Zusammenfassung Es wurden IgG-Antikörperbestimmungen gegen das HHV-6s, unten [1] im indirekten Immunfluoreszenztest mit Seren von 1105 Personen aus verschiedenen Altersstufen durchgeführt. Von diesen Seren wurden 570 mit HHV-6 Stamm St. W. (Prof.Schneweis, Bonn) nachgetestet. Wir konnten bestätigen, daß die mütterlichen Antikörper bis zum 6. Lebensmonat abnehmen und daß die Antikörperpositivrate zwischen dem 7. Lebensmonat und dem 5. Lebensjahr signifikant (auf 79,5%) ansteigt. Vom 6.–10. Lebensjahr liegt die Seropositivrate bei 81,3% und bis zum 40. Lebensjahr um 66%. Das humane Herpesvirus Typ 6 (HHV-6) ist als Erreger des Exanthema subitum (Roseola infantum) anerkannt. Wir konnten bisher bei acht von 14 Kindern mit typischem Roseola infantum eine Serokonversion für Anti-HHV-6-IgG-Antikörper nachweisen. In vier dieser Fälle gelang uns die HHV-6-Isolierung aus peripheren Blutlymphozyten in Gewebekultur und in sechs Fällen der Erregernachweis durch eine positive Hybridisierung. In Einzel- und einigen Folgeseren von Patienten mit akuten Exanthemerkrankungen Röteln (n=28), Parvovirus-B19/Ringelröteln (n=24), Masern (n=17), Mumps (n=27), Adenovirus (n=27), Parainfluenza Typ 3 (n=28) sowie in Seren von Patienten mit Kawasaki-Syndrom (n=20), akuter Varizellen-Zoster-(n=27), Herpes simplex-(n=18) und HIV-1-Infektion (n=50) fanden wir bei negativem Anti-HHV-6-IgM-Titer keine IgG-Titeranstiege und nur eine den festgestellten HHV-6-Durchseuchungsraten entsprechende Anti-HHV-6-IgG-Antikörperprävalenz. Seren von sechs Personen mit HIV-2-Infektion waren negativ für Anti-HHV-6-IgG-und-IgM-Antikörper. Wir stellten aber bei Patienten mit akuten Zytomegalie- (n=32) und Epstein-Barr-Virusinfektionen (n=25) in sechs von 32 bzw. 14 von 25 positiven IgM-Seren für das jeweilige Virus auch IgM-Antikörper gegen das HHV-6 fest. Es wurden dabei keine HHV-6-IgG-Titeranstiege gefunden, und die HHV-6-IgG-Seropositivrate entsprach den Werten in unserer HHV-6-Antikörperprävalenzstudie. Deshalb dürften die HHV-6-IgM-Antikörper bei Patienten mit akuter CMV- und EBV-Infektion durch Kreuzreaktionen oder polyklonale B-Zell-Stimulierung, wie sie bei akuten CMV- und besonders akuten EBV-Infektionen schon länger bekannt ist, bedingt sein (nicht publiziert,Enders et al.).

Summary We determined IgG antibodies against Human Herpesvirus-6 (strain Uganda 1102, M. D. Griffin, London) in the indirect immunofluorescence test [1] in sera from 1105 persons of various age groups. Of these sera 570 were retested using HHV-6 strain St. W. (Prof.Schneweis, Bonn). We could confirm that maternal antibodies decrease between birth and six months of age and the seropositive rate rises rapidly between seven months and five years of age up to 79.5%. Between six and ten years and up to 40 years, the antibody-positive rate lies around 81.3% and 66%, respectively. To confirm the causal nature of human herpes virus type 6 (HHV-6) for exanthema subitum we could demonstrate eight seroconversions testing sera from 14 patients with roseola infantum. In addition, the virus was isolated from peripheral blood lymphocytes of children during the acute fever phase in four cases in tissue culture and in six cases the virus was detected by positive hybridization. In single and some paired sera from patients with acute exanthematous diseases, rubella (n=28), parvovirus B 19 (n=24), measles (n=17), mumps (n=27), adenovirus (n=27) and parainfluenza virus type 3 (n=28) and in sera from patients with Kawasaki syndrome (n=20), acute varicella-zoster-(n=27), acute herpes simplex- (n=18) and HIV-1 infection (n=50), we found no HHV-6 IgM antibodies and no HHV-6 IgG antibody rises. We could only demonstrate an HHV-6 seropositive rate according to our age-prevalence study. In six patients with HIV-2 infection, neither HHV-6 IgG nor IgM antibodies were found. However, by investigating sera from patients with acute Cytomegalo- (n=32) and Epstein-Barr-virus infection (n=25), we detected anti-HHV-6 IgM antibodies in six of 32 CMV-IgM-positive sera and in 14 of 25 EBV-IgM-positive sera. There were no HHV-6 IgG antibody rises and HHV-6 antibody presence corresponded to our age-specific prevalence rate. Therefore, the presence of HHV-6 IgM antibodies in patients with acute CMV or EBV infection is considered to be due to cross-reactivity or polyclonal B-cell stimulation. This IgM crossreactivity has been observed previously in CMV and EBV infection (not published,Enders et al.).
