    Description / Table of Contents: Zusammenfassung Bei 81 Patienten mit tiefen venösen Thrombosen der unteren Extremität wurde eine Fibrinolysebehandlung mit Urokinase nach einem neuen, höher dosierten Therapieschema in Kombination mit Heparin durchgeführt. Unter Gabe einer Urokinasedosis von 1000–2000 IE/kg/h (Initialdosis 150000–250000 IE) konnte bei frischen, bis zu 10 Tage alten Thrombosen eine komplette oder Teileröffnung bei 68% der Patienten erzielt werden. Dabei zeigte die höhere Urokinasedosierung im Beckenvenenbereich mit einer Wiedereröffnungsrate von 67% im Vergleich zur niedrigeren Dosis mit einer Rekanalisierungsrate von nur 43% einen deutlich stärkeren thrombolytischen Effekt. Bei den Thrombosen im V. poplitea- oder Wadenvenenbereich konnte dagegen mit einer Befundbesserung von 80 und 78% bzw. 75 und 62% ein nur gering differierender Behandlungserfolg beobachtet werden. Die Ergebnisse weisen darauf hin, daß die hier angewendete höhere Urokinasedosierung (Erhaltungsdosis 2000 IE/kg/h) der niedrigeren bei Vorliegen langstreckiger Thrombosen überlegen ist. Unter der höheren Dosierung richtete sich die weitere Urokinasedosis nach der Fibrinogenkonzentration nach Clauss, wobei ein Wert von 50–100 mg% angestrebt wurde und ein Wert von 50 mg% nicht unterschritten werden sollte. Die Steuerung der Therapie gestaltete sich dabei problemlos. An Nebenwirkungen wurden bei 8,6% der Patienten Makrohämaturien und nur in 6% der Fälle ein Hb-Abfall von mehr als 2 g% beobachtet. Lebensbedrohliche oder cerebrale Blutungen traten nicht auf. Der hohe Anteil älterer Phlebothrombosen und die dann wesentlich geringeren therapeutischen Erfolgsaussichten (Rekanalisierung in nur 23% der Fälle) unterstreichen die Notwendigkeit einer möglichst frühzeitigen Klinikeinweisung.
    Notes: Summary In 81 patients with deep vein thrombosis of the lower limb, urokinase therapy was performed in combination with heparin according to a new regimen at higher dosages. When urokinase was administered at an initial maintenance dosage of 1,000–2,000 IU/kg/h (loading dose 150,000–250,000 IU), phlebographically documented complete or partial recanalization could be observed in 68% of the cases. The higher dosage schedule induced a more pronounced deobliteration expecially in treatment of iliac vein thromboses (67% recanalization) in comparison to the lower dosage regimen (only 43% recanalization). Nearly comparable therapeutic results could be achieved in therapy of popliteal or saphenous vein thromboses. The data suggest that the higher dosage schedule examined here is indicated in treatment of extensive and large volume thromboses. The dosage of urokinase was further adjusted to attain a reduction of fibrinogen to 50–100 mg/dl. The concentration should not fall below 50 mg/dl. Therapy with urokinase proved practicable. Serious side effects did not occur. 8.6% of the patients showed hematuria and 6% a decrease of the Hb by more than 2 g/dl. The high proportion of older thromboses and the only low rate of recanalization (23%) in these cases suggest the necessity of an early commencement of fibrinolyses in therapy of deep vein thrombosis.

