    Notes: Objectives:The purpose of this study was to look for a relationship between history of respiratory tract infection (RTI) and oral health in very old subjects. Setting and Subjects:302 frail elders (mean age: 85 years) living in a medical care facility were examined by one dentist. Outcome Measures:The incidence of RTI over 1 year had been recorded along with markers of nutritional status and degree of dependency. The oral examination comprised an evaluation of hygiene, quality of prostheses and the prevalence of caries, periodontal disease and mucosal disorders. Results:One third (33%) of the subjects had experienced at least one episode of RTI and a fifth (19%) had visited the dentist in emergency. The incidence of RTI had been greater among dentate subjects and those who came to the dentist in emergency. The dentate subjects with a history of RTI had higher plaque score (P=0.02). Half (49%) of the subjects had oral disorders that could develop in a dental emergency and these subjects had had a higher risk of RTI (relative risk: 1.9, 95% confidence interval: 1.1–3.9). The presence of selected oral disorders associated with low serum albumin increased the relative risk of having had RTI to 3.2 (1.5–6.7). The association between presence of actual oral health problems and previous experience of RTI was more noticeable in those who had poor general health or were more debilitated. Conclusions:The present study suggests that poor oral hygiene and the presence of potential emergency could be major risk factors for RTI among the frail elderly.

    Notes: Aims: To correlate microbial findings obtained by bronchoalveolar lavage in pneumonia patients with the clinical situation of the oral cavity.Method: Quantitative aerobic and anaerobic cultures were carried out in 150 ml samples of bronchoalveolar lavage (BAL) obtained by means of an endoscope (Video Endoscope Pentax®) inserted per as in the infected bronchus.Material: Twenty consecutive patients with a tentative clinical diagnosis of bronchopneumonia in whom BAL was carried out for diagnostic purposes. A clinical evaluation of the oral health status (oral hygiene, caries, periodontal diseases) was subsequently carried out.Results: In seven edentulous subjects wearing complete dentures the culture of anaerobic microorganisms was negative or yielding less than 100 cfu/ml BAL. Two patients yielded high counts of S. aureus and one high counts of P. aeruginosa. In the 13 subjects with natural teeth left one showed high counts of Veillonella spp. (anaerobic)+P. aeruginosa, one high counts of Veillonella spp. +S. aureus, one high counts of P. aeruginosa + S. aureus and one high counts of E. coli. These four subjects showed poor oral hygiene, periodontal pockets and a BAL microflora consistent with periodontal pathology. Conclusion: The results of this pilot study suggest that microorganisms of denture plaque or associated with periodontal diseases may give rise to aspiration pneumonia in susceptible individuals.

    Notes: The purpose of this study was to compare prosthetic, functional and occlusal conditions in twenty-seven patients treated with distally extending cantilever bridges and twenty-six patients treated with removable partial dentures (RPD) in the mandible. All patients had a complete upper denture. Mean age of the patients in both groups was about 69 years. The patients were under a supervised oral hygiene care throughout the 2-year study period. There were 6.9±l.7 mandibular teeth left in the bridge group and 7.5±l.7 in the RPD group and the mean number of posterior teeth (natural teeth/denture teeth/pontics) in occlusion was 4.l±l.l and 7.3±l.4, respectively. During the study period signs and symptoms of mandibular dysfunction became significantly aggravated in the RPD group, P〈0.05. A balanced occlusion in the muscular contact position was observed in 90% of the patients in the bridge group and in 76% of the RPD wearers. During the study period the need for dental or prosthetic treatment was negligible in the patients treated with bridges. In the RPD group, twenty-two teeth were restored with fillings due to caries and in eight patients major adjustments of the sublingual bar were necessary due to irritation of the oral mucosa. This study has shown that treatment with distally extending cantilever bridges in the mandible is a favourable alternative to treatment with removable partial dentures in elderly patients with a reduced dentition.

    Notes: By means of crossed immunoelectrophoresis with intermediate gel the precipitins to C. albicans in sera from 18 patients with deep Candida infection were compared with the findings in a control material consisting of sera from 169 'normal' controls (mainly healthy blood donors), 25 patients with oral candidiasis, 14 patients with vaginal candidiasis, and 6 persons without candidiasis giving false positive reactions by the double diffusion method. Up to 20 different precipitins were found in individual sera, and precipitins were demonstrated in all sera except in 6 'normal' controls. Of 692 precipitins 682 (98%) were identified and titrated in terms of the 78-component reference system described earlier for C. albicans. No free Candida antigens were found, but would have been detected by the method. The best criterion for discrimination between sera from patients and controls was the precipitin score, which accounts for number of precipitins and for titres. A precipitin score of more than 6 1/2 had a diagnostic sensitivity of 88.9% and a specificity of 99.5%. Eleven precipitin specificities were found solely in the patients' sera, but the highest sensitivity for a single precipitin was 33%. Until more evidence has accumulated in favour of a monospecific immunoassay it is therefore rational to use a complex antigen extract for further evaluations of diagnostic Candida serology and a short cut to standardization would be the introduction of the method used in the present study.

    Description / Table of Contents: Résumé Dans ce nouveau mode de fixation horizontal pour arcs vestibulaires et linguaux, une pièce, mobile autour du pas de vis dont est pourvu l'extrémité de l'arc, sert de verrou. L'arc se place dans un tube fendu, ouvert du côté de la face triturante, et l'étrier de la pièce mobile est ramené autour du tube et vient se loger du côté opposé. Il faut noter la simplicité de préparation et d'utilisation de ce verrou, qui permet l'allongement ou le raccourcissement de l'arc, ainsi que ses dimensions minimes, qui en fait recommander l'emploi sur les molaires inférieures.
    Abstract: Synopsis In this new horizontal lock for labial and lingual arches a movible piece, which screws on to the threaded end of the arch-wire, serves to lock it. The arch is placed into a slotted tube, opened towards the masticating surface, and the bow of the movable piece is moved gingivally around the slit tube and comes to rest on the under surface of the latter. Notice should be taken of the ease of construction and handling of this lock, which allows lengthening and shortening of the arch, as well as of its small size, which makes its highly recommendable for use on lower molars.
    Notes: Zusammenfassung Bei dem neuen Horizontalschloß für orale und faziale Bögen dient ein Überfang, der sich auf dem mit Gewinde versehenem Bogenende drehbar bewegt, als Verschluß. Der Bogen wird von der Kaufläche her in ein horizontales Schlitzröhrchen gelegt und der Bügel des Überfanges über das Schlitzröhrchen herübergedreht, an dessen abgeflachter Unterfläche es Halt findet. Zu beachten ist die einfache Herstellung und Handhabung des Schlosses, das eine Verlängerung und Verkürzung des Haupt-bogens ermöglicht, und seine geringe Dimension, die seine Anwendung besonders bei niedrigen Molaren empfiehlt.
