    Notes: Summary An intact microcirculation is essential for normal healing to occur. Wound repair may be impaired by various endogenous and exogenous factors, such as reduced microvascular perfusion, infection and debris. In the nonhealing wound, radical surgical debridement is critical. To supplement healing, various ointments are used in clinical practice. Little is known about their effects on tissue perfusion. We have therefore selected two substances widely used, the antiseptic Betadine® and the enzyme combination Elase® and investigated their impact on the microcirculation and on leucocyte activity, using the cremaster muscle as a model. We found that functional capillary density and arteriolar diameters were significantly reduced by Betadine®, whereas leucocyte activity was not affected. In the Elase® group, capillary flow and arteriolar diameters were significantly increased, and again leucocyte activity was not changed. The mechanism by which Betadine® reduces microvascular flow is believed to be the same as in reperfusion injury. The positive effect of Elase® on the microcirculation might be attributed to plasmin, which has been shown to dilate blood vessels.

Full-thickness defects of the thoracic wall following tumor resection, irradiation damage or secondary wound healing in thoracic surgery require early interdisciplinary cooperation to achieve patient-specific treatment modalities. Plastic surgical differential therapy allowing for sufficient soft tissue coverage, stabilisation of the thoracic wall and space filling in intrathoracic cavities, is presented and critically discussed.

    Description / Table of Contents: Zusammenfassung. Die Fähigkeit von Wachstumsfaktoren, das Wachstum von Zielzellen zu stimulieren, eröffnet interessante Anwendungsmöglichkeiten in der Therapie von Wundheilungsstörungen. Die eingangs hohen Erwartungen an eine klinische Therapie mit Wachstumsfaktoren bei Wundheilungsstörungen haben sich bislang trotz vielfacher Anwendung im Rahmen von Studien jedoch nicht erfüllt. Zwar fanden sich unter topischer Applikation verschiedener Wachstumsfaktoren in unterschiedlichen Dosierungsschemata gewisse Effekte auf die Wundheilung selbst bei langwierigen Heilungsstörungen. Nur bei einem Teil der klinischen Studiengruppen wurde jedoch überhaupt eine definitive Heilung erzielt. Direkte Vergleiche mit chirurgischen Verfahren fehlen bisher, obgleich diese meistens zumindest in Form eines Débridements Bestandteil der Anwendungsprotokolle waren. Das Rationale für eine Anwendung der bislang sehr kostspieligen Faktoren sollte jedoch die Erzielung einer schnelleren und kostengünstigeren Heilung sein. Dieser Effizienz- und Wirtschaftslichkeitsnachweis wurde bislang nicht erbracht. Ein Einsatz von Wachstumsfaktoren vor allem im ambulanten Bereich sollte derzeit nur unter kontrollierten Studienbedingungen mit planimetrischer Dokumentation des Heilungsverlaufs stattfinden. Bei chronischen Wundproblemen ist zu fordern, daß grundsätzlich alle kausaltherapeutischen, prophylaktischen, konventionellen und auch plastisch-rekonstruktiven Möglichkeiten der Wundbehandlung ausgeschöpft werden, bevor adjuvante Therapien mit Wachstumsfaktoren durchgeführt werden. Als Ziel gilt ein frühzeitiger Wundverschluß unter Wiederherstellung eines dauerhaften und belastungsfähigen Integuments.
    Notes: Summary. Clinical trials on exogenous application of polypeptide growth factors in chronic wounds have not fulfilled the high expectations derived from results of experimental studies. There is no convincing evidence that growth factors may substitute for good wound care and efficient surgical approaches to wound closure. The ultimate goal of treatment of chronic ulcerations remains reconstitution of a durable skin envelope without unstable scarring. Therefore, optimization of current methods of wound therapy, including reconstructive vascular and plastic surgery and adequate metabolic and wound control, should be employed before any adjuvant growth factor therapy is attempted. As long as efficient and inexpensive therapy of chronic wounds by growth factors has not been demonstrated, empincal growth factor treatment should be rejected on scientific and economic grounds. Current use appears to be reasonable only under a regime of controlled clinical studies comparing growth factor treatment with conventional wound therapy and operative measures according to the rules of “good clinical practice”.

    Description / Table of Contents: Zusammenfassung. Die Metastasierung colorectaler Tumoren erfolgt regelhaft lymphatisch entlang der abdominellen Lymphabflußwege oder hämatogen in Leber und Lunge. Neben der Kasuistik erfolgt die Diskussion der bisher beschriebenen 13 Fälle. Im folgenden soll der Fall einer 72 jährigen Patientin mit einem Coloncarcinom und atypischer intramuskulärer Metastasierung beschrieben werden. Es erfolgt zusätzlich eine Zusammenfassung der bisher beschriebenen muskulären Metastasierungen colorectaler Tumoren.
    Notes: Abstract. Metastases from colorectal carcinoma usually spread along abdominal lymph nodes or are hematogenous to the liver or lungs. So far, 13 cases of metastases to skeletal muscle have been described. In the following report, the case of a 72-year-old woman with a colonic carcinoma and metastases of this tumor in the thigh is described. Other reports about muscular metastases of colorectal tumors are discussed.

