    Notes: Abstract The paper describes a computer-assisted infusion and nutrition programme, running on an IBM-compatible PC/XT or PC/AT, for a surgical intensive care unit specializing in burn treatment. Using inputs of patient and laboratory data relevant to infusion planning, the programme generates a schedule for balanced enteral or parenteral nutrition of adult and adolescent patients from the age of 14 upwards. Potential side effects and metabolic complications in nutrition therapy are minimized. Graphics provide a rapid status check on individual parameters whenever required.

    Notes: Summary Hypersensitivity vasculitis is an autonomous form of this disease, attacking the minor vessels. It manifests itself either as an idiopathic condition or in secondary, symptomatic form following severe illnesses or as an oversensitivity reaction to certain drugs. Distinctions are drawn between a local, a systemic and a simultaneously local and systemic progression. The problems of diagnosis and therapy are described for a complicated case in the acute generalization phase. Successful treatment of this severe form of the disease requires close cooperation between the internal specialist and the plastic surgeon.

    Description / Table of Contents: Zusammenfassung Trotz der heute üblichen intensivtherapeutischen Maßnahmen überleben schwerverbrannte Patienten die initialen Phasen der Verbrennungskrankheit nur, um später an septischen Komplikationen zugrunde zu gehen. Es wird aufgrund vielfältiger Untersuchungen angenommen, daß erworbene Immundefizienzen dieses erhöhte Risiko einer septischen Komplikation bedingen. Offenbar führt die thermische Gewebsschädigung zu Störungen in der komplizierten Koordination cellulärer Immunreaktionen, wobei suppressiv wirkende humorale Mediatoren entweder eine Hemmung cellulärer Funktionen verursachen oder eine überstimulation in regulativen Mechanismen zur Folge haben.
    Notes: Summary Despite of modern fluid resuscitation, electrolyte management and respiratory care techniques 70% of severely burned patients often survive the early posttraumatic phase only to later succumb to septic complications. Acquired deficiencies in host defense mechanisms are largely held responsible for this increase in susceptibility to sepsis. The thermal insult affects the complex interactions of cells of the immune system by generation and release of suppressive mediators which, in turn, either cause defective cellular functions or overstimulation of regulatory pathways of host defense.

    Description / Table of Contents: Zusammenfassung Prinzip der Organkonservierung in der Transplantationschirurgie ist die Stoffwechselreduzierung durch Hypothermie. Nephrotoxische Medikamente wie Cyclosporin A scheinen aber die maximale Ischämiezeit zu verkürzen. Im eigenen Zentrum werden zwei Methoden verwendet, um Schäden durch die warme Ischämie zu vermeiden: In situ Perfusion mit ‚en bloc' Entnahme - Kühlung bei der Implantation. Letzteres wird durch Verwendung eines 3-Kammer-Beutels erreicht, in dem die Niere, umgeben von eisgekühlter Lösung, implantiert wird. Wir empfehlen diese einfache Technik, um die warme Ischämie möglichst zu vermeiden.
    Notes: Summary Metabolic inhibition, induced by hypothermia, is the principle of organ preservation in transplantation. Nephrotoxic drugs, such as Cyclosporin A appear to reduce the maximum ischemia time. In our center, two methods are introduced into the surgical procedure in order to avoid damage by warm ischemia: in situ perfusion with en bloc dissection and cooling during implantation. The latter is carried out using a three-chamber bag, which is filled with chilled saline and which surrounds the organ during implantation. We recommend this simple technique in order to minimize warm ischemia time.

    Description / Table of Contents: Zusammenfassung Cyclosporin (CyA) verschlechtert die Nierenfunktion nach Ischämieschädigung. 55% aller Nierentransplantierten unter CyA-Therapie mußten dialysiert werden gegenüber 33% der konventionell therapierten Gruppe. Die Dialysedauer war außerdem unter CyA verdoppelt. Um Schäden durch die Ischämie zu verringern werden Nieren nach in situ ‘Perfusion en bloc’ entnommen und unter Verwendung eines speziellen Beutels während der Transplantation gekühlt. Potentiell nephrotoxische Medikamente werden vermieden, intraoperativ wird der ZVD auf +8 H2O und RR auf ca. 160 mmHg gehalten. Das Verfahren soll die primäre Funktionsrate der Transplantate verbessern.
    Notes: Summary An additional detrimental effect can be seen in renal function that is caused by CyA treatment following ischemic injury: 55% of all transplant patients under CyA and 33% under conventional treatment underwent dialysis. Furthermore, the duration of dialysis was prolonged in CyA patients by 100%. To avoid ischemic damage, the kidneys were removed en bloc after in situ perfusion. In addition, the kidneys were cooled during implantation using a newly developed bag. Potentially nephrotoxic drugs were avoided. The blood pressure was kept at approximately 160 mmHg and central venous pressure above + 8 cm H2O. This regimen may help to improve the primary function of renal transplants.
