    Notes: Summary Brain scanning is of great value in detecting glioblastomas. Since there is little stress upon the patient by this method it is specially suitable for follow-up after surgical treatment and radiation therapy of this tumour, thus giving the best chance of an early detection of a recurrence. On the other hand angiography is essential if further surgery is necessary. Diagnostic problems may arise when using arteriography in cases of avascular recurrences and lack of spaceoccupying symptoms due to the following factors: Position and small size of the recurrence, growth into the operation cavity, and postoperative cerebral atrophy. Avascular mass lesions of other origin may mimic a tumour recurrence in the angiogram. Thus, a combined use of isotope studies and radiological investigations is necessary for an unequivocal detection of glioblastoma recurrences.

    Notes: Abstract Vesicoureteral reflux (VUR) was recognized neonatally by voiding cystography in 25 of 117 infants with a dilated fetal urinary tract. There was a male preponderance (76%) and a high percentage (40%) of associated urinary malformations. Thirtynine refluxing units were studied. All grades of VUR were detected but gross dilating VUR dominated (59%). Spontaneous resolution was excellent in lower grades of VUR but was poor in gross VUR. Surgery was successfully performed in 13 renal units of nine patients with gross reflux [8], additional ipsilateral malformations [4], or pyelonephritis during antibiotic prophylaxis [1]. Segmental renal scars developed in four kidneys after urinary infections, and a diffuse parenchymal lesion was noted in nine kidneys even at birth. One boy with duplication had a non-functioning refluxing system. Our results in a small number of infants show differences to children with VUR detected after urinary infections and seem to support the existence of a congenital reflux nephropathy.

    Notes: Abstract The accurate diagnosis of severe congenital ureteropelvic junction obstruction (UPJO) and the indications for surgical intervention are a matter of debate. Differential diagnosis from, for example, congenital cystic kidney malformations is important. In the present study we analysed the value of Doppler sonography (DS) for assessment of obstruction in congenital UPJO and its usefulness for differential diagnosis. A total of 138 infants and children were examined. Forty-three healthy infants and 24 children who had had a pyeloplasty at least 1 year previously served as control groups. Seventy-one children with UPJO and cystic kidney malformations underwent DS in addition to the usual examinations of intravenous urography and scintigraphy. Resistive Index (RI) was measured in both kidneys. Twenty infants showed decompensated UPJO and had needed an operation. Preoperatively all showed significantly increased RI (RI=74%), which returned to normal after operation (RI=68.3%). Thirty-nine children with UPJO could be managed conservatively, showing compensated obstruction on diuretic renography. On DS they had normal and symmetrical RI values (RI=67%). Eleven children had multicystic dysplastic kidneys; 4 underwent nephrectomy. On DS all showed either absence of perfusion signals or very low flow velocities with very high RI values (RI=90–100%) in the renal vessels. One child was found to be suffering from a cystic nephroma. DS was able to reveal quite normal vessel distribution in renal parenchyma with slightly elevated RI values (RI=80%). Our results indicate that DS a non-invasive, non-ionising method of investigation, may serve as an additional functional parameter for defining accurately the degree of obstruction in UPJO. DS can influence the indication for surgical treatment and improves the differential diagnosis of congenital cystic kidney malformations. It provides an adequate method for frequent monitoring in conservative treatment.

    Notes: Abstract Intestinal blood loss as well as chronic inflammation are regarded as the most important mechanisms in the pathogenesis of anemia in Crohn's disease. In addition, cytokines such as interleukin-6 can suppress erythropoietin production. This study was performed to investigate the importance of iron status, inflammatory activity, and endogenous erythropoietin concentrations for the development of anemia in Crohn's disease. In 49 consecutive patients with Crohn's disease, hemoglobin, inflammatory activity (Crohn's disease activity index, C-reactive protein, α1-acid glycoprotein), iron status (serum iron, transferrin, transferrin saturation, ferritin), and serum erythropoietin levels were studied. Anemic (Hb〈12.0 g/dl;N=16) vs nonanemic patients (Hb≥12 g/dl;N=33) showed reduced iron compartments (eg, ferritin 28.7±12.9 µg/liter vs 63.2±15.0 µg/liter, transferrin saturation 6.2±1.4% vs 11.5±1.3%,P〈0.01) but no differences in inflammatory activity. An inverse correlation between erythropoietin and hemoglobin concentrations was found (r=-0.62;P〈0.001), but the increase in erythropoietin levels was inadequate to the degree of anemia. There was no correlation between erythropoietin and interleukin-6 serum levels. Four of five anemic patients with hemoglobin below 10.5 g/dl and erythropoietin levels within the normal range were treated with parenteral iron (200 mg iron saccharate in 250 ml NaCl, weekly, intravenously). Two of them additionally received recombinant human erythropoietin (150 units/kg, 3× weekly, subcutaneously). After five weeks all patients had a marked increase in hemoglobin. However, the mean increase in erythropoietin-treated patients was 5.0 g/dl compared to 2.0 g/dl in the patients with iron therapy only. No side effects were seen. Our data demonstrate that inadequate erythropoietin production and iron deficiency are pathogenetic factors of anemia in Crohn's disease. The therapeutic management using recombinant human erythropoietin and parenteral iron is reasonable and effective.

