    Description / Table of Contents: Zusammenfassung Hypoxie führt beim Schwein zur Erhöhung des pulmonal-vaskulären Widerstandes (pvR). Diese hämodynamische Veränderung ist von einem Anstieg des intraparenchymatösen Spiegels von cyclischem GMP (cGMP) begleitet, während die Konzentrationen des cyclischen AMP (cAMP) im Vergleich zu Werten unter Normoxie unverändert sind. β-Sympathikomimetika sind potente Inhibitoren dieser hypoxie-induzierten Vasokonstriktion. Dieser Effekt ist mit einem Anstieg des intraparenchymatösen Spiegels von cAMP und einem Abfall des cGMP-Spiegels assoziiert. Die Regressionsanalyse zwischen dem pvR und dem Quotienten cGMP/cAMP erbrachte eine signifikante lineare Korrelation. Dieses läßt vermuten, daß cAMP und cGMP bei der hypoxie-induzierten pulmonalen Visokonstriktion und deren Aufhebung durch β-Sympathikomimetika kausal beteiligt sind.
    Notes: Summary Hypoxia leads to an elevation of the pulmonary vascular resistance (pvR) in pigs. This alteration of the hemodynamic was accompanied by a slight increase of intraparenchymatous concentrations of cyclic GMP (cGMP) while cycle AMP (cAMP) levels were not different from values, measured during normoxia. β-adrenergic agents are potent inhibitors of the hypoxia induced vasoconstriction. This effect was associated with an increase of intraparenchymatous cAMP-concentrations and a decrease of cGMP-levels. The regression analysis between pvR versus the ratio cGMP/cAMP revealed a significant linears correlation. These data may indicate, that cAMP and cGMP are involved in hypoxia induced vasoconstriction and its pharmacological inhibition.

    Description / Table of Contents: Zusammenfassung Die licht- und elektronenmikroskopischen Befunde an den Lungentumoren eines Falles mit multiplen pulmonalen Leiomyomen werden beschrieben. Die Differential-Diagnose der leiomyomatösen Lungentumoren und besonders die Abgrenzung von der pulmonalen Lymphangiomyomatose wird besprochen. Multiple pulmonale Leiomyome werden in der Literatur als Metastasen von “low grade” uterinen Leiomyomen oder aber als Hamartome der Lunge betrachtet. Letztere Annahme beruht auf der Tatsache, daß auf der Oberfläche der Tumoren und in ihrem Inneren sich kubisches, drüsenartiges Epithel befindet. Dieses weist jedoch nach unseren Beobachtungen Merkmale der granulären Pneumocyten (Typ II) auf, insbesondere lamelläre Körper im Zytoplasma und Microvilli an der Oberfläche. Die Ausbildung dieser epithelialen Bedeckung wird deshalb als Reaktion des Alveolarepithels auf das Tumorwachstum aufgefaßt. Als mögliches Muttergewebe der multiplen pulmonalen Leiomyome wird das kontraktile System der Lungenacini (kontraktile interstitielle Zellen) diskutiert.
    Notes: Summary The light and electron microscopical features of the lung tumors in a case of multiple pulmonary leiomyomas are described. The differential diagnosis of leiomyomatous tumors of the lung is discussed. They have to be differentiated from lymphangio-leiomyomatosis of the lungs. In the literature, multiple pulmonary leiomyomas are generally considered to be metastases from low grade uterine leiomyosarcoma or to be hamartomatous lung tumors. This is suggested by the glandular structures both within the tumor and on the surface. However, our ultrastructural observations showed these epithelia to have features of granular pneumocytes (type II), in particular they contain lamellar bodies and posess microvilli on their surface. Their formation is considered to be a secondary reaction of alveolar lining cells to tumor growth. A possible origin of multiple pulmonary leiomyomas from the contractile system of the lung acini (contractile interstitial cells) is discussed.

