Zusammenfassung In Skelettmuskelbiopsien von neun Patienten mit progressiver externer Ophthalmoplegie unklarer Ätiologie gelingt der Nachweis von „ragged red“ fibers. Dabei zeigt sich, daß das Auftreten dieser Faseranomalie nicht an das klinische Vorhandensein einer Skelettmuskelschwäche oder anderer Organmanifestationen des „ophthalmoplegia plus“-Syndroms gebunden ist. „Ragged red“ fibers finden sich selten als Zusatzbefund auch bei anderen neuromuskulären Erkrankungen, wobei jedoch eine Reihe sonstiger Strukturanomalien des Muskels vorliegt. Zahlreiche „ragged red“ fibers in einem weitgehend normalen Muskelgewebe stellen dagegen einen wesentlichen morphologischen Befund für die Diagnose eines „ophthalmoplegia plus“-Syndroms dar.

Summary In skeletal muscle biopsies of nine patients with progressive external ophthalmoplegia of unknown origin “ragged red” fibers can be demonstrated. The occurrence of these abnormal fibers does not correlate with clinical muscle weakness, or additional signs of the ophthalmoplegia plus syndrome. Rarely, “ragged red” fibers are seen in other neuromuscular disorders but usually these are associated with various structural alterations. Numerous “ragged red” fibers in an otherwise normal muscle provide an essential criterion in establishing the diagnosis of the ophthalmoplegia plus syndrome.
