    Notes: Venepuncture for routine blood sampling is a very distressing experience for a considerable number of children. Not only do they express high levels of distress during venepuncture but also in anticipation of the procedure. Therefore, prevention or reduction of distress should focus on both phases of the procedure. To this end, three preparation elements were combined: local anaesthesia of the skin, provision of sensory and procedural information, and involvement of the parent. In order to test the effect of this integrated procedure on the distress reactions of young children before as well as during venepuncture, 31 children were randomly assigned to one of two conditions: preparation or no preparation. Independent raters, who were blind to group assignments, scored segments of the videotaped behaviour of the children, according to the Groninger Distress Scale. Prepared children displayed significantly less distress before and during venepuncture than not-prepared children, regardless of their gender, ethnical origin, age, injection history, and the tension of their parent.

    Notes: Mice from four different inbred strains were infected with live Mycobacterium tuberculosis and the immune response to M. tuberculosis was followed for 24 weeks, using Western blotting. Nearly all mice, irrespective of H-2 type, reacted with the 3S-kDa protein band. Antibodies against this secreted 38-kDa protein were ihe first to appear, 4 weeks after infection. Thereafter the secreted 19-kDa protein and non-secreted antigens, such as the 65-kDa and 33-kDa proteins, were recognized. The immune response against the non-secreted antigens was influenced by the mouse strain. However, the 33-kDa protein band was recognized by all mouse strains after a second injection wilh live M. tuberculosis.The specificity of the antibodies was analysed in Western blot using sonicates of M. tuberculosis, M. kansasii, M avium. M. terrae, M. gordonae and Escherichia coli. Antibodies against the 38-kDa and 33-kDa protein bands seemed to be specific for M. tuberculosis, while antibodies against the 19-kDa protein band showed limited cross-reactivity. Antibodies against the 65-kDa prolein were strongly cross-reactive.These results suggest that the 38-kDa protein is secreted in vivo and, therefore, may be available to the humoral immune system at an early stage of infection. The non-secreted 33-kDa protein is only recognized by all mouse strains afler prolonged contact with M, tuberculosis.

    Notes: Two different groups of monoclonal antibodies (MAbs) directed to different epitopes on 30-kDa region proteins of Mycobacterium tuberculosis were isolated; MAbs 5F9, 5D5 and 5D2 reacted with a single 33-kDa protein band, whereas MAb 3A8 readied with A distinct 29,33-kDa doublet when analysed by immunoblotting. The present paper describes the distribution of MAbs defined epitopes in the 29 33-kDa region proteins in well-characterized subcellular fractions: cytosol, plasma membrane, cell wall as well as culture filtrate of M. tuberculosis. MAbs 5F9, 5D5 and 5D2 reactive epitopes were found in cytosol, whereas 3AS epitope is distributed in all cellular compartments of the mycobacterium as well as in the culture filtrate. Localization of these epitopes by indirect immunoflourescence and immunogold-labelling demonstrated that only 3A8 epitope is present on the cell surface of the mycobacterium. Both immunoblotting and EL[SA showed that only MAh 3A8. and not 5F9. 5D5 and 5D2, reacted with secreted BCG 85 antigen complex of Mycobacterium bin-it BCG. Furthermore, using an MAb 3AS-coupled affinity column, we purified antigen 3A8 from the cytosol fraction of M tuberculosis. All these MAhs reacted with antigen 3AK with varying degrees of intensity, thus suggesting that they are directed to a single protein. Absence of 5F9, 5D5 and 5D2 epitopes in the cell wall, culture filtrate and BCG-85 complex suggests that these epitopes might have been lost during the processing of the same 33-kDa protein on its way out from cytosol to the cell wall or when the protein is secreted out into the culture filtrate. Our results demonstrate, for the first time, direct evidence of the presence of a 30-kDa region protein not only in secreted antigens but also in the cell wall and on the cell surface of the mycobacterium.

