Notizen: The effect of intravenous ergometrine 0.5 mg during general anaesthesia for Caesarean section on lower oesophageal sphincter (LOS) function was investigated in eight patients. Ergometrine increased the LOS pressure markedly (p 〈0.005).

Notizen: A cryogenic refrigeration scheme is presented which utilizes the discrete energy levels of quantum dots to customize the electronic Fermi–Dirac distribution, cooling a small but macroscopic reservoir to far below the ambient temperature. Several physical limitations of this scheme are discussed within the context of a model device constructed from a two-dimensional electron gas. These are explored in a simple example where, at an ambient operating temperature of 150 mK, this quantum-dot refrigerator can achieve a cooling power of 1 μW/cm2, a base temperature of 10 mK, and a cooling efficiency of 35%.

Notizen: Paecilomyces, a hyalohyphomycosis, is an uncommon cause of cutaneous and subcutaneous infections. Most cases reported have occurred in patients with impaired host defenses or following a surgical procedure, and have proven highly resistant to anti fungal therapies. A 60-year-old man presented with scattered non-healing verrucous lesions as well as ulcers and excoriations on the upper extremities and trunk. The lesion had begun shortly after the patient was scratched by prickly okra. The patient had no underlying health problems and he was on no medication. Histology revealed epidermal hyperkeratosis, acanthosis, with papillomatosis with intraepidermal micro-abscess formation. Within the dermis there was a mixed acute and chronic granulomatous infiltrate. With GMS stain numerous pleomorphic yeast forms and pseudohyphael forms were seen. Cultures confirmed Paecilomyces species. The patient was treated with Voriconazole with complete resolution of his lesion. Paecilomyces is an emerging fungal pathogen, which in our immune competent patient manifest as widespread cutaneous disease.

Notizen: Prompted by the continuing transition to community care, mental health nurses are considering the role of social support in community adaptation. This article demonstrates the importance of distinguishing between kinds of social support and presents findings from the first round data of a longitudinal study of community adaptation in 156 people with schizophrenia conducted in Brisbane, Australia. All clients were interviewed using the relevant subscales of the Diagnostic Interview Schedule to confirm a primary diagnosis of schizophrenia. The study set out to investigate the relationship between community adaptation and social support. Community adaptation was measured with the Brief Psychiatric Rating Scale (BPRS), the Life Skills Profile (LSP) and measures of dissatisfaction with life and problems in daily living developed by the authors. Social support was measured with the Arizona Social Support Interview Schedule (ASSIS). The BPRS and ASSIS were incorporated into a client interview conducted by trained interviewers. The LSP was completed on each client by an informal carer (parent, relative or friend) or a professional carer (case manager or other health professional) nominated by the client. Hierarchical regression analysis was used to examine the relationship between community adaptation and four sets of social support variables. Given the order in which variables were entered in regression equations, a set of perceived social support variables was found to account for the largest unique variance of four measures of community adaptation in 96 people with schizophrenia for whom complete data are available from the first round of the three-wave longitudinal study. A set of the subjective experiences of the clients accounted for the largest unique variance in measures of symptomatology, life skills, dissatisfaction with life, and problems in daily living. Sets of community support, household support and functional variables accounted for less variance. Implications for mental health nursing practice are considered.

Notizen: Many fish species undergo seasonal changes in distribution, as a result of horizontal migrations between feeding, nursery and spawning grounds. Exploring the processes involved in these movements may be the key to understanding interactions with other species, man and the environment, and is therefore crucial to effective fisheries management. Recent tagging experiments providing information on the distribution of migratory fish stocks have indicated pronounced regional and temporal differences in the migratory behaviour of cod, suggesting complex interactions between this commercially important fish species and the environment. This paper presents a model of the horizontal movements of demersal fish, principally cod, using an individual-based modelling approach to explore and predict the relationship between demersal fish movements and key environmental and ecological factors. The model simulates the basic biological processes of growth, movement and mortality, and is driven by the analysis of physical tagging data recorded by electronic data storage tags (DSTs). Results show that the incorporation of behavioural data from DSTs into spatially explicit individual-based models can provide realistic simulations of large-scale fish stocks, thus giving a better understanding of their basic ecology and allowing more effective management of commercially important fish species. Possibilities of future improvements and extensions to the model are discussed.