    Description / Table of Contents: Zusammenfassung Bei 18 Patienten mit einer Achselvenenthrombose wurde eine fibrinolytische Therapie mit Urokinase in Kombination mit Heparin durchgeführt. Der thrombolytische Effekt war dabei eindeutig vom Alter der Thrombose und der Höhe der Urokinasedosis abhängig. Unter Gabe einer anfänglichen Erhaltungsdosis von 1000–2000 IE/kg/h (Initialdosis 150 000–250 000 IE Urokinase) in Kombination mit Heparin (15–17 E/kg/h) konnte bei neun der 11 Patienten (82%) mit frischen Thrombosen ein nahezu kompletter Behandlungserfolg erzielt werden. Bei einem Thrombusalter von mehr als 10 Tagen war keine phlebographische Befundänderung nachweisbar. Wesentliche Nebenwirkungen wurden nicht beobachtet. Auf Grund der hier mitgeteilten Behandlungsergebnisse sollte bei frischen Thrombosen der V. subclavia oder axillaris eine thrombolytische Therapie mit Urokinase durchgeführt werden. Der hier vorgelegte Erfahrungsbericht erlaubt darüber hinaus generelle Rückschlüsse zur Dosierung und Wirksamkeit der fibrinolytischen Substanz Urokinase.
    Notes: Summary In 18 cases with primary subclavian-axillary vein thrombosis fibrinolytic therapy was performed with urokinase in combination with heparin. The thrombolytic efficacy clearly depended on the thrombus age and the dose of urokinase applied. Under treatment with a median initial maintenance dosage of urokinase of 1,000–2,000 IU/kg/h (loading dose 150,000–250,000 IU urokinase) in combination with heparin (15–17 U kg/h) in nine of 11 patients (82%) with recently developed (8 days or less) thrombosis, a nearly complete deobliteration of the venous system was observed. In the cases with thrombosis of more than 10 days no alteration of the venous occlusions could be seen. Relevant side effects did not occur. Our results emphasize urokinase therapy of acute subclavian-axillary vein thrombosis and permit general inferences concerning the efficacy and the dosage requirements of the thrombolytic substance urokinase.

    Notes: Summary The effect of an immunotherapy with Corynebacterium parvum on the blood coagulation system was investigated in a randomized trial of 18 patients with metastatic breast cancer. All patients received cytostatic therapy. Additionally, C. parvum was given intravenously on day 15 of the cytostatic cycle (group I) or on day 1 (group II) or not at all (group III). Fibrinopeptide A increased within 2 h after intravenous administration of C. parvum in groups I and II and normalized after 24 h (p〈0.05). Platelet counts decreased continuously in all treatment groups (p〈0.05). Prothrombin time, fibrinogen concentration, factor VIII:C and factor VIIIR:Ag were not affected. The fibrinolytic activity showed a slight but not statistically significant increase after intravenous administration of C. parvum. The data suggest that plasma hypercoagulability is induced or enhanced in man even after small intravenous doses of C. parvum.

    Notes: Summary A previous interaction study of sulfinpyrazone (Anturano®) suggested that it induced microsomal drug metabolizing enzymes in the liver. To verify this finding the effect of sulfinpyrazone 800 mg per day for four weeks was investigated in ten healthy volunteers. Both the therapeutic actions of sulfinpyrazone, the uricosuric and the antiaggregating effects, were demonstrated (p〈0.05). The influence on the microsomal drug metabolizing system in the liver was demonstrated by an increase in serum-λ-glutamyl transpeptidase from 15.1 to 23.3 U/l (p〉0.05), a significant increase in the urinary excretion of d-glucaric acid (29.6 to 77.9 µMol/24 h,p〈0.05) and an increase in antipyrine clearance from 50.3 ml/min to 83.9 ml/min (p〈0.05). The possibility of enhancement of drug metabolism during treatment with sulfinpyrazone in combination with other drugs should be kept in mind.

    Notes: Summary In patients on oral warfarin, nicoumalone and phenindione an increase in the anticoagulant effect has been described during concomitant treatment with cimetidine. Therefore the effect of cimetidine on the steady state dynamics of phenprocoumon has been investigated in ten outpatients. No change in the anticoagulant effect of phenprocoumon was observed during or after two weeks on cimetidine, as measured by the thrombotest coagulation method, prothrombin time, fibrinopeptide A concentration and plasma phenprocoumon level. The data show that cimetidine does not interact with the metabolism of phenprocoumon in contrast to warfarin. Thus, phenprocoumon maintenance therapy when combined with concomitant cimetidine treatment can be considered not to carry an increased risk of haemorrhagic complications.