    Description / Table of Contents: Zusammenfassung Bei drei Kindern, 6, 11 und 14 Jahre, einer sonst nicht auffälligen türkischen zehnköpfigen Familie wurde eine Hyperlipoproteinämie Typ I nach Frederickson gefunden. Initial betrugen die Triglyceridwerte des milchig-weißen Serums 1780 bis 3750 mg/dl, Serum-Cholesterin war nicht erhöht. In der Lipoproteinelektrophorese auf Agarose- und Acrylamidgel stellten sich eine ausgeprägte Bande der Chylomikronen, jedoch keine der HDL dar, die post-heparin lipolytische Aktivität war auf 30% der Norm erniedrigt. Bei fettreicher (50% der Gesamtkalorien) Kost konnten Chylomikronen induziert werden, Kohlenhydrate hatten diesen Effekt nicht. Bei fettarmer (5%) Kost verschwanden die Chylomikronen während der stationären Behandlung, die Triglyceridwerte fielen auf etwa 450 mg/dl. Anhalt für Phänokopien wurde nicht gefunden. Als einzige Familienmitglieder waren alle drei Patienten zu klein, die Körpergröße lag bei allen unter der dritten Perzentile. Wiederum hatten nur die Patienten ein retardiertes Knochenalter, minus 18 bis 30 Monate. Es fand sich jedoch kein Hinweis für eine endokrinologische Ursache der Wachstumsverzögerung: Bei 4 verschiedenen Stimulationstests normaler Anstieg des Serum-STH, Schilddrüsen- und Nebennierenrindenfunktionen waren unbeeinträchtigt, die Sexualentwicklung war altersentsprechend. Während der intravenösen und der oralen Glukosebelastung wurde eine erhöhte Glukoseassimilation gefunden. Jedoch waren die Serum-Insulinwerte während dieser Stimulationstest im Normbereich. Die Möglichkeit, daß die angeborene Hypertriglyceridämie eine kausale Rolle bei der Entstehung der Wachstumsverzögerung spielt, muß diskutiert werden.
    Notes: Abstract Familial hypertriglyceridemia or hyperlipoproteinemia type I was detected in three siblings aged 6, 11, and 14 of an otherwise normal Turkish family of 10 members. Initial values ranged from 1780 to 3750 mg/100 ml triglycerides in the milky white serum; cholesterol was normal. Lipoprotein pattern on agarose and acrylamide gel revealed a heavy band of chylomicrons and missing HDL; post-heparin lipolytic activity was decreased to about 30% of normal. Chylomicronemia could be induced by a fat-rich (50% of total calories) diet, but not by carbohydrates. On a low fat diet (5%) during hospitalization chylomicrons disappeared, and triglycerides decreased to about 450 mg/100 ml. Phenocopies of hypertriglyceridemia could be excluded. All three patients were the only members of the family who were small, below the third percentile. Their bone age was retarded from 18 to 30 months. There was no indication for an endocrine cause of the growth retardation: four different stimulation tests revealed normal growth hormone response, thyroid and adrenal functions were not impaired; sexual development was normal. Increased glucose assimilation was observed during intravenous and oral glucose load. Peak serum insulin response was above normal during stimulation tests. The possible etiologic role of hypertriglyceridemia in this concomitant growth retardation is discussed.

    Notes: Abstract During normoxia and acute alveolar hypoxia the pulmonary vascular-resistance, histamine concentrations in the lung parenchyma and partially in the arterial blood, and cAMP levels in the lung tissue of young pigs were measured. A significant correlation between the pulmonary vascular resistance and the lung or blood histamine concentrations was not found during either normoxia or hypoxia. The influence of terbutaline and meclastine on the pulmonary vascular resistance and biochemical parameters was also studied. Terbutaline significantly reduced the hypoxic pulmonary vascular response. An inverse relation between the pulmonary vascular resistance and the cAMP levels during the drug induced inhibition of the Euler-Liljestrand mechanism was found. Meclastine had no influence on the hypoxic pressor response.
