgehend unbeeinflußt. Ein Schock als Ursache für die respiratorischen Störungen kann aufgrund der unveränderten Kreislaufparameter und des normalen Serumlactats ausgeschlossen werden. Die Veränderung der Compliance-Parameter spricht für eine Surfactant-Schädigung im Rahmen der akuten Pankreatitis.
    Notizen: Summary The model of taurocholate-induced acute hemorrhagic pancreatitis in the rat was used to investigate the effects of this disease on the respiratory system. After induction (4 and 24 h) there was a decrease in pH, base excess, and pO2 while pCO2 was slightly increased. The parameters of lung compliance deteriorated due to pancreatitis; however, the extravascular lung water, as indicator for a lung edema, remained unchanged. This respiratory disorder cannot be caused by shock because the circulatory parameters and serum lactete were stable. Deterioration of the compliance parameters may be caused by lung surfactant damage.
