    Notes: Abstract Fifteen men and six women, ranging in age from 20 to 79 years, with malignant fibrous histiocytoma comprised this series. Amputation of the right lower extremity was performed in one patient, wide resection in 7 and marginal excision in 9, respectively. In the other 4 patients, a non-curative resection was carried out. Adjuvant chemotherapy was prescribed post-operatively for 12 patients. Histologic grade of the surgical specimens was Stage I in 3, II in 6, III in 8 and IV in 4. Among fourteen patients living from 10 months to 9 years and 9 months after the operation, the 7 subjected to a wide resection are all disease free. Seven patients died of a local recurrence or a distant metastases. The survival rate of the patients with Stages I, II is significantly higher than those with Stages III, IV. These results show that a wide resection is to be favored and that staging is useful to estimate the prognosis.

    Notes: Abstract Using an ultrasonic probe inserted into the mediastinum during cervical mediastinoscopy, mediastinal ultrasonography (USM) was performed on 63 patients with lung cancer. The patients with a small peripheral mass of less than 2 cm in diameter, according to the chest X-ray results, and with mediastinal lymph nodes smaller than 1 cm in their short axes as determined by computed tomography (CT), were excluded from this study. An analysis of the areas under the receiver operating characteristic curves derived from CT and USM showed that USM was superior (P=0.043) to CT in terms of the diagnosis for mediastinal lymph node metastases, when the short axis dimension of mediastinal lymph nodes was employed for the diagnosis of metastases. The reason for this is that 97% of the mediastinal lymph nodes imaged by USM were located vertically along the body axis of the patient, and hence USM imaged the true short axis of the node in many cases. Our results indicate that USM is useful for performing a safe biopsy of lymph nodes during mediastinoscopy as well as for obtaining a clear imaging of the subcarinal nodes, which are inaccessible by normal cervical mediastinoscopy.

    Notes: Abstract The surgical management of patients with concomitant critical coronary artery disease (CAD) and surgically resectable cancer is controversial. We evaluated 19 patients who underwent concomitant coronary artery bypass grafting (CABG) and curative operation for cancer of the stomach in 9 patients, the colon in 4, the lung in 4, and the breast in 2. Each cancer operation was performed under stable hemodynamics without any serious bleeding tendency, immediately after CABG with an average of 2.5±0.8 grafts. There were no operative deaths and no incidences of perioperative myocardial infarction. Postoperative complications developed in three of the patients with lung cancer: respiratory dysfunction caused by phrenic nerve paralysis in two and mediastinitis in one. During the mean follow-up period of 33±23 months, 5 patients died of recurrent cancer or non-cardiac disease; however, all 19 patients remained free from any postoperative cardiac events and their quality of life apparently improved. This experience suggests that such simultaneous correction would be safe and beneficial in carefully selected patients who have surgically correctable CAD and potentially curable cancer.

    Notes: Abstract We treated a patient with a benign mediastinal teratoma associated with Klinefelter's syndrome which was first thought to be a malignant germ cell tumor because of an anterior mass shadow, gynecomastia, atrophic testis and a high level of HCG. Chromosomal analysis showed a total of 47 XXY. Median sternotomy revealed a tumor originating from the right lobe of thymus. The postoperative course and 2 year follow-up have been uneventful.

    Notes: Abstract Sixty-six patients with primary lung cancer who underwent thoracotomy were studied to determine the correlations among133Xe radiospirometry, surgical procedures and histological extension of the lung cancer. Disturbance in the regional perfusion ( $$\dot Q$$ per cent) was more prominent than disturbance of the regional ventilation ( $$\dot V$$ per cent), as the pathological stage and t factor proceeded, while $$\dot V$$ per cent and $$\dot Q$$ per cent were disturbed almost equally in relation to the pathological n factor. Lobectomy was impossible in patients with a $$\dot Q$$ per cent of less than 33 per cent of the total, but low perfusion did not necessarily contraindicate surgery. The predicted post-operative FEV1.0 was calculated according to the equation of (1-b/a × ( $$\dot V$$ per cent or $$\dot Q$$ per cent)) × (preoperative FEV1.0), where a and b were the number of subsegments in the lung lobes on the involved side and the resected lobe. The predicted and actually measured postoperative FEV1.0 showed significant correlations (〈0.001) in both equations. We conclude that $$\dot Q$$ per cent reflects a complex pattern of lung cancer spread more sensitively than does $$\dot V$$ per cent, and the significance of $$\dot V$$ per cent and $$\dot Q$$ per cent in terms of prediction of postoperative EFV1.0 seems to be equivocal.

    Notes: Abstract Lungs of 3-methylcholanthrene-treated rabbits and those of untreated rabbits were digested with protease, and the cells were separated into an alveolar type II cell-rich fraction and a Clara cell-rich fraction by the use of counter-flow centrifugal elutriation. Drug oxidation activities of the cell homogenate of each fraction were examined. Marked elevation of ethoxyresorufin O-deethylase activity was found mainly in the cell homogenate of the alveolar type II cell-rich fraction after 3-methylcholanthrene treatment. The activity increased 6-fold with the drug treatment. Coumarin hydroxylase activity was found mainly in the cell homogenates of the Clara cell-rich fraction of the 3-methylcholanthrene-treated and untreated rabbits. The activity increased 3.5-fold with the drug treatment.

    Notes: Summary The thymus in patients with myasthenia gravis has germinal centres and the thymic lymphocytes spontaneously produce anti-acetylcholine receptor antibody in vitro. This suggests indirectly that the myasthenic thymus contains mature T and B cells that are autoreactive. We report a two colour flow cytometric analysis of the surface antigen phenotype of lymphocytes in the thymus and in thymoma in 24 patients and in 14 normal thymuses. The results show a decline in the proportion of double positive immature thymocytes with age. This process seems to be partially disrupted in patients with myasthenia gravis. In addition titres of anti-acetylcholine receptor antibody were positively correlated with the proportion of mature single positive T cells and non-T DR positive cells.

    Notes: Summary We studied the effect of a decrease in vital capacity (VC) on the blood lactate threshold detected during exercise in 16 preoperative (PRE) and 10 postoperative (POST) lung cancer patients who had undergone lobectomy or pneumonectomy. The PRE patients were selected on the basis of having normal preoperative pulmonary function. The POST patients were selected on the basis of having normal preoperative pulmonary function and a postoperative VC of less than 80%. The oxygen consumption/body surface area at a 2.2 m.mol·l−1 arterial lactate concentration ( $$\dot V_{{\text{O}}_{\text{2}} } $$ / BSA at La-2.2) was adopted as the blood lactate threshold. VC/BSA in the POST group significantly correlated with $$\dot V_{{\text{O}}_{\text{2}} } $$ /BSA at La-2.2 (r=0.85, P〈0.01), but not in the PRE group. SaO2 at La-2.2 was 95.4+-1.5% in the PRE group and 95.2+-1.3% in the POST group. SaO2 at La-2.2 did not correlate with VC/BSA in either group. The hemoglobin concentration (Hb) in the arterial blood correlated significantly with VC/BSA in the POST group (r= 0.65, P〈 0.05) but not in the PRE group. These results indicate that $$\dot V_{{\text{O}}_{\text{2}} } $$ /BSA at La-2.2 was restricted by VC in patients with restrictive pulmonary function disorder. Of the three elements of oxygen delivery, Hb was a limiting factor for $$\dot V_{{\text{O}}_{\text{2}} } $$ /BSA at La-2.2 but SaO2 was not. Cardiac output, which was not measured in our study, was speculated to be another limiting factor for 388-05 at La-2.2