    Notes: By screening a Mycobacterium lepraeλgt11 genomic DNA library with leprosy-patient sera we have previously identified 50 recombinant clones that expressed novel M. leprae antigens (Sathish et al., 1990). In this study, we show by DNA sequencing and immunoblot analysis that three of these clones express a M. leprae homologue of the fibronectin-binding antigen 85 complex of mycobacteria. The complete gene was characterized and it encodes a 327-amino-acid polypeptide, consisting of a consensus signal sequence of 38 amino acids followed by a mature protein of 289 amino acids. This is the first sequence of a member of the M. leprae antigen 85 complex, and Southern blotting analysis indicated the presence of multiple genes of the 85 complex in the genome of M. leprae. The amino acid sequence displays 75–85% sequence identity with components of the antigen 85 complex from M. tuberculosis, M. bovis BCG and M. kansasii. Furthermore, antibodies to the antigen 85 complex of M. tuberculosis and M. bovis BCG reacted with two fusion proteins containing the amino acid regions 55–266 and 265–327 of the M. leprae protein. The M. leprae 30/31 kDa protein induces strong humoral and cellular responses, as judged by Western blot analysis with patient sera and proliferation of T cells derived from healthy individuals and leprosy patients. Amino acid regions 55–266 and 265–327 both were shown to bind to fibronectin, indicating the presence of at least two fibronectin-binding sites on the M. leprae protein. These data indicate that this 30/31 kDa protein is not only important in the immune response against M. leprae, but may also have a biological role in the interaction of this bacillus with the human host.

    Notes: A variety of methods have been used to identify Mycobacterium spp. isolated from snakehead and Siamese fighting fish, including biochemistry, mycolic acid profiles and antibody-based methods. However, these methods are unable to differentiate between different species of Mycobacterium. Polymerase chain reaction (PCR) followed by reverse cross blot hybridization (RCBH) was adapted in this study to speciate aquatic mycobacteria. The method was highly specific for Mycobacterium spp. and identified the bacteria to species level with a detection limit of 100 fg DNA, equivalent to 20 mycobacteria. Twenty-nine isolates previously collected and cultured from Siamese fighting fish (10 isolates) and snakehead (19 isolates) during outbreaks of mycobacteriosis were analysed using PCR–RCBH. Six of the Siamese fighting fish isolates and nine of the snakehead isolates were identified as Mycobacterium fortuitum, while the remainder were classified as M. marinum. Notably, two isolates recovered from snakehead and Siamese fighting fish, previously identified as M. poriferae and M. piscicida, respectively, were confirmed to be M. fortuitum.

    Notes: Mycobacterium tuberculosis antigens inducing species-specific immune responses are likely to be particularly important for serodiagnosis or for skin testing of tuberculosis. In the present study, we describe the characterization of two novel monoclonal antibodies (MoAbs) A3h4 (IgG2a) and B5gl (IgM) that are directed to M. tuberculosis 27-kDa and 25-kDa proteins respectively. Specificity analysis by immunoblotting using 20 different species of mycobacterial sonicates revealed that MoAb A3h4 was specific for M. tuberculosis complex alone while MoAb B5gl showed a limited cross-reactivity. Direct comparison with previously characterized MoAbs revealed that these MoAbs A3h4 and B5gl defined new antigenic determinants of M. tuberculosis. By using M. tuberculosis complex-specific MoAb A3h4 we have identified a distinct 27-kDa protein in the M. tuberculosis H37Rv culture fluid. Since this MoAb did not bind to the previously characterized MPT44, MPT59, MPT45, MPT51 and MPT64 proteins as well as the 23-kDa superoxide dismutase (SOD) protein of M. tuberculosis, we conclude that MoAb A3h4 recognizes a novel protein in the M. fuberculosis H37Rv culture fluid. Studies of the subccllular distribution of these MoAb-reaetive proteins indicate that the MoAb A3h4-reactive 27-kDa protein is present not only in the culture fluid bnt also in the cytosol and the cell wall of M. tuberculosis. By contrast. B5gl-reactive protein is mainly a cytosolic protein. When these MoAbs were tested in a previously established ELISA with intact mycobacteria derived from early cultures, only MoAb A3h4 showed the positive reactivity to mycobacteria belonging to the M. tuberculosis complex.In addition, during the present comparative studies of MoAbs we have also found that the previously described MoAb F116-5, which is known to recognize the mycobacterial 23-kDa SOD protein [17], cross-reacted with the MPT44, MPT59, MPT45 and MPT51 secreted proteins but not with MPT64 and MPB70. These findings indicate that the family of four secreted proteins of M. tuberculosis share a common epitope with M. tuberculosis SOD protein.