    Notes: Summary The heart rate/work performance (f c/W) curve is usually S-shaped but a flattening at the top is not always seen. By means of radionuclide ventricular scintigraphy, the left ventricular ejection fraction (LVEF) of 15 sports students was investigated. The behaviour of the f c/W curve during cycle ergometry with increasing exercise intensities was examined. During exercise, the LVEF showed a distinct initial increase reaching roughly constant values at stress levels below-maximum, and sometimes even falling again. The inflections of the f c/W curve and left ventricular ejection fraction/performance curve (LVEFPC) were calculated from a second degree polynomial fit. From this function, the slopes of the tangents at the points of aerobic threshold and maximum performance were calculated together with the differences of the angles as a measure of the f c/W curve and LVEFPC inflections. It follows that the f c/W curve inflection became less pronounced or was even absent altogether when the decrease in LVEF towards the end of the ergometer exercise became more distinct. A significant negative correlation was found between the existence and extent of the f c/W curve inflection and the stress-dependent myocardial function, expressed as the inflection of the LVEFPC (P〈0.01, r=0.673). Thus, it would seem that the absence of a f c/W curve inflection was related to a diminished stress-dependent myocardial function.

    Notes: Abstract We report the preliminary results of a prospective study demonstrating tetrofosmin uptake in surgically and histologically proven parathyroid adenomas. In ten patients with primary chronic hyperparathyroidism, parathyroid imaging was performed using (1) technetium-99m methoxyisobutylisonitrile (MIBI) and (2)99mTc-1,2-bis(bis(2-ethoxyethyl)phosphino)ethane (tetrofosmin) within a time interval of 3–5 days. Both tracers correctly identified the parathyroid adenomas by focal prolonged tracer retention. On visual inspection image contrast was generally higher with MIBI than with tetrofosmin in all the patients studied. Tetrofosmin showed a slower elimination from the parathyroid adenomas than MIBI in six of the ten cases. Our preliminary results show that tetrofosmin, like MIBI, as a feasible, sensitive tracer for parathyroid scintigraphy. For routine use, the rapid kit preparation without heating and the lower radiation dose to the patient make tetrofosmin an alternative tracer for parathyroid scintigraphy. Further evaluation is needed to determine which of the two tracers is the more sensitive for the detection of parathyroid adenomas, and which tracer properties better reflect the degree of endocrine activity.[/p]

    Notes: Abstract A computer macro-program was developed for standardized, semiquantitative measurement of ossal radiotracer uptake. The method uses profiles of selectable length, width, and inclination according to the anatomical structures of the pathological and the corresponding healthy region. The ratio of the two curves is calculated automatically and shown as an ‘uptake curve’. The essential use of osteoscintimetry is in the follow-up examination if small changes of regional uptake have to be identified. The profile-technique rather than the ROI-technique was selected, because the main benefit of the profile-technique is that the results of follow-up examinations are comparable to previous examinations with a standard deviation of ±5%.

    Notes: Abstract The in vivo 99mTc-RBC labelling efficiency and stability of labelling was assessed after pretinning with a high-stannous content DTPA kit (Sn DTPA) in comparison with Sn-pyrophosphate (Sn PPi) and a low-stannous DTPA kit (DTPA). The distribution in Sprague Dawley rats showed that similar fractions of administered 99mTc remained within the blood pool after pretinning with Sn DTPA and Sn PPi when equal quantities of stannous ions (15 μg/kg) and equal time intervals (30 min) between successive IV injections of pretinning agent and 99mTc-pertechnetate were used. Significantly lower fractions were found when DTPA (1.9 μg Sn2+/kg) was used for pretinning. The rate of 99mTc elution emphasises the importance of the Sn2+ concentration used, not only for labelling efficiency but also for stability of the labelling. Satisfactory intravascular activity, exceeding 80% during the first hour post-injection, was demonstrated in three volunteers after 99mTc injection, when Sn DTPA was used for pretinning. Left ventricular ejection fractions (LVEF) measured by equilibrium radionuclide angiography angiography after pretinning with Sn DTPA in 24 patients correlated well (r=0.98) with those obtained by contrast angiographies over a broad spectrum of values (0.14–0.72). Four repeated LVEF measurements at 45-min intervals in six additional patients at rest showed excellent reproducibility in each patient: maximum variation was less than 6%.
