    Description: Research software has become a central asset in academic research. It optimizes existing and enables new research methods, implements and embeds research knowledge, and constitutes an essential research product in itself. Research software must be sustainable in order to understand, replicate, reproduce, and build upon existing research or conduct new research effectively. In other words, software must be available, discoverable, usable, and adaptable to new needs, both now and in the future. Research software therefore requires an environment that supports sustainability. Hence, a change is needed in the way research software development and maintenance are currently motivated, incentivized, funded, structurally and infrastructurally supported, and legally treated. Failing to do so will threaten the quality and validity of research. In this paper, we identify challenges for research software sustainability in Germany and beyond, in terms of motivation, selection, research software engineering personnel, funding, infrastructure, and legal aspects. Besides researchers, we specifically address political and academic decision-makers to increase awareness of the importance and needs of sustainable research software practices. In particular, we recommend strategies and measures to create an environment for sustainable research software, with the ultimate goal to ensure that software-driven research is valid, reproducible and sustainable, and that software is recognized as a first class citizen in research. This paper is the outcome of two workshops run in Germany in 2019, at deRSE19 - the first International Conference of Research Software Engineers in Germany - and a dedicated DFG-supported follow-up workshop in Berlin.

    Notes: Summary Skin biopsies were performed to examine dermal capillary basement membranes for albumin by immunofluorescence microscopy in children and young adults with Type 1 (insulin-dependent) diabetes from simplex and multiplex families, their non-diabetic parents and control subjects. Circumferential vessel staining for albumin was 12.0 ±14.4% (mean ± SD) in diabetic patients from multiplex families, 18.8 ±20.0% in diabetic patients from simplex families, and 18.5 ±24.2% in unaffected parents from simplex families. These values were significantly higher (p 〈 0.005) than those obtained in unaffected parents from multiplex families (0.4 ± 0.9%) or control subjects (0.8 + 1.7%). There was no statistically significant difference between Type 1 diabetic probands from simplex or multiplex families and unaffected parents from simplex families. In the simplex families, positive capillary staining for albumin was present only in parents from families in which the diabetic proband had positive staining. In the parents, the presence of dermal capillary staining for albumin did not correlate with an abnormal glucose tolerance test or the presence of cytoplasmic islet cell antibodies. These observations suggest an abnormality in the dermal capillary basement membrane in some unaffected parents of Type 1 diabetic patients from simplex families.

    Notes: Summary We reviewed all medical records concerning patients suffering from spinal cord or cauda equina compression (SCC) secondary to cancer, in the eastern part of Denmark, from 1979 through 1985. During the period the incidence of SCC in cancer patients went up from 4.4% to 6%. However, this increase was not significant. The series comprised 398 cases, with carcinoma of the prostate (19%), lung (18%), breast (14%) and kidney (10%) accounting for 61%. The symptoms were evaluated in accordance with the patients rating of pain, motor deficits, sphincter control and paraesthesia, whereas the clinical manifestations were classified on the basis of motor deficit and bladder dysfunction. During the period preceding the diagnosis of SCC, 83% of the patients suffered from back pain, 67% from deteriorating gait and 48% had retention of the urine. In 35% of the patients there was no sphincter disturbance and 10% had normal sensory function. The outcome of treatment was estimated by changes in motor deficits and sphincter function, and depended primarily on the patients condition at the time of the diagnosis. Of the patients who were able to walk before treatment, 79% remained ambulatory, wheras only 18% of the non-ambulatory patients regained walking ability. Patients treated by decompressive laminectomy followed by radiotherapy apparantly had a better response than patients treated with surgery or irradiation alone, but when the patients pre-treatment motor function was taken into account, no significant difference was observed. The study may call for a properly randomized trial with careful stratification of tumour biology, performance status and neurological deficits.

