    Notes: Background  Adult atopic dermatitis (AD) can seriously affect quality of life of patients and their families, and patients' disease is frequently not satisfactorily controlled with topical therapy. There is a need for alternatives to topical treatment in patients with moderate to severe AD.Objectives  To investigate the efficacy and safety of oral pimecrolimus, and to determine the response to three different doses in the treatment of AD.Methods  In a double-blind, placebo-controlled, parallel-group, dose-finding study, patients with moderate to severe AD were randomized to receive either placebo, or oral pimecrolimus 10, 20 or 30 mg twice daily. The study consisted of a pretreatment phase, a 12-week double-blind treatment phase, and a 12-week post-treatment phase.Results  In total, 103 patients were randomized. A clear, dose-dependent therapeutic effect of pimecrolimus treatment was observed, with a statistically significant onset of efficacy at week 2 and the greatest reduction from baseline of the Eczema Area and Severity Index of 66·6% at week 7 in the 30 mg twice daily dose group. Oral pimecrolimus was well tolerated and there were no signs of nephrotoxicity or the induction of hypertension.Conclusions  These data demonstrate the clinically relevant efficacy and short-term safety of oral pimecrolimus in adults with moderate to severe AD. Longer-term studies in larger cohorts are now required.

    Notes: Objective  To validate the Eczema Area and Severity Index (EASI) by assessing its internal consistency, reliability and sensitivity to change and by correlating it to other efficacy parameters.Design  Three short-term and two long-term double-blind, randomized, controlled trials, performed in 138 study centres in Europe, South Africa, Australia, New Zealand, and North and South America.Patients and methods  In total, 1550 paediatric patients with atopic dermatitis were studied. Pimecrolimus cream 1% was used twice daily to treat atopic dermatitis. The three short-term studies were placebo controlled. The two long-term studies evaluated the efficacy and safety of early intervention with pimecrolimus to prevent progression to disease flare requiring topical corticosteroid treatment, compared with reactive treatment with topical corticosteroids to treat flares of atopic dermatitis.Main outcome measures  Five parameters were measured: (i) the EASI (range of score 0–72); (ii) Investigators' Global Assessment (IGA), using a six-point (0–5) scale; (iii) patients' assessment, using a four-point (0–3) scale; (iv) severity of pruritus assessment, using a four-point (0–3) scale; and (v) a quality-of-life evaluation.Results  The EASI score varied in parallel and in correlation with the IGA, pruritus and patients' assessment. All correlation coefficients were statistically different from 0 (P 〈 0·05). The EASI correlated well with each of its components, and all paired comparisons were within agreed limits. The EASI showed good sensitivity to changes in severity.Conclusion  In a large, multinational patient population with atopic dermatitis, the EASI showed good validity, reliability and sensitivity to change and correlated well with other measures of severity. It therefore qualifies as a valid method of assessment in clinical studies of atopic dermatitis.

    Notes: Psoriasis is a chronic, immune-mediated disorder that usually requires long-term treatment for control. Approximately 25% of patients have moderate to severe disease and require phototherapy, systemic therapy or both. Despite the availability of numerous therapeutic options, the long-term management of psoriasis can be complicated by treatment-related limitations. With advances in molecular research and technology, several biological therapies are in various stages of development and approval for psoriasis. Biological therapies are designed to modulate key steps in the pathogenesis of psoriasis. Collectively, biologicals have been evaluated in thousands of patients with psoriasis and have demonstrated significant benefit with favourable safety and tolerability profiles. The limitations of current psoriasis therapies, the value of biological therapies for psoriasis, and guidance regarding the incorporation of biological therapies into clinical practice are discussed.

    Notes: Abstract A new formula for the gravitational potential for spheroidal systems is proposed, and is applied to the galactic system. The applied model consists of a disk, a nucleus and a massive halo. Although this model is not quite so accurate as those produced by the superposition of large numbers of simple spheroids, it has the important advantage of retaining analytic simplicity for its acceleration formulae, thereby producing significant economies for orbit computations. As better observational data become available, the method can be readily expanded.

    Description / Table of Contents: Résumé On a étudié dans quelques espèces typiques de boues rouges de Hongrie l'effet des variations de pression sur la forme des courbes TG, TGD et ATD ainsi que les spectres de masse des phases hydroxyde et carbonate. La variation de la pression entraîne des vitesses de décomposition différentes. Il en résulte une meilleure séparation des phénomènes dans le cas où ceux-ci se chevauchent ainsi que des avantages du point de vue de l'analyse des phases. L'analyse des phases présentes dans les boues rouges a été effectuée par extraction à l'eau. Les extraits et les résidus solides ont été identifiés par spectrométrie infrarouge et par diffraction des rayons X.
    Abstract: Zusammenfassung Die Wirkung der Druckänderung auf die Form der TG-, DTG- und DTA-Kurven, sowie der MS-Spektra der Hydroxid- und Carbonatphasen in einigen typischen ungarischen Rotschlamm-Sorten wurde geprüft. Die Druckänderung verursachte verschiedene Zersetzungsgeschwindigkeiten der Phasen, führte zur besseren Auftrennung der sich überlappenden thermischen Kurven, und war ausserdem auch hinsichtlich der Phasenanalyse vom Vorteil. Zur Phasenanalyse wurden die Rotschlämme mit Wasser extrahiert und die Extrakte und festen Rückstände mittels IR- und Röntgenmethoden identifiziert.
    Notes: Abstract The effect of reduction of pressure on the shapes of the TG, DTG and DTA curves and the mass-spectra of hydroxide and carbonate phases was investigated in some typical Hungarian red muds. The pressure change caused different decomposition rates of the phases and resulted in better separation of the overlapping thermal curves; this led to advantages as regards phase analysis. For phase analysis the red muds were extracted with water, and the extracts and solid residues were identified by IR- and X-ray methods.

    Notes: Abstract Hydrogen desorption during heat treatments of (crystalline) Zr3FeH5.5 was studied by Mössbauer spectroscopy (MS), X-ray (powder) diffraction (XRD) and thermal gravimetry (TG). In case of sufficiently slow heating rate all hydrogen effuses below 650°K and no phase segregation occurs. At higher temperatures retained hydrogen induce phase separation into Fe2Zr (or more likely Fe2ZrHx) and zirconium hydride. The existence of a highly disordered f.c. tetragonal ternary hydride phase was also detected both by XRD (ao=4.76 Å, c=4.96 Å) and MS $$(\bar I\bar S = - 0.14,\bar \Delta \bar E = 0.34)$$ .
