    Description / Table of Contents: Zusammenfassung Es wird ein Patient beschrieben, der hypokaliaemische Muskelnekrosen, ausgelöst durch einen exogenen Überschuß von Mineralcorticoiden, aufweist. Die Ursache war der langdauernde Gebrauch eines 9-alpha-fluorprednisolon-Nasensprays, ein Steroid mit einer starken mineralcorticoiden Aktivität. Intranasal verabreichte Steroide müssen in der Differentialdiagnose von Hypokaliaemie und dadurch hervorgerufenen Muskelveränderungen in Betracht gezogen werden.
    Notes: Summary A patient is described, who had muscle necrosis occurring with hypokalemia and a factitious mineralocorticoid excess syndrome. The syndrome was due to the prolonged use of a nasal spray containing 9-alpha-fluoroprednisolone acetate, a steroid with potent mineralocorticoid activity. It is emphasized that intranasal administration of steroid compounds must be considered in the differential diagnosis of hypokalemia and related muscle disorders.

    Notes: Summary A patient, suffering from an oat-cell bronchial carcinoma, presented with complex partial seizures, complete loss of recent memory, mild disorientation and confabulation. There was no complaint of anxiety. The rest of the neurological examination and four computed tomographic scans of the head were normal. Repeated EEG recordings were abnormal. Antiepileptic and antipsychotic treatment led to full remission within 10 days. Post-mortem examination of the brain revealed no pathological changes.

    Description / Table of Contents: Sommario I potenziali evocati acustici del tronco cerebrale (BAEP) sono stati registrati in 55 pazienti affetti da sindrome soggettiva postcraniotraumatica (PCS), per evidenziare un possibile danno organico subclinico del tronco cerebrale. Quindici pazienti (27.3%) hanno mostrato risposte alterate unilateralmente o bilateralmente, soprattutto a causa della presenza di una o più latenze interpicchi oltre i limiti fiduciali superiori del 99%. Altri 9 pazienti hanno mostrato risposte borderline. Non sono state ritrovate correlazioni tra il reperto di BAEP alterati e la presenza di vertigini all'atto dell'esame, né con il comportamento dell'esame vestibolare. Benché le alterazioni del BAEP si possano osservare anche a distanza di molti mesi dal trauma, la maggioranza dei pazienti con BA EP alterati, ad un secondo esame ha mostrato un miglioramento della risposta. Tali dati dimostrano che i BAEP sono in grado, in alcuni pazienti affetti da PCS, di evidenziare un danno organico del tronco cerebrale, passibile di regressione.
    Notes: Abstract Brainstem auditory evoked potentials (BAEPs) were recorded from 55 patients with postconcussion syndrome (PCS) to elicit evidence of an organic and subclinical brainstem disorder. Fifteen patients (27.3%) showed abnormal responses unilaterally or bilaterally, especially for one or more interpeak latencies prolonged beyond the upper 99% confidence limits. Other 9 patients had borderline responses. The BAEP alterations were not correlated either with dizziness at the time of recording, or with vestibular troubles in the routine caloric test. Though BAEP abnormalities may be present a long time after injury, we found an improvement of responses in the majority of 14 re-tested patients. These data show that BAEP can give an objective demonstration of a reversible brainstem disorder in patients with PCS.

    Description / Table of Contents: Sommario I principali parametri farmacocinetici periferici della associazione levodopa/carbidopa sono stati studiati in 11 volontari sani ed in 16 pazienti affetti da morbo di Parkinson in differenti stadi di malattia, con e senza fluttuazioni delle performances motorie. Dopo somministrazione per via orale di una dose standard del farmaco (levodopa 250 mg., carbidopa 25 mg.), i valori della concentrazione plasmatica massima, del tempo di comparsa del picco e dell'area sotto la curva concentrazione/tempo sono risultati simili in tutti i gruppi esaminati. La farmacocinetica periferica dell'associazione levodopa/carbidopa non è risultata essere differente nei parkinsoniani che presentavano delle fluttuazioni delle performances motorie in confronto ai pazienti con stabile risposta clinica alla terapia.
    Notes: Abstract The principal peripheral pharmacokinetic parameters of the levodopa/carbidopa association were investigated in 11 healthy volunteers and in 16 patients at various stages of Parkinson disease, with and without the on-off phenomenon. After oral administration of a standard dose of drug (levodopa 250 mg + carbidopa 25 mg) the peak plasma concentrations, peak onset time and area under the curve/time proved to be similar across the groups. There was no difference in peripheral pharmacokinetics of the association between parkinsonian patients with swings in response and those without.

    Description / Table of Contents: Sommario Sono stati esaminati 30 pazienti affetti da morbo di Parkinson idiopatico, di cui 13 maschi e 17 femmine, di età compresa tra 50 edi i 70 anni, in terapia stabilizzata con L-Dopa o con Bromocriptina o con entrambe. In tutti i soggetti, questi farmaci non consentivano un adeguato controllo della sintomatologia, con particolare riferimento al tremore. Lasciando invariata la terapia in atto, si è somministrato Bornaprine od und placebo identico, secondo una sequenza randomizzata, in doppio cieco con cross-over. I pazienti sono stati esaminati per mezzo della Webster Rating Scale a tempi predeterminati, prima, durante e dopo ciascuna fase del trattamento con farmaco o con placebo. L'analisi statistica dei risultati ha dimostrato la significativa superiorità del Bornaprine rispetto al placebo nel ridurre il tremore (p〈0.01) e, in minor misura, alcuni altri sintomi della serie parkinsoniana. Non si sono rilevati effetti collaterali di rilievo, sia dal punto di vista clinico sia da quello strumentale. L'unica eccezione è rappresentata dalla maggiore incidenza di xerostomia nel corso del trattamento con farmaco attivo rispetto a quello con placebo.
    Notes: Abstract The study covers 30 patients with idiopathic Parkinson disease, 13 men and 17 women, aged betwen 50 and 70, on stabilized L-Dopa and/or bromocriptine, which failed to ensure adequate control of the symptoms, especially tremor. To this regimen was added Bornaprine/placebo in randomized sequence. The patients were tested according to the Webster Rating Scale before, during and after each stage of the treatment. Statistical analysis of the results showed the superiority of Bornaprine over the placebo in reducing tremor (p〈0.01) and, to a lesser degree, some other parkinsonian symptoms. No noteworthy side effects were found apart from dryness of the mouth, which was more frequent with Bornaprine.
