    Notes: Summary Sural nerve biopsy was performed on a six-year-old girl with a slowly progressive distal mixed polyneuropathy. The nerve fiber population was reduced, and of those remaining, approximately half were greatly enlarged, measuring up to 35 microns in cross section. Longitudinal sections showed axonal enlargement to be segmental. Ultrastructurally, swollen axonal segments were filled with neurofilaments in closely-packed swirls which appeared to tie on occasion into granular electron-opaque condensations. Both, myelinated and unmyelinated fibers were involved, and some degree of onion-bulb formation was observed. Although masses of neurofilaments are found in peripheral axons in other disorders, none approaches the proportions encountered in this unique case.

    Notes: Summary Giant axonal neuropathy (GAN) is a disease characterized by a slowly progressive neuropathy and signs of central involvement, manifested by visual impairment, corticospinal tract dysfunction, ataxia, and dementia. Pathological hallmarks of the disease include axonal swellings packed with neurofilaments in both peripheral and central nervous systems, and accumulations of intermediate filaments in Schwann cells, fibroblasts, melanocytes, endothelial, and Langerhans cells. Rosenthal fibers, sometimes appearing in masses and mimicking Alexander's disease, emerge as a conspicuous characteristic in longstanding GAN.

    Notes: Abstract Visceral leishmaniasis (kala-azar) affecting HIV-infected patient is being reported in increasing frequency. A 40-year-old German bisexual patient with full-blown AIDS is described who presented with Kaposi's sarcoma, epigastric pain, diarrhea, and weight loss but without fever.Leishmania amastigotes were initially found in biopsies from stomach, duodenum, and a cutaneous Kaposi's sarcoma lesion but were later also recovered from bone marrow and lymph node. The patient received three courses of a combination of pentavalent antimony and interferon-γ. In addition to the common side effects such as fever, thrombocytopenia, and elevated amylase and lipase, a vivid progression of the Kaposi's sarcoma was noted. Tumor progression was temporally closely associated with treatment with interferon-γ. Because this phenomemon has also been observed in other patients, we advise caution when using interferon-γ in patients with Kaposi's sarcoma.

    Notes: Abstract Objective To study whether high fat, low carbohydrate enteral nutrition could reduce $$\dot VCO_2 $$ in patients during ventilator support and weaning from the ventilator in order to facilitate the weaning process. Design Prospective, randomized controlled study. Setting Medical ICU of a university hospital. Patients 32 ventilator-dependent patients with a prospect of weaning from mechanical ventilation. Interventions High fat feeding administered to 15 patients and standard isocaloric feeding administered to 17 patients, both in a dosage of 1.5 times basal metabolic rate. Measurements and results Respiratory and metabolic measurements were obtained both during mechanical ventilation and weaning procedures. High fat feeding was associated with significantly lower RQ values compared with standard feeding; the mean (±SEM) RQ values during mechanical ventilation amounted to 0.91±0.01 and 1.00±0.02 and during weaning to 0.72±0.02 and 0.86±0.02 for high fat and standard nutrition respectively (bothp-values〈0.001). High fat feeding reduced the CO2-excretion both during mechanical ventilation and weaning, but only the decrease during weaning proved to be significant; the mean (±SEM) CO2-excretion amounted to 0.177±0.010 and 0.231±0.011 l/min STPD for the high fat and standard feeding respectively (p〈0.01). No significant differences were found in the PaCO2 during weaning between the two feeding groups. Conclusion High fat, low carbohydrate enteral feeding significantly reduced the RQ values in ventilated patients with decreases $$\dot VCO_2 $$ , but in this study failed to reduce PaCO2 during weaning from the ventilator.

    Notes: Abstract Objective: To assess the feasibility of expiratory flow-volume curves as a measurement of respiratory mechanics during ventilatory support: to what extent is the shape of the curve affected by the exhalation valve of the ventilator? Design: Prospective, comparative study. Setting: Medical intensive care unit of a university hospital. Patients: 28 consecutive patients with various conditions, mechanically ventilated with both the Siemens Servo 900C and 300 ventilators, were studied under sedation and paralysis. Interventions: The ventilator circuit was intermittently disconnected from the ventilator at end-inspiration in order to obtain flow-volume curves with and without the exhalation valve in place. Measurements and results: Peak flow (PEF) and the slope of the flow-volume curve during the last 50 % of expired volume (SF50) were obtained both with and without the exhalation valve in place. The exhalation valve caused a significant reduction in peak flow of 0.3 l/s (from 1.27 to 0.97 l/s) with the Siemens Servo 900 C ventilator and of 0.42 l/s (from 1.36 to 0.94 l/s) with the Siemens Servo 300 ventilator (p 〈 0.001). The SF50 was not affected. Conclusion: In mechanically ventilated patients, the exhalation valve causes a significant reduction in peak flow, but does not affect the SF50. This study further suggests that the second part of the expiratory flow-volume curve can be used to estimate patients' respiratory mechanics during ventilatory support.

    Notes: Abstract. Despite its lack of specificity, magnetic resonance imaging (MRI) of the bone marrow has the potential to play a role in the management of patients with primary neoplastic disorders of the hematopoietic system, including lymphomas, leukemias and multiple myeloma. In addition to its use in the assessment of suspected spinal cord compression, bone marrow MRI could be used as a prognostic method or as a technique to assess the response to treatment. The current review addresses the common patterns of bone marrow involvement observed in primary neoplasms of the bone marrow, basic technical principles of bone marrow MRI, and several applications of MRI in selected clinical situations.

    Notes: Abstract. The appearance in magnetic resonance imaging (MRI) of the bones depends, to a large extent, on the unmineralized content of the bone cavities. Because yellow marrow contains a large number of fat protons and red marrow a significant number of water protons, MRI offers the opportunity to map the distribution of red and yellow marrow. In addition, red marrow MR appearance varies according to the relative proportion of fat and nonfat cells. Variations in the composition of red marrow and its distribution among normal subjects, mainly in relation to age and sex, contribute to creating a wide spectrum in bone MR appearance, which must be known in order to avoid confusion with bone marrow abnormalities.