    Description / Table of Contents: Summary The present study investigates the effects of dopexamine on the splanchnic perfusion and the general circulation in patients undergoing coronary artery bypass grafting. Methods: 34 Patients undergoing elective coronary artery bypass grafting were randomized to receive either dopexamine (1μg/kg/min) or placebo (0,9% NaCl.) Cardiac output was measured by thermodilution using a pulmonary artery catheter. Splanchnic blood flow was determined by constant infusion technique of indocyanine green dye (ICG) using a hepatic vein catheter. Results: Under steady state conditions before surgery, dopexamine increased systemic oxygen supply and cardiac output by an increase in heart rate and stroke volume, with no increase in systemic oxygen demand. Before surgery dopexamine increased splanchnic blood flow together with an increase in cardiac output. After cardiopulmonary bypass dopexamine increased splanchnic perfusion without a concomittant rise in cardiac output. Conclusion: These results provide evidence that dopexamine improves splanchnic blood flow in patients with coronary artery disease before and after pulmonary bypass without impairment of general hemodynamics.
    Notes: Zusammenfassung In der vorliegenden Studie wurde der Einfluß von Dopexamin auf die Splanchnikusperfusion und die allgemeine Hämodynamik während einer aortokoronaren Bypassoperation untersucht. Methodik: 34 Patienten, die sich einer elektiven aortokoronaren Bypassoperation unterzogen, wurden nach Randomisierung in zwei Gruppen eingeteilt und erhielten perioperativ 1μg/kg/min Dopexamin oder als Placebo 0,9% NaCl. Die Splanchnikusdurchblutung wurde mit Hilfe der ICG-Extraktionsmethode über einen Lebervenenkatheter bestimmt, das Herzzeitvolumen nach der Thermodilutionsmethode über einen Pulmonaliskatheter; der globale Sauerstoffverbrauch wurde berechnet. Ergebnisse: Unter Ruhebedingungen steigerte Dopexamin durch eine Zunahme der Herzfrequenz und des Schlagvolumens das HZV, während der globale Sauerstoffverbrauch sich nicht veränderte. Parallel dazu kam es zu einer HZV-abhängigen Steigerung der Splanchnikusperfusion. Nach der extrakorporalen Zirkulation war unter Dopexamin die Splanchnikusperfusion unabhängig vom Herzzeitvolumen erhöht. Schlußfolgerung: Die vorliegende Studie belegt, daß die perioperative Infusion von Dopexamin bei kardiochirurgischen Eingriffen zu einer Verbesserung der regionalen Perfusion im Gastrointestinaltrakt führen kann, ohne die allgemeine Hämodynamik zu beeinträchtigen.

    Notes: Das schwere Lungenemphysem, definiert als irreversible Erweiterung der distal der Bronchioli terminales liegenden Lufträume mit begleitender Zerstörung der Alveolarsepten, ist mit einer erheblichen Morbidität und Mortalität belastet. Schon frühzeitig leiden die Patienten unter Atemnot, die anfänglich bei körperlicher Belastung und später auch in Ruhe nachweisbar ist und dadurch die körperliche Aktivität der Patienten einschränkt. Die medikamentösen Therapiemöglichkeiten sind begrenzt und bestehen im wesentlichen in einer symptomatischen Behandlung der Bronchialobstruktion und einer konsequenten Therapie bronchopulmonaler Infekte sowie einer Besserung der körperlichen Leistungsfähigkeit durch Muskelaufbau- und Atemtraining. Im fortgeschrittenen Stadium mit respiratorischer Partialinsuffizienz stellt die Sauerstoff-Langzeittherapie eine weitere Behandlungsoption dar. Unter den chirurgischen Therapieansätzen konnte sich in der Vergangenheit nur die Resektion großer Emphysemblasen beim bullösen Emphysem durchsetzen. Die Lungenvolumenreduktion (LVR) hingegen ist ein erst kürzlich wieder eingeführtes Operationsverfahren [8], das nach ersten veröffentlichten Berichten [7, 10, 19, 23] bei ausgewählten Patienten mit ausgeprägtem Lungenemphysem das Ausmaß der Dyspnoe verringert als auch die körperliche Leistungsfähigkeit der Patienten und damit deren Lebensqualität verbessert. Die Langzeitprognose des neuen Operationsverfahrens muß jedoch noch durch weitere Untersuchungen evaluiert werden.
