    Description / Table of Contents: Abstract. Tissue engineering offers the possibility to fabricate living substitutes for tissues and organs by combining histogenic cells and biocompatible carrier materials. Pluripotent mesenchymal stem cells are isolated and subcultured ex vivo and then their histogenic differentiation is induced by external factors. The fabrication of bone and cartilage constructs, their combinations and gene therapeutic approaches are demonstrated. Advantages and disadvantages of these methods are described by in vitro and in vitro testing. The proof of histotypical function after implantation in vivo is essential. The use of autologous cells and tissue engineering methods offers the possibility to overcome the disadvantages of classical tissue reconstruction – donor site morbidity of autologous grafts, immunogenicity of allogenic grafts and loosening of alloplastic implants. Furthermore, tissue engineering widens the spectrum of surgical indications in bone and cartilage reconstruction.
    Notes: Zusammenfassung. Tissue engineering ermöglicht die Herstellung lebender Konstrukte zum Gewebeersatz durch die Kombination histogener Zellen und biokompatiblen Trägermaterialien. Pluripotente mesenchymale Stammzellen können isoliert, ex vivo vermehrt und ihre gewebetypische Differenzierung durch Faktoren induziert werden. Es wird die Herstellung von Knochen- und Knorpelkonstrukten, deren Kombination sowie gentherapeutische Ansätze dargestellt. Vor- und Nachteile der Methoden werden durch in vitro und in vivo Testung beschrieben. Essentiell ist der Nachweis der gewebetypischen Funktion der Konstrukte nach Implantation in den Defekt. Die Verwendung autogener Zellen und Methoden des Tissue engineerings bietet die Möglichkeit die Probleme des klassischen Gewebeersatzes – Morbidität der autogenen Spenderstelle, Immunogenität des allogenen Transplantats und Lockerung von alloplastischen Implantaten – zu überwinden und erweitert das Indikationsspektrum in der rekonstruktiven Chirurgie.

    Notes: Abstract  To circumvent well-known drawbacks inherent in cultured epidermal sheet grafts, a new keratinocyte delivery system was experimently investigated. Human subconfluent keratinocytes were labeled with fluorescence cell linker and cultured in two brands of fibrin glue (TissucolR and BeriplastR) in vitro for 5 days. Keratinocyte fibrin glue suspensions were grafted onto full thickness wounds in athymic nude mice for macroscopic and microscopic investigation. Keratinocytes survived in both fibrin glues in vitro for at least 5 days. When grafted onto wounds, cells maintained a high proliferation potential and at 5 days formed small masses or well-organised cell clusters within the fibrin network. After 7 days new epithelia enlarged and became thicker with high keratinization and occasional downward projections. At 10 days, wounds in both fibrin groups were totally epitheliazied but only partially in the control group. Anti-involucrin and anti-laminin immunostainings indicated well-differentiated new epithelium derived from human keratinocytes and early reorganised basement membrane. The fibrin glues seem not only to deliver highly proliferative keratinocytes easily and effectively, but also provide an optimal milieu for their migration, proliferation and differentiation.

    Notes: Summary The case of a patient with a phyllodes tumor of the breast who over a period of 12 years underwent 15 operations for local recurrences, finally succumbing to the disease, is presented. Occurring at the age of 42 as a benign lesion, the tumor underwent malignant sarcomatous transformation after prolonged surgical management. An attempted ultraradical resection failed, because the explosively growing tumor had passed the borders of resectability and also metastasized. Surgical management of the phyllodes tumor of the breast, a rare pathologic entity of variable behavior, is discussed with regard to the literature.

    Notes: Summary Glabellar flaps have traditionally been used to cover nasal skin and soft tissue defects. The narrowing of its vascular pedicle to the cutaneous arterial branch of the angular artery categorizing it as an axial flap pattern. Three cases are presented, the first of which typifies the standard glabellar flap with an intact skin and subcutaneous pedicle. The other two cases utilize a glabellar island flap based on a subcutaneous pedicle containing cutaneous arterial branch of the angular artery. This technique extends the mobility of this flap to allow one-stage reconstruction of adjacent defects.

    Notes: Abstract In a patient with burns of 88% TBSA autologous cultured keratinocyte fibrin glue suspension (KFGS) overgrafted with fresh split thickness allogenic skin was used as definitive wound coverage after epifascial debridement and temporary allogenic closure. A total of 22% TBSA were closed in this way including a complete leg except the foot. For comparison, the contralateral leg was covered with conventional autologous sheet grafts (CEA). Histology revealed a layered squamous epithelium, and evidence of at least partial integration of allogenic dermis including the formation of a well-organized rete structure even after deep necretomy. Besides the rapid stable closure and good skin quality, the new technique has the advantages of early availability, simple handling, easy repetition, and the transfer of actively proliferating epidermal cells. This is the first case proving that KFGS with allogenic overgrafts leads to permanent epithelialization of large epifascial wounds. The technique compared favorably with CEA with regard to availability, handling, and stability.

    Notes: Summary In 38 patients who underwent tissue expansion, microbiology swabs from the implant bed and expander lumen were routinely cultured at the time of expander removal. Of the 61 cultures performed 42% were positive. Along with various potentially pathogenic bacteria and skin commensales, staphylococci were the strains isolated most frequently. In only two cases this contamination correlated with a clinically apparent infection or expander exposure. The results suggest that bacterial contamination is a common feature in expansion, which does not necessarily lead to an infection and failure of the method. Infection is predominantly a problem of impaired local resistance, especially microcirculatory derangements.

