Notes: Summary Pedicled temporoparietal fascial flaps have been used extensively in head and neck reconstruction for many years. Also, the fascia has been grafted, and the skin graft transformed into a vascularized pedicled flap (prefabricated temporoparietal fasciocutaneous flap). On the other hand, some authors have used free temporoparietal fascial flaps for defects that require to be filled in lower and upper extremity wounds. In this case, a free prefabricated temporoparietal fascio-chondro-cutaneous flap having only an arterial inflow was used in eyelid reconstruction.

Notes: Abstract Helium-neon laser irradiation was applied to the denuded dermis and full-thickness open wounds on rabbit skin and cell cultures of human skin fibroblasts to investigate its effects on the wound healing process. To determine the effects of He-Ne laser radiation on epithelialization rate, 3×3 cm denuded dermis areas on the flank of 16 rabbits were irradiated daily until complete epithelialization occurred. For histopathological evaluation biopsies were taken on the first day and on the day on which epithelialization was complete. As a second part of in vivo study, identical full-thickness skin wounds were created bilaterally on the middle flank area of 12 rabbits. He-Ne laser irradiation was applied daily to the wounds until complete healing occurred to determine the effects of the low-energy laser on the contraction of open wounds. The contralateral wounds were left untreated, serving as controls. In a separate in vitro study, the effect of single or multiple applications of He-Ne laser irradiation on normal human skin fibroblasts in cell cultures was evaluated using growth measurement. The mean epithelialization time was 11±0.63 days for the laser-treated wounds and 12±0.12 days for the control wounds. The difference was not significant. No significant difference was found between the contraction rates of the full-thickness wounds (e.g., on the seventh postoperative day, the average wound area was 70.2±6.75% of original wound area in the laser-treated group and 66.±8.75% in the control group). Histopathologically, epidermal thickening and an increase in dermal vascularity were observed in healed wounds of the laser-treated groups. However, in vitro, this low-energy laser promoted cell growth in human fibroblast cell cultures in 2-and 3-day treated groups (p〉0.05).

Notes: Abstract A new technique for the functional reconstruction of the ankylotic temporomandibular joint (TMJ) is presented. Interposition of a custom-made inverted T-shape silicone implant after osteotomy of the condylar neck has given good results. A case is presented and the advantages of the method are discussed.

Notes: Abstract  A new chrondrocutaneous flap raised from the posterior area was used to reconstruct composite marginal ear defects of two patients. In this technique, a posterior rectangular advancement flap with a cartilage strip was advanced into the defectly horizontal. Two weeks later, the pedicle was divided and the flap was tailored. The donor area was covered with a skin graft. This flap provides both ideal skin cover and support for the cartilaginous framework. It can be transferred without delay and can be used in immediate or secondary reconstructions. It is recommended as an alternative method for the reconstruction of the middle third marginal auricular defects.

Notes: Abstract  The disadvantages and limitations of imaging methods to investigate velopharyngeal incompetence have created some difficulties in the management of this condition. Seven normal volunteers and seven patients who were suffering from a speech disorder were examined using magnetic resonance imaging. The velopharyngeal aperture was evaluated at rest and during phonation. In normal volunteers, the velopharyngeal aperture area had a mean value of 1.632 cm2 while at rest and complete closure was obtained during the phonation of /s/ sound. Detailed information was obtained about the function of the levator palati muscle. In five patients during the phonation of /s/ sound there was an increase in the area of the velopharyngeal aperture when compared to the volunteers. Investigation with magnetic resonance imaging is helpful in the pretreatment evaluation and postopertive follow-up examination of velopharyngeal insufficiency. As a result, this noninvasive method can be used as an alternative to conventional radiological investigations.

Notes: Summary The widely documented treatment of nonunion with various clinical applications of electrical stimulation has met with an excellent success rate (70–93.6%). Local muscle flaps are widely accepted in the treatment of osteomyelitic lesions, and in our departments a combined, one-stage application of electrical stimulation with local muscle flap has been used on six cases of infected nonunion of the tibia with excellent results.

Notes: Abstract Skin flaps are normally characterized by arterial inflow and venous outflow. However, there are several reports about experimental and clinical applications of venous and arterialized venous flaps. On the other hand, some studies evaluated the importance of different pedicle types in prefabricated flaps. It was discovered by chance that a prefabricated free flap, having arterial-only inflow, could be used successfully in eyelid reconstruction. An experimental study on arterial flaps in rabbits showed that an arterial-only inflow was adequate for flap viability until the establishment of venous neovascularization.

Notes: Summary In 1982, Dibbell described a procedure to correct this distortion by rotating and advancing the nostril medially and superiorly. We used this method in our department for several years, but as Dibbell himself experienced, we didn't always obtain satisfactory results. Like many authors, I agree that the best approach for proper correction of the secondary unilateral cleft lip nose deformity is the external approach using transcolumellar incisions, because the lower lateral cartilage exposure is inadequate with Dibbell's method. I added the transcolumellar incision similar to Bardach's to solve this problem. In this paper, this combination technique will be presented and contrasted with Dibbell's and Bardach's techniques.

Notes: Summary Rhinophyma is classically a benign condition whose etiology and pathogenesis are not well understood. A few cases of giant rhinophymas have been reported in the literature. In this article, a case of gigantic, lobulated rhinophyma is presented. The appearance was unusual, and there was cyst formation histologically.

Notes: Summary Popliteal pterygium syndrome is a rare congenital anomaly. The literature contains only a few reports about treatment of this malformation. This paper outlines our approach in one case and compares our surgical technique with the reports previously described.
