    Notes: Summary The case of a 15-year-old boy with a pineal germinoma is reported. The patient first underwent a ventriculoperitoneal (VP) shunt followed by a stereotactic biopsy, then, because of the rapidly deteriorating neurological status, an emergency craniotomy with subtotal removal of the tumour was performed. Two months after surgery, a left femoral metastasis and extensive peritoneal lesions became evident; they were regarded as due to haematogenous and VP shunt spread of the germinoma. At that time, extremely high serum levels of placental alkaline phosphatase were detected. The patient died 6 months after the inital diagnosis. The occurrence of extraneural metastases as well as of shunt related peritoneal deposits from primary intracranial germinoma is discussed. As far as we know this is the first reported case of a combination of haematogenous as well as VP shunt spread of a pineal germinoma.

    Notes: Summary Regenerated central axons have been shown experimentally to penetrate in peripheral nerve segments transplanted into the spinal cord (SC). However, if the nerves are transplanted between the stumps of the transected SC regeneration is impaired by local cavitation and scarring. Our experiment was designed to study whether nerve grafts bridging a severe transverse SC lesion might provide to central regenerating axons a pathway to by-pass the lesion. To this purpose, 2 segments of autologous peripheral nerves were inserted through small dural openings into dorsal longitudinal myelotomies rostral and caudal to a transverse SC lesion in rats. Eighteen weeks after transplantation a large number of well myelinated fibres filled the grafted nerves. Only a few of these fibres, however, could be followed into the SC; they were located in the outer layers of the dorsal white matter. The problems regarding the origin and destination of these fibres are discussed.

    Notes: Summary Autologous sciatic nerve grafts were implanted to the lower thoracic spinal cord (SC) of adult rats. The grafts were longitudinally placed on both sides of the SC midline over the dura mater and their cut ends were inserted into the dorsal white matter of the SC. Eight to 60 weeks later the grafts were exposed. In a first experimental group (A) either horseradish peroxidase (HRP) or lectin conjugated horseradish peroxidase (WGA-HRP) was injected into the grafts in order to investigate the origin and the course of regenerated fibres entering the grafts. In a second experimental group (B) the SC was acutely transected between the upper and lower graft insertions and either HRP or WGA-HRP was injected into the caudal stump of the SC in order to investigate the ability of regenerating axons once entered the grafts to re-enter the SC. In group A retrogradely labelled cells were found in the SC scattered in proximity of both the caudal and rostral graft insertions and in the ventral horns as far as 30 mm rostrally to the grafts. Labelled cells were also located in the spinal ganglia, at the level of the grafts and up to 6 segments caudal to them. In group B retrogradely labelled cells were present in the SC rostrally to the transection, both in proximity of the upper graft insertions and in the ventral horns as far as 30 mm rostrally to the grafts. These findings demonstrate that PN grafts implanted to the SC of adults rats can be innervated by regenerated axons arising from both intraspinal neurons and dorsal root ganglion cells (group A); furthermore axons from intraspinal neurons entered and elongated in the grafts can reenter the SC (group B).

    Notes: Summary Four patients suffering for severe drug-resistant epilepsy from bihemispheric cortical dysplasias underwent anterior callosotomy. One of these patients also presented mental retardation of mild degree associated with the epileptic syndrome. There were no operative complications in this series. Clinical signs of interhemispheric disconnection were not detectable postoperatively. Twenty-eight to 53 months after surgery, the generalized seizures were completely suppressed in 2 cases, and were reduced by 89–97% in frequency in the other 2 cases. Partial seizures were less affected by callosotomy being reduced by 14–87%. In an additional fifth case of intractable epilepsy from bihemispheric cortical dysplasias with associated severe mental retardation operated upon elsewhere for callosotomy and followed at our institution, the outcome for seizures was completely unsatisfactory. Neurophysiological studies revealed that the interhemispheric transfer (IHT) of visuo-motor responses was functionally impaired after callosotomy only in one patient who harboured bilateral cortical dysplasias in the occipital lobes. This malformation might affect the pattern of axonal projection to the posterior portion of the corpus callosum which is considered of crucial importance for the integration of crossed visuo-motor responses. From this paper the following conclusions can be drawn: a) epileptic patients with severe drug-resistant epilepsy due to bihemispheric cortical dysplasias are good candidates for callosotomy, b) one-stage extensive anterior callosotomy sparing the splenium is the procedure of choice, c) associated severe mental retardation seems to contra-indicate callosotomy, d) the neurophysiological study of the IHT can yield information on the functional status of the corpus callosum.