    Description / Table of Contents: Summary In 40 patients with non-metastasising (n=31) and metastasising (n=9) renal cell carcinoma, evidence of Stauffer's syndrome (increase in alkaline serum phosphatase and prolongation of prothrombin time) was found in 18 patients. Prolongation of prothrombin time was not due to depletion of vitamin K-dependent coagulation factors or manifest fibrinolysis, but due to the presence of circulating fibrinogen fibrinmonomer-FDP complexes. Ethanol gelation test was found to be positive in 28/40 subjects and soluble fibrin monomer complexes were increased in 38/40 patients. The resulting disturbance of fibrinogen-fibrin conversion was reflected by an increase in thrombin coagulase time and reptilase time. These findings suggests a state of latent compensated intravascular coagulation (presumably triggered within the vascular tumor). For diagnostic purposes the most sensitive indicator is thrombin coagulase time. Thrombin coagulase time normalised after tumor resection and was positive in patients with recurrent metastases. The increase in alkaline serum phosphatase was due to an increase in the hepatic isoenzyme. Such an increase was much more common than the elevation of total alkaline serum phosphatase. Regan's isoenzyme was only found in 1 subject. In parallel, gamma-GT was elevated in 24 patients. The study shows that Stauffer's syndrome occurs more frequently than commonly assumed when thrombin coagulase time, gamma-GT and the hepatic isoenzyme of alkaline serum phosphatase are determined in patients with renal cell carcinoma. DIC and low grade fibrinolysis may account for the coagulation abnormalities of the syndrome.
    Notes: Zusammenfassung Ein Stauffer-Syndrom (erhöhte alkalische Phosphatase und verlängerte Prothrombinzeit) wurde bei 18 von 40 Hypernephrom-Patienten gefunden. Es konnte gezeigt werden, daß die verlängerte Prothrombinzeit nicht auf eine Verminderung Vitamin K-abhängiger Gerinnungsfaktoren, sondern auf zirkulierende Fibrinogen-Fibrinomer-Fibrinspaltproduktkomplexe zurückzuführen ist. Der Alkoholtest nach Godal war bei 28 von 48 Patienten positiv und erhöhte Mengen an zirkulierenden Fibrinmonomeren wurden bei 38 von 40 Patienten gefunden. Eine gesteigerte Fibrinolyse ließ sich in 19 von 40 Patienten nachweisen. Die Verlängerung der Thrombinkoagulase-und Reptilasezeit wird auf die zirkulierenden Fibrinmonomer-Fibrinspaltproduktkomplexe zurückgeführt, die die gestörte Umwandlung von Fibrinogen in Fibrin verursachen. Die vorliegenden Befunde sprechen für eine latente kompensierte intravasale Verbrauchskoagulopathie, die wahrscheinlich innerhalb des gefäßreichen Tumors ausgelöst wird. Als empfindlicher Indikator für diagnostische Zwecke erwies sich die Thrombinkoagulasezeit. Die Thrombinkoagulasezeit normalisierte sich nach chirurgischer Entfernung des Tumors und wurde nach Auftreten von Metastasen wieder pathologisch. Die Erhöhung der alkalischen Phosphatase war in der Regel nur auf einen Anstieg des hepatischen Isoenzyms zurückzuführen. Zum Nachweis des Stauffer-Syndroms erwiesen sich das hepatische Isoenzym der alkalischen Phosphatase und die Gamma-GT empfindlicher als die Gesamt-alkalische Phosphatase.

Der Anwendungsbereich der ersteren ist durch die schlechtere Gewebsverträglichkeit limitiert. Biogene Kleber bestehen aus Fibrinogenkonzentraten, FXIII, Thrombin, die in Anwesenheit von Ca++ ein anhaftendes Fibringerinnsel ergeben. Eigene experimentelle Untersuchungen bestätigen den positiven Einfluß auf Hämostase und Wundheilung. Ein auf der Basis von Kollagen entwickelter Trockenkleber stellt eine neuartige Form biogenen Klebematerials dar.
    Notes: Summary Tissue glues can be divided into two groups: synthetic and biogenic. The use of the first is limited by poor tissue tolerance. Biogenic glue consists of fibrinogen, factor XIII, and thrombin, which form a fibrin clot in the presence of calcium. In our experiments we were able to prove an advantageous effect on hemostasis and healing. A new dried gluten developed on the basis of collagen represents a new type of biogenic gluten.

    Notes: Abstract Thickness correction of single crystal57Fe quadrupolar spectra is treated. Since it is dependent on the polarization of an absorption line, the thickness correction cannot be straightforward. We discuss a self-consistent method which uses the concept of the intensity tensor. In the case of monoclinic crystals, a linear manifold of solutions for the electric field gradient is obtained. We discuss that for hexacoordinated complexes the range of possible EFG tensors can be limited by using arguments of p-isomorphism of a T2g state.

    Notes: Abstract A series of compounds all containing the ferrous hexaquo complex were investigated by Mössbauer spectroscopy at 4.2K and in magnetic fields up to 5T and far infrared techniques. The results are interpreted in terms of a T2g ligand field model for which normal distortions of the water octahedron are used as parameters. The ferrous hexaquo complex can be described in all compounds by the same covalency, spin-orbit coupling and hyperfine coupling constants.
