    Notes: Summary We report on incidental findings during family screening of two kindreds with multiple endocrine neoplasia type IIa. Pheochromocytoma and medullary thyroid carcinoma of considerable size were detected. The results underline the importance of early diagnosis of the syndrome, since the afflicted may be almost or wholly asymptomatic. High resolution chromosome banding studies were carried out in both families, but no abnormality was found. Linkage analysis using DNA markers closely related to the chromosomal locus at chromosome 10 was carried out and was positive in two asymptomatic offspring of one family, whereas the markers were not informative in a second family. We recommend early linkage analysis for establishing the genetic status in offspring of multiple endocrine neoplasia type IIa families to identify for further screening those who are predicted to be gene carrier.

    Description / Table of Contents: Abstract Scintigraphic recording of regional ventilation and perfusion with 99mTc-Aerosol and 99mTc-MAA remain in the foreground of nuclear medicine pulmonary diagnostics. The most important indication for ventilation scintigraphy is the prediction of postoperative pulmonary function, wich is still performed in many hospitals wth perfusion scintigraphy, and with which, in turn, intrapulmonary right-left shunts can be simply and also semiquantitatively recorded. Combined ventilation/perfusion scintigraphy offers a very high degree of sensitivity in the proof of acute pulmonary embolism, is therefore exceptionally well suited for exclusion diagnostics, while specificity compared to pulmonary angiography and spiral CT still needs some clarification. The self-cleaning mechanism of the lung can be quantitatively examined using mucociliary and resorptive clearance. The clinical areas of application are limited for methodical reasons. Primary diagnostics of bronchial carcinoma and dignity differentiation of solitary pulmonary nodules, preferably with 18F-FDG PET are gaining steadily in importance.
    Notes: Zusammenfassung Im Vordergrund der nuklearmedizinischen Lungendiagnostik steht nach wie vor die szintigraphische Abbildung der regionalen Ventilation und Perfusion mit 99mTc-Aerosol und 99mTc-MAA. Wichtigste Indikation für die Ventilationsszintigraphie ist die Voraussage der postoperativen Lungenfunktion, die vielerorts noch mit der Perfusionsszintigraphie durchgeführt wird, mit der sich wiederum intrapulmonale Rechts-links-Shunts einfach und auch semiquantitativ erfassen lassen. Die kombinierte Ventilations-/Perfusionsszintigraphie bietet ein Höchstmaß an Sensitivität beim Nachweis der akuten Lungenembolie, ist deshalb für die Ausschlussdiagnostik hervorragend geeignet, die Spezifität im Vergleich zur Pulmonalisangiographie und Spiral-CT ist weiterhin klärungsbedürftig. Die Selbstreinigungsmechanismen der Lunge lassen sich mit der mukoziliären und resorptiven Clearance quantitativ erfassen, die klinischen Einsatzgebiete sind auch aus methodischen Gründen beschränkt. Zunehmende Bedeutung erlangen die Primärdiagnostik des Bronchialkarzinoms sowie die Dignitätsabklärung solitärer Lungenrundherde vorzugsweise mit 18F-FDG PET.

    Description / Table of Contents: Zusammenfassung Von 1960–1983 wurden 423 Patienten mit Schilddrüsencarcinomen behandelt. Bei folliculären, medulldren und anaplastischen Carcinomen wurde die Thyreoidektomie angestrebt. Die Hemithyreoidektomie mit Keilexcision der gesunden Seite erfolgte beim papillären Carcinom mit einseitigem Befall. Nach Thyreoidektomie wurde die Hormontherapie durchgeführt. Anaplastische Carcinome wurden postoperativ extern bestrahlt. Die Zehnjahresüberlebensraten betrugen beim folliculären Carcinom ohne Metastasen 65%, mit Metastasen 30%, beim papillären Carcinom ohne Metastasen 90%. Nur 50% der Patienten mit medullarem Carcinom überlebten 5 Jahre. Beim anaplastischen Carcinom starben 80% innerhalb von 6 Monaten.
    Notes: Summary From 1960 to 1983, 423 patients with thyroid carcinoma were treated. In follicular, medullary, and anaplastic carcinomas, thyroidectomy was carried out where possible. Hemithyroidectomy with wedge excision of the healthy side was performed in unilateral papillary carcinoma. After thyroidectomy, hormone therapy was given. Anaplastic carcinomas were postoperatively irradiated externally. The 10-year survival rates were 65% in follicular carcinoma without metastases, 30% in follicular carcinoma with metastases, and 90% in papillary carcinoma without metastases. Only 50% of the patients with medullary carcinoma survived for 5 years. Among anaplastic carcinoma cases, 80% died within 6 months.
