    Notes: Summary Three adult dialysis patients developed ascites after having received repeatedly the plasma substitue hydroxyethyl starch (HES 40/0.5). In two cases (total dose 180, and 330 g HES, respectively) the ascites was reversible after discontinuation of the HES administration. In the third case (total dose 915 g HES) the ascites could be controlled only by implantation of a Denver shunt. In this latter case it was shown by histological, electron microscopical, and biochemical findings that the ascites was caused by hepatic sinusoidal obstruction due to an extreme storage of HES in the sinusoidal lining cells. Additional storage was detected in hepatocytes, bile duct epithelia, endothelial cells, and fibroblasts in the portal tracts. Biochemically HES was found in liver tissue at a concentration of 4% (w/w). Although in renal impairment plasma clearance of HES is not significantly different from normal individuals, long-term administration of HES must be regarded inadvisable because of tissue storage which apparently is especially significant in this condition.

    Description / Table of Contents: Zusammenfassung Bei diätetisch behandelter Niereninsuffizienz (Kartoffel-Ei-Diät, 0,4 g Protein /kg Kgw.) wird ein alimentärer Vitamin B6-Mangel nachgewiesen, der durch die Gabe von ca 20 mg Vitamin B6/die beseitigt werden kann. Unter der Hämodialyse kommt es zu Vitamin B6-Verlusten, die größenordnungsmässig der Urinausscheidung von Normalpersonen vergleichbar sind. Hinweise für eine Hemmung der Pyridoxalphosphatsynthese konnten nicht gefunden werden.
    Notes: Summary In patients with chronic renal failure on potatoe-eggdiet (0.4 g protein/kg body weight) an alimentary vitamin B6-deficiency occurs, which can be overcome by 20 mg vitamin B6/day. Chronic hemodialysis causes a vitamin B6 loss which amounts to a quantity similar to the daily urinary excretion in normal persons. No hints for an inhibition of the synthesis of pyridoxal phosphate could be found.

    Description / Table of Contents: Zusammenfassung 6 Patienten mit suizidalen Vergiftungen wurden bisher in unserem Zentrum mit der Haemocol-Kapsel behandelt. Davon haben 4 Patienten mit Schlafmittelvergiftung überlebt. 2 Patienten mit Vergiftung durch Agrochemikalien sind trotz Kohleperfusion verstorben. An Nebenwirkungen wurden Blutdruckabfälle, Thrombozytopenie, Depletion an Immunglobulinen und Adsorption von Medikamenten beobachtet. Zur Zeit müssen diese Epiphänomene bei der Indikationsstellung als unvermeidbar mit ins Kalkül eingehen.
    Notes: Summary 6 patients with severe self-poisoning were treated by charcoal-haemoperfusion in our centre up to now. In four of them (all suffering from sleeping drug overdosage) the treatment was successful. Two patients with intoxications by agrochemicals died in spite of haemoperfusion. Side effects of haemoperfusion were drops of blood pressure and platelet count, depletion of immune bodies, and adsorption of remedies. Up to now, the indication for haemoperfusion has to consider these secondary actions of encapsulated charcoal as inevitable.

    Description / Table of Contents: Zusammenfassung Im normalen Liquor und bei Kindern mit viraler Meningitis findet man kein Lysozym. Kinder mit bakterieller Meningitis (0,23±0,14 mg/100 ml) und cerebralen Krampfanfällen (0–0,82 mg/100 ml) haben im Liquor Lysozym in unterschiedlich hoher Konzentration. Lysozym ist somit von Bedeutung bei der Differentialdiagnose zwischen viraler und bakterieller Meningitis; die Anwesenheit von Lysozym belegt eindeutig deren bakterielle Ätiologie. Die β2-Mikroglobulinkonzentration im Liquor gesunder Kinder beträgt 0,11±0,05 mg/100 ml; bei Kindern mit viraler Meningitis 0,20±0,06 mg/100 ml und bei bakterieller Meningitis 0,44±0,17 mg/100 ml. Bei cerebralen Krampfanfällen findet man ebenfalls leicht erhöhte β2-Mikroglobulinwerte.
    Notes: Summary Lysozyme is absent from normal cerebrospinal fluid (C.S.F.) and in C.S.F. from children with viral meningitis. Appreciable amounts of lysozyme were noted in C.S.F. from children with bacterial meningitis (0.23±0.14 mg/100 ml) and cerebral convulsions (0–0.82 mg/100 ml). The C.S.F.-lysozyme content is a sensitive indicator for bacterial meningitis and important in the differential diagnosis between viral and bacterial meningitis. The β2-microglobulin content of C.S.F. in healthy children was 0.11±0.05 mg/100 ml; in children with viral meningitis 0.20±0.06 mg/100 ml and in children with bacterial meningitis 0.44±0.17 mg/100 ml. Children with cerebral convulsions had also a rise in C.S.F. β2-microglobulin.

