    Description / Table of Contents: ZusammenfaBung „Wer behauptet, das ideale Verfahren zur Behandlung des Magengeschwürs gefunden zu haben, muB als unbescheiden gelten.” Dieser Satz von Schnitzler (1914) gilt im Prinzip bis heute. Pathogenese und operative Verfahren sind nicht kongruent. Es mangelt noch am schlüBigen Konzept der Pathogenese. In kleinen Schritten werden weitere pathogenetische Faktoren und Fakten bekannt, so u. a. für das Ulcus duodeni lokal wirksame chemische Noxen (z. B. Propionitril) für das Ulcus ventriculi der Reflux von Duodenalsaft (z. B. Lysolecithin), der die Schleimhautbarriere vom Magen zu korrodieren vermag. Die derzeit bekannten pathogenetischen Mechanismen werden im einzelnen für die üblichen Geschwürslokalisationen dargestellt.
    Notes: Summary “Anyone who claims to have discovered the ideal procedure in the treatment of gastric ulcer, should be considered immodest.” These words by Schnitzler (1914) are in principle still valid. The pathogenesis and operative procedure are not identical. There is still no conclusive proof of the pathogenesis. The disclosure of other pathogenetic factors and facts is gradual. Locally active chemical noxae (e.g., propionitril) are responsible for the development of duodenal ulcer. Gastric ulcer is caused by the reflux of duodenal juice (e.g., lysolecithin, which is capable of corroding the mucosal barrier of the stomach. The current pathogenetic mechanisms of ulcer of the usual locations are specified.

    Description / Table of Contents: Zusammenfassung Bei der Anastomose zwischen Ösophagus und Dünndarm kommt der Gefahr einer Nahtinsuffizienz nach wie vor eine prognostisch maßgebliche Bedeutung zu. Um dieser Komplikation vorzubeugen, haben wir eine Methode entwickelt, bei der wir eine terminolaterale Ösophagojejunostomie mit einem überstehenden Darmsegment einmanschettieren. Wir haben unser Vorgehen bisher bei fünf Patienten störungsfrei, d. h. ohne Insuffizienz und auch ohne Stenose, erproben können. Wir interponieren ein langes oberes Jejunalsegment orthograd und isoperistaltisch zwischen Ösophagus und Duodenum. Dieses Vorgehen kommt unseres Erachtens der Erhaltung der operativ faßbaren Partial-funktionen des normalen Magens unter allen anderen vergleichbaren Methoden am nächsten.
    Notes: Summary To overcome the specific problems of esophagojejunostomy, leakage and reflux, a method of end-to-side anastomosis completely surrounded by the proximal jejunal stump was developed. A proximal jejunal loop is isolated and used for isoperistaltic interposition between esophagus and duodenum. About 15 cm of the jejunal segment are left orally to the proximal anastomosis which is performed end-to-side using a single layer of interrupted sutures. The blind loop is then wrapped completely and symmetrically around the anastomosis and sutured to itself and the esophagus to keep it in place. Complete envelopment of the leakage-prone anastomosis is thus achieved. The distal end of the jejunal segment is anastomosed to the duodenum end-to-end. The technique in addition provides protection against reflux into the esophagus by gaseous distension of the proximal jejunal segment surrounding the anastomosis without impeding the passage due to a valve-like action which allows unidirectional flow. Clinical results so far are promising and show absence of leakage and/or stenosis in all cases of esophagojejunostomy operated according to the method described.

    Notes: Abstract Antilambda production is studied inK − p interactions at 32 GeV/c. Both total and differential cross sections are presented. The inclusive $$\bar \Lambda $$ production cross section amounts to 109±7 μb. A remarkable energy dependence is observed, σ( $$\bar \Lambda $$ ) increasing by a factor of four between 14.3 and 32 GeV/c. Thep ⊥ 2 distribution exhibits an exponential fall-off with a slope of 3.3±0.2 (GeV/c)−2. Most of the $$\bar \Lambda $$ 's are emitted in the forward hemisphere. The invariantx distribution increases between 14.3 and 32 GeV/c. Data are presented for $$\bar \Lambda $$ production inK - p→Λ $$\bar \Lambda $$ +X K - p→ $$\bar \Lambda $$ K n +X, andK - p→ $$\bar \Lambda $$ p+X.
