    Description / Table of Contents: Résumé La ligne de réflexion du péricarde autour du pôle veineux du cœur est étudiée au cours de 107 dissections anatomiques. L'étude porte sur des cadavres frais avec des cavités cardiaques remplies artificiellement. Les auteurs étudient les variations du nombre de vv. pulmonaires, les diamètres et les portions intra-péricardiques de ces veines et les caractéristiques des récessus veineux pulmonaires, post-cave et du sinus oblique du péricarde. Ces variations déterminent cinq types de morphologie péricardique différents. Une classification est alors proposée. Les résultats obtenus, en particulier lors de l'étude des vv. pulmonaires, ne sont pas ceux de la littérature probablement car les cœurs étudiés ici sont, au préalable, placés en réplétion pour être le plus près possible de la disposition du vivant. Ces données morphométriques sont utiles pour l'enseignement anatomique, pour l'imagerie du médiastin, pour la chirurgie des tumeurs malignes, et la transplantation pulmonaire.
    Notes: Summary The authors have studied the pericardial recesses of the venous aspect of the heart. They report 107 anatomical dissections on fresh cadavers with the cardiac cavities artificially filled. Variations in the number of the pulmonary vv., in their diameters and their pericardial segments are reported. The characteristics of the oblique sinus, of the postcaval and pulmonary recesses are also reported. These variations define five types of pericardial morphology. A classification is proposed. These results differ from those in the literature, possibly because the pericardial reflection is delineated in diastolic filling of the cavities. These morphometric data are useful in anatomic instruction, imaging of the mediastinum, and the surgery of malignant tumors and pulmonary transplantation.

    Description / Table of Contents: Résumé A l'aide de coupes anatomiques étagées, de niveeaux précis, les possibilités de transfixion du squelette, sans léser les axes vasculo-nerveux, les tendons principaux et sans pénétration articulaire sont étudiées. Après détermination de secteurs de transfixion sans danger sont précisés le lieu sur la peau de la cuisse où la Pénétration et la sortie des broches doit se faire. Deux bandes représentant le quart de la circonférence locale sont individualisées servant d'accès: l'une débute ventralement sur la hanche pour se terminer à la face médiale du genou, l'autre débute à la face dorsale de la fesse pour se terminer à la face latérale du genou.
    Notes: Summary Serial cross-sections of the thigh have been used to indicate where osseous transfixion is possible without damaging neuro-vascular structures or major tendons or penetrating a joint. Safe areas for transfixion using fine wires and their corresponding cutaneous zones are indicated. Two cutaneous zones each representing a quarter of the circumference of the thigh form Safe Zones for transfixion. The first begins on the ventral surface of the hip and ends on the medial aspect of the knee, while the second begins on the posterior aspect of the buttock and ends on the lateral aspect of the knee.

    Notes: Summary Part of the beta-globin genes ofMacaca cynomolgus andGorilla gorilla has been cloned and sequenced. Ten putatively neutral nucleotide polymorphisms have been described at the beta-globin locus in humans. They are associated in seven combinations, which define seven different haplotypes of the beta-globin gene: four major frameworks—1, 2, 3, and 3*—and three minor frameworks, which we term KI1, KA1, and OR1. The nucleotide sequences of these frameworks are compared with those of homologous sequences in chimpanzee, colobus, macaque, and gorilla. This comparison provides strong evidence that framework 2 was the earliest framework in the human lineage. From framework 2, a rooted parsimonious tree for the six other frameworks is constructed. This phylogenetic tree is discussed in terms of the evolution of nucleotide polymorphisms as well as in terms of genetic affinities between human populations. For each position at which there is base difference in comparing human, gorilla, and chimpanzee beta-globin genes, the phyletic lineage where the corresponding substitution occurred has been identified using the maximum parsimony procedure. The data provide evidence that polymorphisms may represent a significant component of differences between closely related species. If so, nucleotide polymorphisms may strongly bias estimates of small evolutionary distances.

