    Description / Table of Contents: Zusammenfassung. Weichteildefekte der Extremitäten sind insgesamt selten und treten im Rahmen der Allgemeinchirurgie vor allem nach Störungen der Makro- und Mikrozirkulation an der unteren und nach Cytostaticatherapie, Tumorresektion und Radiatio an der oberen Extremität auf. In diesem Artikel werden die häufigsten Ursachen für komplexere Wunden sowie die gängigsten Lappenplastiken für die Defektrekonstruktion dargestellt und Entscheidungshilfen für eine patientenadaptierte Indikationsstellung gegeben.
    Notes: Summary. Soft tissue defects of the extremities are infrequent within the spectrum of general surgery and are predominantly found with perfusion disorders of macro- and microcirculation in the lower extremities. In the upper extremity, they are more frequently seen after i. v. cytostatic drug administration and tumor resections. This article describes the most frequent causes of complex defects of the extremities. Common flaps for the reconstruction of these defects and the pathway to decision making are elucidated.

    Notes: Abstract  Modern plastic and reconstructive surgery continues to significantly improve the functional and aesthetic post-burn sequelae in children. It has become a major factor in restoring mobility, aesthetic appearance, self-esteem, and quality of life. The entire spectrum of plastic reconstructive procedures should be available to offer the patient an optimal solution to his problems. Therefore, early interdisciplinary co-operation between pediatric and plastic surgeons is mandatory to plan and perform post-burn reconstruction in children and adolescents.

    Notes: Abstract Industrially manufactured foam rubber (polyurethane) is clinically used as variable, hygienic mattresses and also serves as a positioning aid during surgery.     Notes: Abstract  Functional results after deep excision and split thickness skin grafts are often limited as a consequence of unstable grafted areas and contractures. In two patients with a full-thickness thermal injury to the lower extremities and one patient with a chronic unstable skin area over the knee, the IntegraTM bilayered membrane dermal substitute was applied. After uneventful dressing changes graft take was complete without infection or other complications. Complete wound closure was achieved in all three patients within 3–4 weeks after the initial operation. The functional range of motion of the involved joints and the skin quality and contour was superior to conventional grafted skin after excision down to fascia and rather more comparable to skin grafts over a tangentially excised eschar. After wound healing, the neodermis was histologically similar to normal dermis. Considering the high incidence of unstable skin and contractures after regular grafting of deeply excised burns, this concept may present a significant improvement not only for the primary but also for secondary reconstructive procedures, with respect to the long-term quality of life for burn patients.