    Notes: Abstract  A step toward an improved understanding of the complex mechanisms of growth factor interactions may lie in the detection of endogenous growth factors during normal wound healing. The findings of this study on standardized full thickness wounds in swine, provide direct evidence that growth factors were present in the wound fluid in the picogram range (highest concentrations ranging from 1273 pg/ml for transforming growth factor-beta (TGFβ) to 85.6 pg/ml for platelet-derived growth factor-AB (PDGF-AB ) during healing. The presence of transplanted autologous keratinocyte suspensions and cultured epithelial sheet graft had no significant effect upon the observed growth factor levels, although transplanted keratinocyte cell suspensions (KCS) and cultured epidermal autografts (CEA) did accelerate healing in comparison to control wounds in our model (KCS treated wounds healed in 13.2±0.9 days, CEA in 13.7 days±0.8 and control wounds in 14.7 days±0.3). The variable occurrence of growth factors during normal wound healing may suggest possible mechanisms of growth factor interaction which could have an impact on the future design of their therapeutic use.

    Notes: Abstract  The distally based posterior interosseous artery flap is routinely used for covering soft tissue defects on the hand. However problems in dissecting the flap can arise due to an inconstant vascular anatomy. A case is described where it was not possible to employ a distally based posterior interosseous artery flap because of the absence of the posterior interosseous vascular axis and an unusually high bifurcation of the brachial artery. The preoperative Doppler investigation showed only a positive retrograde blood flow in the area of the distal anastomosis to the anterior interosseous artery. After the arteriogram, the defect was finally closed with a free lateral arm flap. The authors point out that a complete Doppler investigation by a skilled investigator is necessary to detect unexpected vascular variations and to accurately indicate the patients on whom one should perform a preoperative angiogram.

    Description / Table of Contents: Zusammenfassung Die doppelseitige Unterbindung der A. iliaca interna ist zur vorsorglichen Blutstillung und bei Verblutungsgefahr erfolgreich. Trotz der Unterbindung mit einem resorbierbaren Faden (Catgut) konnte im Tierversuch erst nach neun Monaten eine vollständige Rekanalisation dieser großen Arterie nachgewiesen werden. In der Praxis ist die erektive Potenz, ein gesundes Gefäßsystem vorausgesetzt, durch these Unterbindungen in situ ohne Durchtrennung der Arterie unbeeinflulßt geblieben. Da alle hier genannten Patienten nach der Ligatur schon vor der Rekanalisation keine Störung der Erektion aufwiesen, genügt der sehr bald einsetzende Umgehungskreislauf für die nötige Durchblutungssteigerung. Eine Gegenindikation für die Unterbindung sind vorhergegangene sehr große Eingriffe an den Beckenorganen, vor allem wenn eine Röntgenbestrahlung eingewirkt hatte oder wenn eine obliterierende Gefäßkrankheit vorliegt. Diese Arbeit widerspricht dem Zeitgeist. Heute steht die Rekonstruktion von Blutgefäßen im Mittelpunkt und nicht die Unterbindung einer so großen Arterie. Können wir she aber verantworten, um eine starke Blutung im Beckenbereich zu beherrschen?
    Notes: Summary The bilateral ligation of the internal iliac arteries in situ is successful in preventing haemostasis and the treatment of potentially lethal haemorrhage. In spite of using a resorbable thread (catgut) for ligation a complete recanalization of this large artery could only be observed after nine months in an animal experiment. Clinically this ligation of the artery in situ without severance has not impaired erective potency in patients with an otherwise healthy vascular system. Since many patients did not show any disturbance prior to the vessel's recanalization the early established parallel circulation is sufficient to compensate for the perfusion deficit. A contraindication for the ligation is a previous major procedure, especially in cases of irradiation or an occlusive vascular disease.

    Description / Table of Contents: Summary Tissue expanders (inflatable silicone balloons) were implanted into a bypassed ileum segment of seven pigs and consecutively filled over 8 to 12 weeks. The mean increase of the volume of the small bowel loops was 12 fold with a maximum of 22.8 times the original volume. The final volume 9 weeks after expander removal still presented an increase of 500%. Histologically there was an increase of the thickness of all layers of the bowel wall, especially in the tunica muscularis. The mucosa showed a value above normal in mean in vitro 14C-resorption capacity per surface area in the reintegrated bowel. Possible clinical applications of the augmentation of the gut by expansion as a rectum substitute after proctocolectomy, as a urinary bladder or stomach substitute, and for the treatment of short bowel syndrome are discussed.
    Notes: Zusammenfassung Gewebeexpander (auffüllbare Silikonballons) wurden bei sieben Schweinen in ein ausgeschaltetes Ileumsegment implantiert und anschließend über 8 bis 12 Wochen aufgefüllt. Die mittlere Volumenzunahme der Dünndarmschlinge lag beim Zwölffachen des Ausgangswertes (maximal 22,8fach). Neun Wochen nach Expanderentfernung und Wiedereinschaltung in die Kontinuität betrug das mittlere Endvolumen immer noch 500%. Histologisch fand sich eine Zunahme der Dicke aller Darmwandschichten, insbesondere der Tunica muscularis. Die mittlere in-vitro-l4C-Glukoseresorptionsleistung des wiedereingeschalteten Ileums lag über den Kontrollen. Eine mögliche Nutzung der Darmvergrößerung durch Expansion als Mastdarm-, Blasen- oder Magenersatz und zur Behandlung des Kurzdarmsyndroms werden diskutiert.