    Description / Table of Contents: Zusammenfassung β2-Mikroglobulin ist ein Plasmaprotein niedrigen Molekulargewichts (11800), das in geringen Mengen auch im Harn und Liquor vorkommt. Seine Serumkonzentration liegt bei gesunden Kindern bei 0,12±0,04 mg/100 ml, seine renale Ausscheidung bei 0,07±0,05 mg/24 Std. Bei glomerulärer Nephropathie sind die Serumspiegel bei eingeschränkter GFR erhöht, während bei tubulärer Nephropathie die Ausscheidung im Harn erheblich gesteigert ist. Es besteht eine enge Korrelation zwischen β2-Mikroglobulin und der GFR.
    Notes: Abstract β2 microglobulin is a plasma protein of low molecular weight (11800), which also appears in urine in small quantities and cerebrospinal fluid. Its serum concentration in healthy children is 0.12±0.04 mg/100 ml, its renal excretion is 0.07±0.05 mg/24 hrs. In glomerular nephropathy serum levels are augmented with diminished GFR, while in tubular nephropathy excretion in the urine is raised considerably. There is a close correlation between serum β2 microglobulin and GFR.

    Notes: Summary Postoperative parenteral nutrition can only be optimally effective if the characteristics of post-traumatic metabolism are taken into account. Two main possibilities are discussed for the carbohydrate component of parenteral nutrition during this phase: glucose with high doses of insulin or non-glucose carbohydrates (sugar substitutes) possibly in a suitable combination with glucose. The risks as well as the technical and organisational problems involved in the use of them are discussed and the authors prefer the second of the two alternatives. Possible side effects of nonglucose carbohydrates are pointed out and it is shown how these can be avoided by observing dose guidelines. So far a combination of fructose : glucose : xylitol in a ratio of 2 : 1 : 1 with a total dose of 0.50 g/kg/hour has been studied most thoroughly. This combination normalises the fat metabolism and improves glucose tolerance without requiring exogenous insulin. Experiences with this combination as well as individual non-glucose carbohydrates in the postoperative phase are mentioned. Controlled infusions of non glucose carbohydrates on operated patients have been given continuously for up to 7 days and in some cases even for several weeks. No side effects, no deviations from a steady state and no abnormal changes of the laboratory values occurred. The authors are of the opinion that non glucose carbohydrates are necessary if the facilities for frequent blood sugar controls are not available.

    Description / Table of Contents: Zusammenfassung In Versuchen an Meerschweinchenherzen in situ wurde eine Zunahme der Asphyxietoleranz im frühen hämorrhagischen Schock auf das Vierfache des Kontrollwertes gefunden. Diese Befunde sprechen für einen energiesparenden Mechanismus im Schock, der das Herz nicht schädigt, sondern metabolisch entlastet. Myokardanalysen zeigen, daß als Folge der durch den Schock bedingten kardialen Entlastung der intrazelluläre Kaliumbestand unter einer gleichzeitigen Asphyxiebelastung verzögert und deutlich geringer abnimmt.
    Notes: Summary In tests on guinea pig hearts in situ asphyxia tolerance was found to be increased early in hemorrhagic shock four times control value. These results are suggestive of an eergy saving mechanism in shock whereby the heart is not damaged but metabolically relieved. Myocardial analyses showed that, owing to shock-related cardiac relief, under a simultaneous asphyctic stress the intracellular potassium stock decreases later and markedly less.
