    Description / Table of Contents: Abstract Introduction. Patients suffering from migraine and tension-type-headache are at risk of misusing analgesics and therefore developing chronic drug-induced headache. The present study compares migraine patients with and without abuse to identify psychological descriptors promoting drug abuse. Methods. We examined 21 in-patients with migraine and drug-abuse and 21 out-patients with migraine and no drug-abuse who were comparable regarding age and diverse pain variables (intensity, frequency, duration, etc. ). Psychometric questionnaires measured the constructs “pain-”, “depression”, “self-efficacy” and “pain-coping”. Using a standardized interview we investigated the external (environment) and internal (mental factors) conditions of all patients during medication intake. Results. Migraineurs who misused medications showed significantly greater disability, greater helplessness and anxiety due to pain and a tendency towards more marked general depression than non-abusers. The extent of the helpless and depressive coping with pain was best suited for distinguishing the patients with and without drug abuse, followed by the fact that those with abuse receive drugs from several doctors at the same time and demand that the drugs free them of all complaints. Conclusions. These findings suggest that migraine patients with drug abuse often take analgesics not according to their headache, but rather depending on certain other factors.
    Notes: Zusammenfassung Fragestellung. Patienten mit Migräne oder Kopfschmerzen vom Spannungstyp sind gefährdet, Schmerzmittel zu missbrauchen und in der Folge einen medikamenteninduzierten Dauerkopfschmerz zu entwickeln. In der vorliegenden Untersuchung sollten durch einen Vergleich von Migränepatientinnen mit und ohne Abusus psychologische Deskriptoren identifiziert werden, die einen Medikamentenmissbrauch begünstigen können. Methode. Es wurden 21 stationär aufgenommene Migränepatientinnen mit und 21 ambulante Migränepatientinnen ohne Abusus hinsichtlich ihres Alters und diverser Schmerzparameter parallelisiert. In beiden Gruppen wurden die störungsrelevanten Konstrukte “Beeinträchtigung”, “Depressivität”, “Selbstwirksamkeitsüberzeugung” und “Schmerzcoping” psychometrisch gemessen, die individuellen äußeren und inneren Reizbedingungen der Medikamenteneinnahme wurden in einem standardisierten Interview erhoben. Ergebnisse. Die Abususpatientinnen weisen eine signifikant größere Alltagsbeeinträchtigung, eine ausgeprägtere (schmerzbedingte) Hilflosigkeit und Angst sowie eine tendenziell stärkere generelle Depressivität auf als die Nichtabususpatientinnen. Das Ausmaß der hilflos-depressiven Verarbeitung von Schmerzen trennt beide Gruppen am besten, gefolgt von der Tatsache, dass die Abususpatientinnen gleichzeitig Schmerzmittel von mehreren Ärzten erhalten und an diese Mittel den Anspruch der Beschwerdefreiheit stellen. Schlussfolgerungen. Abususpatientinnen orientieren die Einnahme von Medikamenten häufiger nicht am Kopfschmerzgeschehen, sondern machen diese vielmehr von bestimmten Reizbedingungen abhängig.

    Notes: Abstract The application of a new apparatus for the mechanical analysis of residual stress is described; this enables the determination of the distribution of residual stresses of the first kind over the cross-section of bar-shaped specimens. Results of residual stress analysis on heat-treated specimens of heat treatable steel 42CrMo4 (1.7225) are presented and compared with those from the X-ray diffraction method and the incremental hole drilling method. The residual stresses measured by the X-ray diffraction method at the surface agree well with the results of the layer removal method, whereas in this case incremental hole drilling led to erroneous results.