    Description / Table of Contents: Summary Magnetic resonance imaging (MRI) assessment of traumatized temporomandibular joints (TMJ) usually focuses on disc position, defining regular joint function by normal, excentric or displaced disc position. So far, there are only few reports regarding disc position after open reduction of diacapitular or high condylar fractures of the TMJ with dislocation. The aim of the present study was to evaluate the role of the disc as regards postoperative functional outcome by electronic axiographic recordings of condylar movements and MRI, displacement of the disc and lesions of TMJ soft tissues being frequent in this type of mandibular fractures. A total of 30 subjects with 37 condylar fractures in whom osteosynthesis was performed using a preauricular approach were imaged postoperatively (mean 24 months) with a 1.5-Tesla MRI system to determine, (a) the position of the disc, (b) the range of mobility of the disc and (c) condylar mobility in closed and open mouth position, comparing fractured sides (FS) vs nonfractured sides (NFS). Linear movements between the two jaw positions in the sagittal plane were measured by superimposing transparencies. The results indicate: (1) more than 70% of the discs (FS) were found to be in normal position; there was no disc displacement without reduction. However, these data stood in contrast to severe limitations of the axiographic tracings as presented by almost 30% of the subjects. (2) Significant correlations were found between fixed (α = 0.05) or highly immobilized (α = 0.01) discs and axiographic limitations, suggesting disc mobility to be a valuable parameter for assessment of the postoperative functional outcome.
    Notes: Zusammenfassung Bei diakapitulären Frakturen bzw. hohen Kollumluxationsfrakturen mit Beziehung zum Lig. laterale sind Verlagerungen des Diskus häufig und werden nach konservativer Therapie mit einer Häufigkeit zwischen 50 und 100% angegeben. Informationen über die Diskusposition nach operativer Versorgung dieser Frakturgruppen liegen bisher nicht vor. Ziel der vorliegenden Studie war es, die Bedeutung von Position und Mobilität des Diskus für das postoperative funktionelle Ergebnis dieser Frakturgruppen zu klären. Die Objektivierung der operativen Ergebnisse bei 30 Patienten mit 37 über einen präaurikulären Zugang versorgten Gelenkfrakturen erfolgte mittels elektronischer Achsiographie und Kernspintomographie (1,5-T-System), im Mittel 24 Monate postoperativ. Erhoben wurden die Position und der Bewegungsumfang des Diskus bzw. des Kondylus im Seitenvergleich. Die linearen Bewegungen zwischen mundoffener und -geschlossener Position wurden durch Folienüberlagerung korrespondierender sagittaler MRT-Schichten ermittelt. Die Ergebnisse zeigten zum einen, dass 〉 70% der Disci auf der Frakturseite orthotop (Position A) lagen und keine fixierten anterioren Dislokationen (C) auftraten. Die achsiographischen Befunde zeigten in Diskrepanz dazu bei immerhin 30% der versorgten Gelenke höhergradige Limitationen der Exkursionsbahnen. Zum anderen bestehen signifikante Zusammenhänge zwischen fixierten (α = 0,05) bzw. hochgradig immobilisierten (α = 0,01) Disken und achsiographisch erfassten Limitationen der Translationsbewegung. Im Gegensatz zu bisherigen Studien sollten nach der operativen Versorgung von Gelenkfrakturen nicht nur die (statische) Diskusposition, sondern in erster Linie die Diskusmobilität als Parameter für das funktionelle Ergebnis berücksichtigt werden.