    Notes: Summary A 5600-base-pair (bp) fragment including the beta-globin gene and about 4000 bp of its 5′ flanking sequence was cloned from the DNA ofMacaca cynomolgus (an Old World monkey), and the 5′ flanking region was sequenced. Comparison with human, chimpanzee, mouse, rabbit, andXenopus orthologous sequences reveals a tandemly repeated sequence called RS4 at the same position (about 500 bp 5′ from the transcription start of the adult beta-globin gene) in all six species. We suggest that a tandemly repeated sequence has been maintained by functional constraints since the divergence between amphibians and reptiles. Excluding tandemly repeated sequences as well as about 400 nucleotides upstream from the cap site, the average base substitution frequencies among human, chimpanzee, and macaque intergenic sequences were calculated. They appear to be strongly correlated with the delta T50 values measured between the corresponding nuclear DNAs. They are also similar to base substitution frequencies calculated by Chang and Slightom (1984) at the pseudoeta-globin locus. Thus, exclusion of sequences involved in specific modes of variation might allow the use of intergenic sequences for the accurate calculation of genetic distances. Using a time scale based on the dating of the Atlantic split, we estimate the base substitution rate of primate noncoding DNA to be 1.0×10−9 substitution/site/year.

    Notes: Abstract Regurgitation is a common manifestation in infants below the age of 1 year and a frequent reason of counselling of general practitioners and paediatricians. Current management starts with postural and dietary measures, followed by antacids and prokinetics. Recent issues such as an increased risk of sudden infant death in the prone sleeping position and persistent occult gastro-oesophageal reflux in a subset of infants receiving milk thickeners or thickened “anti-regurgitation formula” challenge the established approach. Therefore, the clinical practices for management of infant regurgitation have been critically evaluated with respect to their efficacy, safety and practical implications. The updated recommendations reached by the working party on the management of infant regurgitation contain five phases: (1A) parental reassurance; (1B) milk-thick ening agents; (2) prokinetics; (3) positional therapy as an adjuvant therapy; (4A) H2-blockers; (4B) proton pump inhibitors; (5) surgery.

    Notes: Abstract This study was undertaken to define the pharmacokinetics of omeprazole in children and included 13 patients, heterogenous in terms of age (0.3 to 19 years), underlying disease and biological constants, indication of omeprazole administration and associated therapy. The dose administered ranged from 36.9 to 139 mg·1.73 m−2. The pharmacokinetic parameters of omeprazole were: systemic clearance, 0.23 1·kg−1·h−1; volume of distribution, 0.45 1·kg−1; elimination half life 0.86 h; but were highly variable between individuals. Dosage, differences in hepatic and renal function and associated therapy may contribute to inter-individual variability. Within the range of doses administered, the pharmacokinetic parameters were similar to those reported in adults. The drug has been well tolerated in all children.

    Notes: Conclusion Dans le cadre des interventions de réinnervation laryngée sélective, il est indispensable de disséquer la branche antérieure intralaryngée du n. laryngé inférieur jusqu'à l'origine du n. aryténoïdien sans oublier les premiers millimètres de ce nerf. On isolera ainsi tous les pédicules destinés au m. crico-aryténoïdien dorsal, c'est-àdire tous les pédicules abducteurs. Nous nous proposons dans un premier temps de réséquer la petite corne du cartilage thyroïde qui permettra dans 60% des cas une bonne vision de tous les pédicules abducteurs. Dans les 40% restants on complétera cette résection par la taille d'un volet cartilagineux thyroïdien postéro-inférieur. Dans les innervations du m. cricoaryténoïdien dorsal de type I, le seul pédicule est abducteur et la portion de la branche antérieure située au-dessus est considérée comme adductrice. Dans les innervations de type II et III on n'utilisera qu'un seul pédicule abducteur pour les anastomoses, les autres devant être sectionnées. La branche antérieure située au-dessus du pédicule choisi est la branche adductrice.

Cette étude est un travail d'approche pour les interventions de réinnervation laryngée sélective chez l'homme. Leur conclusion est qu'il est nécessaire de réséquer la corne inférieure du cartilage thyroïde pour aborder les branches abductrice et adductrice des plis vocaux.
    Notes: Summary The studies are based upon 60 dissections of the recurrent laryngeal n. (inferior laryngeal n.). The authors describe in detail the branches destined for the intrinsic musculature of the larynx. This study is a working approach for interventions of selective laryngeal reinnervation in man. The conclusion is that, it is necessary to resect the inferior cornu of the thyroid cartilage, to reach the abductor and adductor branches of the vocal cord.
