    Description / Table of Contents: Zusammenfassung In einer jugendlichen schweizerischen Bevölkerungsgruppe wurde die Häufigkeit eines klinisch manifesten und latenten Diabetes mellitus untersucht. Diese betrug 0.06 resp. 1,7%. Die Problematik der Diabetes-Prävalenz in einer definierten Population zeigt sich nicht nur in der Anwendung bestimmter Definitions-Kriterien, sondern auch in der angewandten Methodologie. Es werden die Abweichungen der mittleren Blutzuckerwerte bei simultaner Bestimmung nach sieben verschiedenen Methoden angeführt. Es wird auf fehlende Abhängigkeiten von Alter, Gewicht, Körperoberfläche und biochemischen Parametern (Harnstoff-N, Eiweiß und Cholesterin) sowie vorgängiger Kohlenhydratzufuhr auf den Blutzuckerwert hingewiesen, wenn enzymatische Glucosebestimmungen durchgeführt werden.
    Notes: Summary The prevalence of juvenile diabetes has been examined in a sample of a Swiss population. There was an estimate of 0.06% of clinical and of 1.7% latent diabetes in a group of males of age 20. The problems of assessing diabetes prevalence are discussed in relation to criteria of definition and blood sugar methodology. The differences of seven simultaneous determinations of blood glucose are presented. There was no dependance of fasting blood glucose values in a hospital population in relation to age, weight, body surface and chemical parameters as BUN, total proteins, cholesterol, and the previous intake of carbohydrates as long as an enzymatic glucose determination was performed.

    Notes: Abstract Background: In February 1993 a prospective randomized multicenter trial was initiated to compare laparoscopic transabdominal preperitoneal hernioplasty to Shouldice herniorrhaphy as performed by surgeons of nonspecialized clinics. Methods: Until January 1994, 87 patients with 108 hernias took part in the trial (43 Shouldice and 44 laparoscopic repairs). Results: The laparoscopic procedure took significantly longer than did the open operation but caused less pain as measured by pain analogue score and consumption of paracetamol and narcotics. The postoperative complication rate was 26% in the open and 16% in the laparoscopic group. The patients in the laparoscopic group were discharged earlier and their convalescence was shorter than after open hernia repair. There has been one early recurrence in the laparoscopic and two in the open group to date with a mean follow-up of 201 days. Conclusions: Laparoscopic hernia repair causes less pain than the conventional operation and enables the patient to return to full work and usual activities earlier. The recurrence rate will not be known for 5 years.

    Notes: Summary It is known that the exceedance points of a high level by a stationary sequence are asymptotically Poisson as the level increases, under appropriate long range and local dependence conditions. When the local dependence conditions are relaxed, clustering of exceedances may occur, based on Poisson positions for the clusters. In this paper a detailed analysis of the exceedance point process is given, and shows that, under wide conditions, any limiting point process for exceedances is necessarily compound Poisson. More generally the possible random measure limits for normalized exceedance point processes are characterized. Sufficient conditions are also given for the existence of a point process limit. The limiting distributions of extreme order statistics are derived as corollaries.