Mit der konsequenten Einführung eines sequentiellen radikalen Débridements und der postprimären Weichgewebsrekonstruktion konnte die Amputationsrate, der Hospitalaufenthalt, die chronische Osteitis, die Rehabilitationsperiode und die Zahl sekundärer rekonstruktiver Eingriffe bei drittgradig offenen Unterschenkelfrakturen deutlich reduziert werden. Durch die Distraktionscorticotomie und biologische Osteosyntheseverfahren wurde die Knochenrekonstruktion deutlich verkürzt. Es werden die Indikation, taktisches Vorgehen und technische iatrogene Probleme der aktuellen interdisziplinären Verfahren geschildert, die eine Modifikation auch bei der Weichgewebsrekonstruktion erfordern.
    Notes: Summary. Sequential radical debridement and early soft-tissue reconstruction have considerably decreased the amputation rate, length of hospital stay, chronic osteitis, the rehabilitation period and secondary reconstructive procedures in lower leg injuries. The introduction of distraction osteotomy and „biologic osteosynthesis procedures“ have led to shorter and safer osteoplastic methods. The indication, tactics and technical pitfalls of current interdisciplinary treatment options requiring modifications in soft-tissue coverage are presented.

    Notes: Summary Bilateral loss of arms following high-voltage electrical injury results in severe problems for prosthetic replacement. To allow a unilateral myoelectric prosthetic fitting directed by contralateral contact switches, stump construction was achieved by bilateral tissue expansion and free non-vascularized double-fibula transplantation.     Notes: Summary Limb-sparing resection in combination with radio/chemotherapy is recommended in cases of soft tissue sarcomas of the extremities. Plastic surgery techniques and clinical results in 69 patients with 71 malignant soft tissue tumors who had hadergone resection, as well as additional reconstructive procedures for functional improvement, are discussed. The integration of patients suffering from soft tissue sarcomas of the extremities into the treatment protocols of interdisciplinary tumor boards is recommended. Plastic surgery salvage procedures (including microsurgical free-tissue transfer) are discussed in detail. If the patient is judged unsuitable for these procedures or if distalization is not possible, amputation is the final option.

    Description / Table of Contents: Zusammenfassung Die Untersuchung befasst sich mit kritischen Punkten bei der Behandlung von komplizierten Verletzungen der unteren Extremität bei 48 Patienten (1980–1986). Um Verzögerungen bei der Diagnose zu verhindern, sind sorgfältige Untersuchung und aggressiver Einsatz der Angiographie notwendig. Doppler-Untersuchung kann die Diagnose fehlleiten. 68% der Extremitäten konnten erhalten werden. Ligatur zusätzlicher Venenverletzungen ging mit einer erhöhten Komplikationsrate einher. Die Ischämiezeit wurde durch den Einsatz des Javid-Shunts verkürzt. Fasciotomiert wurde bevorzugt präoperativ. Vascularisierte Muskel- und Hautlappen wurden zur Verbesserung von Funktionsergebnissen gebraucht.
    Notes: Summary This study evaluates the critical points in the management of 48 patients with injured arteries of the lower limb between 1980 and 1986. 77% of the traumas were resultant from blunt vehicular traffic mishaps. 13 patients were treated within 6 h, 8 patients later than 6 h, 21 patients later than 24 h post injury. Delays in diagnosis occurred due to lack of or no indication for vascular trauma. Careful physical examination and aggressive use of angiography is essential in improving limb salvage rates, while doppler investigation may lead to improper diagnosis. Overall limb salvage was 69%, however, no reconstruction was possible in 6 cases (13%). 38 of the 42 arterial reconstructions required interposition of venous bypass grafts, four end to end anastomoses. Eleven patients had associated venous injuries, which in nine cases were repaired. Venous ligation in three cases was attributed with increased complication. In three cases ischemia time was shortened by the use of temporary javid shunts for rapid restoration of arterial flow. 50% of the patients were found to require fasciotomy, either pre- or postvascular repair. Early fasciotomy, however was found to be most beneficial. Fractures were treated in 15 cases with the external fixator, in 21 cases with internal fixation. Delayed revascularization after 24 h combined with aggressive debriding of muscle necrosis and the employment of vascularized muscular and skin flaps resulted in a decline in amputation rates and improved functional results.
