    Notes: Summary Lipid peroxidation and enhanced arachidonic acid metabolism is activated after blood-brain cell contact. Previous studies have indicated that cysteinyl-leukotrienes (cys-LT) have the capacity to constrict arterial vessels in vivo and in vitro suggesting their involvement in the pathogenesis of cerebral vasospasm. The purpose of this study was to measure the amount of cyst-LT in the cerebro-spinal fluid (CSF) in correlation with transcranial Doppler findings (TCD) in patients with aneurysmal subarachnoid haemorrhage (SAH). In all patients early surgery was performed. In the first cisternal CSF-sample which was already collected intra-operatively an initial peak of cys-LT was detected, followed by decreasing amounts of cys-LT during the next 5 days. The CSF-levels of immunoreactive cys-LT were significantly higher in those patients who showed signs of vasospasm on transcranial Doppler sonography (TCD) (p 〈 0.001). Normalization of TCD values was accompanied by decreasing levels of CSF-cys-LT. We found a significant correlation between the amounts of immunoreactive cys-LT in cerebrospinal fluid and cerebral vasospasm measured by TCD.

    Notes: Summary In order to avoid invasive procedures (transfrontal, transcallosal) in the surgical treatment of colloid cysts the stereotactic aspiration technique was introduced by Bosch, RÄhn and Backlund in 1978. The viscosity of the intracystic colloid and the displacement of the cyst away from the aspiration needle are possible reasons for unsuccessful aspiration. GT-guided stereotactic endoscopic technique gives the opportunity to fenestrate the cyst wall under direct visual control. After CT-guided stereotactic puncture of the right lateral ventricle with the foramen of Monro as target a steerable endoscope is introduced and the foramen of Monro is passed. The wall of the cyst is fenestrated and coagulated by means of monopolar (or laser) coagulation. Using this technique we have treated four patients with colloid cysts. Immediately postoperatively all of them were relieved of their complaints and have been symptomfree for a mean follow up period of twelve months. Studies of cerebrospinal fluid flow patterns, performed prior to and every six months after the endoscopic intervention, confirmed a remarkable reduction of foraminal obstruction.

    Notes: Summary Accidental and operative trauma are able to induce a systemic reaction of the organism characterized by fever, leukocytosis, catabolism, and an activation of the coagulation system. Interleukin-6 (IL-6) has been found to be an important mediator of this acute-phase response. In this study the influence of elective craniotomy on IL-6 plasma levels was evaluated. Blood samples were obtained from 20 patients undergoing elective craniotorny for vascular or tumorous diseases of the brain. IL-6 increased significantly (p 〈 0.05) from the pre-operative (0 (0–5.4) pg/ml) to the intraoperative (180 min after beginning of surgery) time-point (10.6 (0–18.5) pg/ml). The maximum was reached on the first postoperative morning (13.9 (4.3–45.0) pg/ml). Interleukin-10 (IL-10) is an anti-inflammatory cytokine which suppresses IL-6 synthesis in vitro in various cell lines. IL-10 plasma concentrations showed no alterations throughout the study period. Epinephrine plasma concentrations increased significantly from pre-operative values (15 (0–74) pg/ml) to the postoperative time-point (57 (9–459) pg/ml). A 4.5-fold increase (p 〈 0.05) of norepinephrine plasma concentrations was found when comparing the data obtained 60 min after beginning of surgery with the data of the first postoperative morning. In monocytes, which are a major source of plasma IL-6, an elevation of intracellular cAMP stimulates the IL-6 synthesis. The postoperative maximum of IL-6 in plasma could be due to a release of catecholamines. In conclusion this study demonstrated an elevation of IL-6 plasma concentrations during and after elective craniotomy. Increased plasma catecholamine concentrations as well as a damage in the blood-brain barrier due to the surgical trauma with a spill-over of IL-6 from brain tissue into plasma could have contributed to this result.

    Description / Table of Contents: Summary The term ”juxta facet cyst” was coined in 1974 by Kao et al. and combines synovial cysts arising from the synovial outpouchings through areas of weakened or destroyed capsular tissue and ganglion cysts developing from mucinous degeneration of periarticular connective tissue. We have operated on 16 patients with juxta facet cysts during the last 3 years. One cyst was located in the thoracic spine, causing a transverse syndrome; 15 cysts were located in the lumbar spine, causing nerve root compression. Diagnosis was established preoperatively by MR in 10 and CT in 3 cases, respectively. The definite classification of the cystic processes was made by histopathologic examination. Surgical treatment consisted of resection of the cyst. Postoperatively, all but 2 patients were free of complaints and no recurrence was observed in the mean follow-up period of 15.5 months. Juxta facet cysts are an uncommon finding, but must be considered in the differential diagnosis of epidural compressive syndromes of the spine.
    Notes: Zusammenfassung Der Terminus „Juxta-Facett-Zyste” wurde 1974 von Kao et al. geprägt und faßt Synovialzysten, die von der Synovia der degenerativ veränderten Intervertebralgelenke ausgehen, und Ganglionzysten, die durch mukoide Degeneration von periartikulärem Bindegewebe entstehen, zusammen. Wir behandelten in den letzten 3 Jahren 16 Patienten mit Juxta-Facett-Zysten. Eine thorakal gelegene Zyste führte zu einer Querschnittslähmung, die 15 lumbal gelegenen Zysten führten in 14 Fällen zu einem langsam progredienten, in einem Fall zu einem akuten Nervenwurzelkompressionssyndrom. Die Diagnose wurde präoperativ in 10 Fällen mittels MR und in 3 Fällen mittels CT gestellt; die endgültige Diagnose und Klassifikation erfolgte durch histopathologische Untersuchungen. Die Behandlung bestand in der mikrochirurgischen Resektion der Zyste. Nach einer mittleren Nachbeobachtungsdauer von 15,5 Monaten waren 87,5% der Patienten beschwerdefrei. In keinem Fall kam es zu einem Rezidiv. Juxta-Facett-Zysten sind selten, müssen aber in der Differentialdiagnose der spinalen epiduralen Kompressionssyndrome berücksichtigt werden.
