Notes: Summary This study reports subcortical lesions in 7 newborn babies after transient circulatory arrest and/or asphyxia. Basal ganglia, diencephalon, tegmentum of the brain stem and spinal grey matter exhibited extensive necroses in a columnar pattern. The lesions of the telencephalic and cerebellar cortex are less prominent. The spinal cord, available in 3 children, revealed subtotal neuronal loss in all segments. The lesions represent the extreme anoxic damage of the CNS in the perinatal period, comparable with brain death in the adulthood. Moreover, the pattern with predominating subcortical lesions indicates that anoxia affects the grey matter in all levels of the CNS.

Description / Table of Contents: Zusammenfassung Bei 22 Neugeborenen, die wegen Hyperbilirubinämie eine Phototherapie erhielten, wurden Wachstumshormon (GH) und Cortisol im Plasma bestimmt.
Notes: Abstract In 22 neonates plasma growth hormone (GH) and cortisol concentrations were measured during phototherapy which was administered because of hyperbilirubinemia. These values were compared to the hormone levels before and after therapy and to control values taken from healthy nursery infants. Phototherapy significantly increased GH concentrations to 195%±108 (SD) (healthy control group=100%). Cortisol was not found to be augmented. There was no correlation between GH and cortisol, nor between bilirubin and GH or cortisol. The mechanisms by which phototherapy induces GH increase are not evident. However, this may be due to constant covering of the eyes, deprivation of the day-night rhythm, or other environmental alterations that are brought about by incubator care.

Notes: Abstract Retinal atrophy in rats following illumination with artificial fluorescent light is reported. The intensity of irradiation was equivalent to that applied in phototherapy of human newborns. The retinas of infant rats exposed to irradiation soon after birth for periods of 28 to 44 days, were less affected than those of adult animals illuminated over a period of 12 days only. After 12 days of illumination no retinal damage was found in the eyes of infant rats, whose eye lids were still closed, even when they were exposed to an especially high light dosage. Therefore, utmost care to shield the eyes of newborn babies during phototherapy seems advisable and may be regarded as effective.

Notes: Abstract In pharmacokinetic studies we proved that the efficacy of phototherapy in jaundiced Gunn rats was enhanced by administration of human albumin. The albumin injections provoked a bilirubin shift from extravascular compartments into the circulation. In this study, calculations on the optimal albumin dose were carried out. After s.c. injections a good effect could be ascertained with doses between 2 and 4 g/kg body weight. After i.v. injections effective plasma concentrations could be maintained for an acceptable time by about 2 g albumin/kg body weight. When overdoses were given, a considerable amount of albumin was eliminated very quickly—especially after i.v. injections with the rapid rise of the plasma level. Clinical implications are discussed.