    Notes: Summary Neoformation of connective tissue occurring at the level of spinal cord injury is considered a factor in the failure of regeneration in the mammalian spinal cord. The purpose of the present research was to experimentally investigate the origin and characteristics of connective proliferation following spinal cord lesion produced by compression in the rat. The role of the dural sheath and that of the tissues surrounding the spinal cord were studied. In one group of animals (1), the dura mater was left intact; in a second group (2) a transverse incision of the dura was performed at the level of the spinal cord compressive lesion. In group (1) a few collagenous fibres were seen within the lesion but no connective septum was observed. In group (2) a transversely orientated septum of fibrous scar tissue was constantly found within the lesioned cord. Our experimental study shows that: 1. dural continuity prevents the formation of connective tissue scarring and limits fibrous reactions in the epidural space; 2. opening of the dural sheath is followed by a vigorous fibroblastic reaction in the epidural tissue which extends into the spinal cord to form a connective septum.

    Notes: Abstract Normally, simple digital or manual responses to a light stimulus in the right or left visual hemifields are performed faster with uncrossed hand-field combinations than with crossed hand-field combinations. Because of the organization of visual and motor pathways, the integration of uncrossed responses is assumed to occur within a single hemisphere, whereas a time-consuming inter-hemispheric transfer via the corpus callosum is considered to be necessary for the integration of crossed responses. However, callosal transfer may be dispensable for those crossed responses which can be controlled through ipsilaterally descending motor pathways by the hemisphere receiving the visual stimulus. We investigated crossed-uncrossed differences (CUDs) in speed of simple visuomotor responses to lateralized flashes in one subject with total section of the corpus callosum and two subjects with complete callosal agenesis. We recorded the reaction times as well as the premotor times, as indicated by the electromyographic latencies of the prime movers, of three types of responses: a distal response involving a thumb flexion, a proximal response chiefly involving a forearm flexion and an axial response involving a shoulder elevation. Further, the three types of responses to a single lateralised flash were performed both unilaterally and bilaterally. The three acallosal subjects showed CUDs greatly exceeding normal values on distal responses, either unilateral or bilateral, and on unilateral proximal responses. These abnormally long CUDs stood in sharp contrast to the insignificant CUDs exhibited by the same subjects on bilateral proximal responses and on unilateral and bilateral axial responses in agreement with correspondingly insignificant CUDs reported for normal subjects. These results confirm that a callosal contribution is important for the execution of fast distal and unilateral proximal responses to a visual stimulus directed to the hemisphere ipsilateral to the responding hand. By contrast, the other types of crossed responses appear to be efficiently coordinated across the midline without the aid of the corpus callosum. This is in keeping with the hypothesis that they are governed by a bilaterally distributed motor system which is preferentially activated for the execution of symmetrical bilateral movements, employing axial and proximal limb muscles.

    Notes: Summary We report the results over 15 years (1977–1991) for biochemical diagnoses of patients referred from throughout Italy and suspected of having a mucopolysaccharidosis. Of these, 147 patients were diagnosed as being homozygous or hemizygous for a specific lysosomal enzyme deficiency; 74 pregnancies at risk were monitored in their families; 76 heterozygote diagnoses were performed on their relatives, with a total of 48 positive diagnoses. We also report the analysis of genomic DNA from 11 unrelated Italian Hunter patients, using pc2S15 probe. DNA from two patients, digested with Pst-I, showed a variant pattern of hybridization caused by deletion or rearrangement of the gene.
