    Notes: Summary Lipid peroxidation and enhanced arachidonic acid metabolism is activated after blood-brain cell contact. Previous studies have indicated that cysteinyl-leukotrienes (cys-LT) have the capacity to constrict arterial vessels in vivo and in vitro suggesting their involvement in the pathogenesis of cerebral vasospasm. The purpose of this study was to measure the amount of cyst-LT in the cerebro-spinal fluid (CSF) in correlation with transcranial Doppler findings (TCD) in patients with aneurysmal subarachnoid haemorrhage (SAH). In all patients early surgery was performed. In the first cisternal CSF-sample which was already collected intra-operatively an initial peak of cys-LT was detected, followed by decreasing amounts of cys-LT during the next 5 days. The CSF-levels of immunoreactive cys-LT were significantly higher in those patients who showed signs of vasospasm on transcranial Doppler sonography (TCD) (p 〈 0.001). Normalization of TCD values was accompanied by decreasing levels of CSF-cys-LT. We found a significant correlation between the amounts of immunoreactive cys-LT in cerebrospinal fluid and cerebral vasospasm measured by TCD.

    Description / Table of Contents: Summary Sera of 38 patients with various intracranial tumours were examined by indirect immunofluorescence for antibodies against their own tumours. The frequency of antibodies found in this investigation corresponds to the frequency of antibodies against tumours of other organs. Antibodies were found in 9 out of 14 glioblastomas, in 3 out of 10 slower growing gliomas, and in 2 sarcomas. Antibodies could not be demonstrated in metastases, meningiomas, or Lindau tumours. Localization of antigens by immunofluorescence suggests that they are surface antigens. Antibodies are found in sera collected two days after surgery as well as in sera obtained two weeks after surgery. The method used did not demonstrate any complement dependency. Possibilities for diagnostic use are discussed.
    Notes: Zusammenfassung Mit der indirekten Immunfluoreszenz wurden Seren von 38 Patienten mit unterschiedlichen intrakraniellen Tumoren auf Antikörper gegen die eigenen Geschwülste untersucht. Die gefundene Antikörperhäufigkeit entspricht etwa der, wie sie auch für Tumoren anderer Lokalisation angegeben wird. Der Antikörpernaehweis gelang bei 9 von 14 Glioblastomen, bei drei von zehn langsamer wachsenden Gliomen und bei zwei Sarkomen. Er gelang nicht bei Metastasen, Meningeomen und Lindau-Tumoren. Die fluoreszenzoptische Lokalisation der Antigene legt die Vermutung nahe, daß es sich hierbei um Oberflächenantigene handelt. Die Antikörper sind gleichermaßen in kurz postoperativ wie in zwei Wochen nach der Operation gewonnenem Serum nachweisbar. Bei der verwendeten Methodik ließ sich eine Komplementabhängigkeit nicht erkennen. Möglichkeiten zur diagnostischen Nutzung werden diskutiert.

    Notes: Summary 42 human intracranial tumours were investigated by the leucocyte migration inhibition technique at various antigen concentrations with and without autologous serum addition. A correlation was found between migration rate and antigen concentration. Autologous serum addition resulted in a non-specific stimulation of migration, however, stimulation by immune complexes was also present. Migration stimulation, as an in-vitro-equivalent of immunologic tumour enhancement, is discussed.

    Notes: Summary Intervertebral plates of hydroxy apatite ceramic (HAC) have been used in three patients for cervical vertebral interbody fusion after anterior discectomy. In one case a pure HAC “Disc” was used, which proved to be too friable. Specially designed intervertebral plates, which were composed of an HAC-coated core of alumina ceramic, were used in the other two cases. Clinically and radiologically optimal results after 1-year- and 2-year-follow-up suggest that HAC-ceramic might be a very promising material for vertebral interbody fusion. Possible complications and pain due to bone removal from the iliac crest are avoided, and the operative procedure is simplified.

    Notes: Summary In order to avoid invasive procedures (transfrontal, transcallosal) in the surgical treatment of colloid cysts the stereotactic aspiration technique was introduced by Bosch, RÄhn and Backlund in 1978. The viscosity of the intracystic colloid and the displacement of the cyst away from the aspiration needle are possible reasons for unsuccessful aspiration. GT-guided stereotactic endoscopic technique gives the opportunity to fenestrate the cyst wall under direct visual control. After CT-guided stereotactic puncture of the right lateral ventricle with the foramen of Monro as target a steerable endoscope is introduced and the foramen of Monro is passed. The wall of the cyst is fenestrated and coagulated by means of monopolar (or laser) coagulation. Using this technique we have treated four patients with colloid cysts. Immediately postoperatively all of them were relieved of their complaints and have been symptomfree for a mean follow up period of twelve months. Studies of cerebrospinal fluid flow patterns, performed prior to and every six months after the endoscopic intervention, confirmed a remarkable reduction of foraminal obstruction.

    Notes: Summary  External drainage of CSF in children is indicated in acute hydrocephalus e.g. after intraventricular bleeding or infection. In these cases the drainage has to remain in place until physiological CSF circulation is restored or an internal shunt can be inserted. External drainage is mostly performed using a silicon catheter connected to a plastic bag. An external CSF drainage left for several days in situ bears an increasing risk of bacterial infection. In these cases a metal needle minimizes the risk of ventriculitis.  For prevention of accompanying infection we implanted a CytocanR port needle with a 20 G diameter in 7 infants. The needle is easily inserted with low risk of complications. By its plastic top angulated at 90° to the needle the system can be fixed to the skin very safely by suture or tape.  With this system neither an implantation related intracerebral haemorrhage nor an accompanying ventriculitis was encountered. In two cases a pre-existing ventriculitis was cured by this needle drainage and antibiotic treatment. In five cases an acute hydrocephalus after intraventricular haemorrhage was drained by the port needle system.

    Notes: Summary Immunoelectrophoresis of extracts of 200 intracranial tumours against rabbit anti-glioblastoma serum gave positive results (= precipitation) in all cases of tumours of neuroectodermal origin such as glioblastoma, astrocytoma, oligodendroglioma, ependymoma, neurinoma, and spongioblastoma. No immunoelectrophoretic precipitation was seen for any of the tumours of mesenchymal origin, for instance meningioma and metastases of cancer. On the basis of these findings, immunoelectrophoresis is considered to be a reliable method for differentiation between tumour tissue of neuroectodermal and non-neuroectodermal origin. Among the 41 posterior fossa tumours some unusual observations were made. Cerebellar angioblastoma (Lindau tumour) showed an atypically located precipitation line, which for the present is interpreted as an immunological reaction to vascular wall tissue. Furthermore, among the group of so-called medulloblastomas, two subgroups were distinguished on the basis of three parameters. The first of these subgroups comprises tumours whose immunoelectrophoretic pattern resembles that of gliomas, which are histologically characterized by neuroectodermal structures and which occur in younger children (5–10 years). The tumours of the second subgroup, which do not show this neuroectodermal immunoelectrophoretic pattern, have a sarcomatous character histologically, and occur in patients aged between 10 and 50 years. The view that medulloblastoma comprises a number of different types of tumour seems to be confirmed by this finding.

Three cases with manifestation on the left side are presented. Embryology is discussed briefly. Knowledge of such anomalies is of importance to surgeons as well as angiographers.
