    Description / Table of Contents: Zusammenfassung Patienten mit Hyperthyreose haben häufig erhöhte Serumaktivitäten der alkalischen Phosphatase. Durch Bestimmung von Isoenzymen gelingt es, diese Aktivitätsvermehrung bestimmten Organen zuzuordnen [5]. Es wird über eine Patientin mit Hyperthyreose berichtet, in deren Serum elektrophoretisch durch Vergleich mit Organextrakten Schilddrüsenphosphatase identifiziert werden konnte. Vermehrung dieses Isoenzyms kann neben Knochen-, Leber- und Dünndarmphosphatase zu Aktivitätserhöhung der alkalischen Gesamtphosphatase im Serum von Hyperthyreosepatienten beitragen.
    Notes: Summary Patients with hyperthyreoidism often present an increased activity of alkaline phosphatase in serum. Determination of isoenzymes may help to assign to different organs an elevated enzyme-activity in serum. This report deals with a patient suffering from hyperthyreoidism. In his serum alkaline phosphatase of the thyroid gland could be identified by comparison with organ-extracts. Raised activity of this isoenzyme can contribute besides bone, liver and small bowel to the increased serum-activity of total alkaline phosphatase in patients with hyperthyreoidism.

    Description / Table of Contents: Zusammenfassung Bei fünf Patienten mit M. Crohn wurde vor und nach Resektion entzündeter Darmbezirke Serumlysozym (Muramidase) bestimmt. Präoperativ deutlich erhöhte Lysozymkonzentrationen fallen postoperativ innerhalb weniger Stunden zur Norm ab. Die Vermutung, daß die granulomatös veränderten Darmanteile Produktionsort des erhöhten Serumlysozyms sind, wird damit gestützt.
    Notes: Summary Serum lysozyme (muramidase) concentrations were determined in five patients with Morbus Crohn before and after resection of inflammated bowel areas. The serum lysozyme activity which was elevated before surgical treatment in all patients fell to normal values after bowel resection within a few hours. Our findings suggest, that the elevated serum lysozyme reflects an increased lysozyme production in the inflammated bowel tissue.

    Description / Table of Contents: Zusammenfassung Das Isoenzym P der N-Acetyl-β-Glucosaminidase wird durch Elektrofokussierung in Polyacrylamidgel in Seren von Gesunden und von Patienten mit akuten und chronischen Lebererkrankungen dargestellt. Bei Gesunden ist das Isoenzym P mit dieser Methode nicht zu erkennen. Bei Patienten mit akuten und chronischen Lebererkrankungen unterschiedlicher Ätiologie und Aktivität wird dann eine Erhöhung des Isoenzyms P beobachtet, wenn die Glutamat-Pyruvat-Transaminase (GPT) erhöht ist. Isoenzym P und GPT korrelieren signifikant. Dagegen wird eine Erhöhung der Gesamt-β-NAG bei chronischen Lebererkrankungen nicht parallel zu cytoplasmatischen Enzymen (wie GPT), sondern abhängig vom Ausmaß der Mesenchymreaktion in der Leber beobachtet.
    Notes: Summary The isoenzyme P of N-Acetyl-β-Glucosaminidase (β-NAG) is demonstrated in sera from healthy persons and patients with acute and chronic liver diseases by electrofocusing in polyacrylamide gel. Elevation of isoenzyme P is observed only together with elevation of Glutamate-Pyruvate-Transaminase (GPT) in patients with acute and chronic liver diseases of different etiology and activity. The correlation between isoenzyme P and GPT is significant. On the contrary whole β-NAG is elevated due to the activity of the mesenchymal reaction in the injured liver tissue, not related to enzymes of the cytosol like GPT.

    Description / Table of Contents: Summary Oral carbohydrate tolerance tests with xylose, galactose and lactose were performed. After an interval of at least 24 h the same tests were repeated following an i.v. bolus and during infusion of somatostatin. Somatostatin does not influence xylose absorption. However, absorption of galactose and lactose is significantly reduced (p〈0.01/0.008) during somatostatin infusion. On the other hand, serum levels of galactose remain unchanged despite administration of somatostatin, when galactose is given parenterally. These results support the assumption that the absorption process in small intestine is affected by somatostatin. Possible effects of somatostation on hormones regulating the intestinal absorption and on energy-depending carrier mechanisms are discussed.
    Notes: Zusammenfassung Bei gesunden jungen Männern wurden Kohlenhydratbelastungen mit Xylose, Galaktose und Laktose durchgeführt. 72 h später wurden die gleichen Kohlenhydratbelastungen nach zusätzlicher, intravenöser Gabe von linearem Somatostatin geprüft. Somatostatin beeinflußt nicht die Absorption von Xylose. Galaktose-Spiegel nach oraler Gabe von Laktose und Galaktose sind unter Somatostatin jedoch deutlich vermindert. Eine Änderung der Galaktose-Spiegel nach intravenöser Galaktosezufuhr wird jedoch durch dieses Hormon nicht beobachtet. Diese Ergebnisse lassen vermuten, daß Somatostatin einen hemmenden Einfluß auf die Absorption von Monosacchariden hat. Mögliche Einflüsse des Somatostatins auf solche Hormone, die die Absorption im Dünndarm regulieren und direkt auf Carrier-Mechanismen werden diskutiert.

