Zusammenfassung Bei 4 Patienten mit arterieller Hypertonie bei Cushing-Syndrom wurden das Herzminutenvolumen, der arterielle Blutdruck, die Herzfrequenz und das Erythrocyten-und Plasmavolumen bestimmt. Aus den genannten Größen wurden der Herzindex, der Schlagindex, der periphere Strömungswiderstand und der Körperhämatokrit berechnet. Es fanden sich folgende Ergebnisse: 1. Der Herzindex war bei 2 Patienten normal, bei 2 Patienten gegenüber einem Normalkollektiv etwa gleichen Lebensalters erniedrigt. Die Verkleinerung des Herzindex kam bei 1 Patienten durch eine Abnahme der Herzfrequenz, bei 1 Patienten durch eine Abnahme des Schlagindex zustande. 2. Der Schlagindex war bei 3 Patienten gegenüber einem Vergleichskollektiv etwa gleichen Lebensalters erniedrigt, bei 1 Patienten im Normbereich. 3. Die Herzfrequenz lag bei 2 Patienten höher und bei 1 Patienten niedriger als bei den Normalpersonen. Bei 1 Patienten fand sich eine normale Herzfrequenz. 4. Der berechnete periphere Strömungswiderstand war bei allen 4 Patienten gegenüber einem Normalkollektiv etwa gleichen Lebensalters erhöht. 5. Das Blutvolumen war bei Bezug auf das Körpergewicht und die Körperoberfläche gegenüber den Normalkollektiven gleichen Geschlechts bei 2 Patienten normal und bei 2 Patienten verkleinert. Die Abnahme des Blutvolumens kam bei 1 Patienten durch eine Verminderung des Erythrocytenvolumens, bei 1 Patienten durch eine Verminderung des Plasmavolumens zustande. 6. Auf Grund der erhobenen Befunde darf angenommen werden, daß die Blutdruckerhöhung beim Cushing-Syndrom auf eine Zunahme des peripheren Strömungswiderstandes bei normalem bis erniedrigtem Herzminutenvolumen zurückzuführen ist.

Summary Cardiac output, blood pressurc, heart rate, red cell volume and plasma volume were measured in 4 patients with Cushing syndrome. Cardiac index, stroke index, total peripheral resistance and plasma volume/red cell volume ratio were calculated herefrom, showing: 1. Cardiac index was normal in 2 patients, and decreased in 2 patients in comparison with normal subjects of the same age. In 1 patient, the decreased cardiac index was due to lower heart rate; 1 patient showed decreased stroke index. 2. In 3 patients, the stroke index was lowered compared to that of normal subjects of the same age; it was normal in 1 patient. 3. The heart rate was higher in 2 patients and lower in 1 patient as compared to that of normal subjects; it was normal in one patient. 4. In all 4 patients the calculated total peripheral resistance was increased compared with normal subjects of the same age. 5. Blood volume related to body weight and body surface was reduced in 2 patients and normal in two, all compared with normal subjects of the same sex. The reduced blood volume was caused by lowered red cell volume in 1 patient, and to lowered plasma volume in another. 6. It can be concluded herefrom that the elevated arterial pressure in Cushing's syndrome may result from increased total peripheral resistance along with normal or decreased cardiac output.