    Description / Table of Contents: Conservative treatment of TMJ condylar fractures (groups V and VI according to Spiessl and Schroll 1972) often shows poor clinical results. This follow-up study was designed to objectify the benefits of an operative procedure by electronic computer-assisted recording of condylar movements. Sixty-three individuals with a total of 77 condylar fractures (42 diacapitular fractures, 35 high condylar process fractures) underwent surgery over a preauricular approach. Osteosynthesis was performed according to the intraoperative site either by PDS pins, titanium lag screws, microplates, miniplates or a combination of these materials. Radiological controls showed a considerable loss of height of the condylar process in the diacapitular group of fractures affecting the lateral condylar pole (n = 20). Condylar deformations in these cases were similar to the high condylar process group, whereas diacapitular fractures not affecting the lateral condylar pole (n = 22) had better radiological results. Axiographic tracings of TMJ movements in all groups as well as clinical functional analysis demonstrated highly satisfactory functional results within physiological limits. Compared to data referring to individuals treated conservatively, far fewer irregularities of axiographic tracings were observed in this group. Limitations of condylar movements nevertheless were found frequently in the high condylar process group (35%). As a severe complication, permanent facial nerve palsies occurred in six individuals due to the preauricular approach employed. After a modification using a temporal extension, only one minor partial lesion occurred in 97 operations.
    Notes: Die Ergebnisse der konservativen Therapie von Frakturen der Gelenkwalze und hohen Gelenkfortsatzluxationsfrakturen können nur in wenigen Fällen als wirklich befriedigend bezeichnet werden. Ziel der vorliegenden Untersuchung war es, die funktionellen Ergebnisse eines operativ versorgten Kollektivs klinisch und elektronisch-achsiographisch zu objektivieren. 63 Patienten mit 77 Frakturen der Gelenkwalze bzw. des hohen Gelenkfortsatzes wurden über einen präaurikulär-diaartikulären Zugang operativ versorgt. Zur Osteosynthese wurden je nach Operationssitus PDS-Pins, Stellschrauben, Mikro- und Miniplatten, ggf. in Kombination, verwendet. Die Gruppe der diakapitulären Frakturen mit Beteiligung des lateralen Kondylenpols (n = 20) zeigte postoperativ in den radiologischen Kontrollen die deutlichsten Höhenminderungen, der Deformierungsgrad des Köpfchens entsprach dem der hohen Gelenkfortsatzfrakturen (n = 35). Die Gruppe der diakapitulären Frakturen mit Erhalt des lateralen Kondylenpols (n = 22) zeigte die geringsten Deformierungen. Die mittels elektronischer Achsiographie ermittelten Funktionswerte ergaben überwiegend zufriedenstellende Gelenkfunktionen, mit im Vergleich zur konservativen Therapie deutlich harmonischeren Kurvenverläufen. Höhergradige Limitationen der Gelenkbahnen konnten v. a. bei den hohen Gelenkfortsatzfrakturen beobachtet werden (35%). Schwerwiegendste Komplikation waren 6 permanente Läsionen des N. facialis, die initial hohe Läsionsrate konnte aber durch Modifikation des Zugangs mit einer temporalen Entlastung auf 1% gesenkt werden. In Hinblick auf die günstigen klinischen und funktionellen Ergebnisse, insbesondere unter weitgehendem Erhalt der Translationskomponente, sehen wir die Indikation zu einer operativen Versorgung bei Luxationen bzw. Dislokationen des kleinen Gelenkfragments und Höhenverlust des Gelenkfortsatzes gegeben. Die Compliance der Patienten für eine funktionelle Nachbehandlung sollte dabei gewährleistet sein.

    Notes: Abstract The feasibility of using the synthetic sex pheromone, disparlure, as a mating confusant of the nun moth (Lymantria monacha L.) was investigated on wild populations occurring in outbreak proportions in Poland and in low density in Czechoslovakia. The size of experimental plots ranged from 1 to 12 hectares. The amounts of pheromone applied were 10 or 20 g/hectare. Three types of slow-release formulations of a racemic mixture of disparlure were tested: (1) a spray formulation with latex as a carrier, (2) a tubing of natural rubber, and (3) sawdust of a porous material used for making tips of fiber-tip pens. All three pheromone formulations prevented significant numbers (98.6–100% in the low density situation, 90–96.2% during the outbreak) of the males from locating a discrete source of pheromone (a trap containing synthetic pheromone or a female) during the whole flight period, indicating long-lasting efficiency of the formulations.

    Notes: Abstract In 191 Tanzanian patients admitted to hospital with suspected extrapulmonary tuberculosis (TB), TB was diagnosed in 158 patients; the remaining 33 patients had neither microbiological nor clinical evidence of TB.Mycobacterium tuberculosis was detected in the blood of 25 patients, in 92% by a polymerase chain reaction (PCR) technique and in 52% by culture of buffy coat cells. The presence of mycobacterial DNA orMycobacterium tuberculosis bacteria in peripheral blood (positive culture) was significantly associated with HIV infection; it was detected in 22 (21.4%) of 103 HIV-seropositive patients compared to only 3 (3.5%) of 55 HIV-seronegative patients (p〈0.009). In two-thirds of the patients with mycobacteraemia, TB can be detected by simple smears from other organ sites. In patients with suspected extrapulmonary tuberculosis in whom smears from the infected site are negative or not available, PCR on blood will confirm the diagnosis within 24 hours in one third of the cases.