    Description / Table of Contents: Zusammenfassung Im zweiten Teil dieser Übersicht werden die klinischen Aspekte des Bindegewebsstoffwechsels in der fibrotischen Leber dargestellt. Besonders eingegangen wird auf die Möglichkeit, die Aktivität der Leberfibrose durch neuere Laborproben zur Erfassung von Enzymen und Metaboliten des Bindegewebsstoffwechsels zu verfolgen. Für diesen Zweck geeignet erscheinen die Enzyme Prokollagen-Prolyl-Hydroxylase, die lysosomale N-Acetyl-β-Glucosaminidase und das N-terminale Peptid vom Prokollagen Typ III. Die Aktivitäten bzw. Konzentrationen im Serum von Patienten mit chronisch aktiven Leberkrankheiten korrelieren gut mit der histologisch geschätzten Aktivität der Fibrose im punktierten Lebergewebe der gleichen Patienten. Weiterhin werden die therapeutischen Versuche zur Behandlung der Leberfibrose mit bindegewebsspezifischen Substanzen besprochen.
    Notes: Summary In this second part, clinical aspects of connective tissue metabolism in the liver will be described and two main aspects considered. The first is the possibility to monitor the activity of fibrosis by the use of metabolites and enzymes of connective tissue metabolism. In recent studies the qualification for this purpose of enzymes such as procollagen prolyl hydroxylase and lysosomal N-Acetyl-β-glucosaminidase and the N-terminal peptide of procollagen type III has been tested. The serum activities or concentrations of these substances in patients with chronic active liver diseases increase in due relation to the histologically estimated activity of liver fibrosis. The second aspect deals with therapeutic approaches to fibrosis in the liver by using connective tissue specific agents. So far none of the antifibrotic substances such as proline analogues, colchicine, lathyrogens and penicillamine has been used in longer-term antifibrotic treatment.

    Notes: Summary Fibronectin is a glycoprotein belonging to the opsonic system and able to mediate phagocytosis by the reticuloendothelial system (RES). Severe intoxications are often followed by a diminished capacity of RES-clearance. Patients with severe drug intoxications have lower concentrations of fibronectin in plasma than healthy persons. Charcoal hemoperfusion lowers plasma fibronectin by an average of 4.7 mg/dl. Initial values below 15 mg/dl did not undergo a significant decrease in the case of hemoperfusion. Regarding these findings, determination of plasma fibronectin seems a useful parameter for monitoring RES function during treatment of drug-intoxicated patients by hemoperfusion.

    Notes: Summary Homozygous deficiency of alpha-1 antitrypsin is the most common inborn error or metabolism in Europe. Severe deficiency of this major protease inhibitor in serum is associated with chronic obstructive lung disease, chronic liver disease in adults and neonatal hepatitis. An overview is given of the role of heredity, and the diagnostic criteria and clinical and histological findings in this disorder. Emphysema seems to be caused by the free elastolytic activity of white cells, leading to the degradation of elastin. The pathophysiology of liver disease — less well understood — is discussed with special emphasis on the importance of heterozygous alpha-1 antitrypsin deficiency. Exogenous noxae seem to play an important role in the pathogenesis of heterozygous deficiency. In view of the 7% frequency of heterozygous alpha-1 antitrypsin deficiency in the European population and the role of noxae in the development of pulmonary and liver diseases, improved prophylaxis is mandatory.

    Notes: Summary Specific radioimmunoassays were used to quantify two basement membrane components, 7S collagen and laminin P1, in sera of 70 patients suffering from diabetes mellitus types I and II with and without clinical signs of chronic diabetic complications. Serum levels of both antigens were increased in diabetics compared to controls (p〈0.001). Concentrations of 7S collagen were significantly different in diabetics with signs of microvascular damage compared to those without small vessel disease (p〈0.05). The difference between laminin P1 concentrations in the two groups of diabetics was not significant (p〈0.2). The augmented levels of circulating 7S collagen and laminin P1 may reflect alterations of basement membrane metabolism. Thus, the measurement of concentrations of these basement membrane components in serum may be a useful tool for monitoring the development of chronic diabetic complications.

    Description / Table of Contents: Summary In 5 mg of liver tissue obtained by needle biopsy from patients with chronic liver diseases, activity of both Procollagen-prolyl-hydroxylase and N-Acetyl-β-glucosaminidase was measured. The activity of both enzymes increased due to the activity of fibrosis, but prolyhydroxylase increased more. The relation of these two enzyme activities in the development of liver fibrosis is discussed.
    Notes: Zusammenfassung In 5 mg Lebergewebe, gewonnen durch Punktion von Patienten mit chronischen Leberkrankheiten, wurde die Aktivität der Prokollagen-Prolyl-Hydroxylase und der N-Acetyl-β-Glucosaminidase bestimmt. Die Aktivität beider Enzyme ist parallel zum histologisch bestimmten Aktivitätsgrad der Fibrose erhöht. In Relation zur N-Acetyl-β-Glucosaminidase steigt die Aktivität der Prolylhydroxylase stärker an. Die Relation beider Enzymaktivitäten in Zusammenhang mit der Entwicklung einer Leberfibrose wird besprochen.
